Pediatric age estimation from thoracic and abdominal CT scout views using deep learning.

ABSTRACT: Age assessment is regularly used in clinical routine by pediatric endocrinologists to determine the physical development or maturity of children and adolescents. Our study investigates whether age assessment can be performed using CT scout views from thoracic and abdominal CT scans using a deep neural network. Hence, we retrospectively collected 1949 CT scout views from pediatric patients (acquired between January 2013 and December 2018) to train a deep neural network to predict the chronological age from CT scout views. The network was then evaluated on an independent test set of 502 CT scout views (acquired between January 2019 and July 2020). The trained model showed a mean absolute error of 1.18 ± 1.14 years on the test data set. A one-sided t-test to determine whether the difference between the predicted and actual chronological age was less than 2.0 years was statistically highly significant (p < 0.001). In addition, the correlation coefficient was very high (R = 0.97). In conclusion, the chronological age of pediatric patients can be assessed with high accuracy from CT scout views using a deep neural network.

Project description:PurposeTo automate skeletal muscle segmentation in a pediatric population using convolutional neural networks that identify and segment the L3 level at CT.Materials and methodsIn this retrospective study, two sets of U-Net-based models were developed to identify the L3 level in the sagittal plane and segment the skeletal muscle from the corresponding axial image. For model development, 370 patients (sampled uniformly across age group from 0 to 18 years and including both sexes) were selected between January 2009 and January 2019, and ground truth L3 location and skeletal muscle segmentation were manually defined. Twenty percent (74 of 370) of the examinations were reserved for testing the L3 locator and muscle segmentation, while the remaining were used for training. For the L3 locator models, maximum intensity projections (MIPs) from a fixed number of central sections of sagittal reformats (either 12 or 18 sections) were used as input with or without transfer learning using an L3 localizer trained on an external dataset (four models total). For the skeletal muscle segmentation models, two loss functions (weighted Dice similarity coefficient [DSC] and binary cross-entropy) were used on models trained with or without data augmentation (four models total). Outputs from each model were compared with ground truth, and the mean relative error and DSC from each of the models were compared with one another.ResultsL3 section detection trained with an 18-section MIP model with transfer learning had a mean error of 3.23 mm ± 2.61 standard deviation, which was within the reconstructed image thickness (3 or 5 mm). Project description:Background Assessment of liver lesions is constrained as CT radiation doses are lowered; evidence suggests deep learning reconstructions mitigate such effects. Purpose To evaluate liver metastases and image quality between reduced-dose deep learning image reconstruction (DLIR) and standard-dose filtered back projection (FBP) contrast-enhanced abdominal CT. Materials and Methods In this prospective Health Insurance Portability and Accountability Act-compliant study (September 2019 through April 2021), participants with biopsy-proven colorectal cancer and liver metastases at baseline CT underwent standard-dose and reduced-dose portal venous abdominal CT in the same breath hold. Three radiologists detected and characterized lesions at standard-dose FBP and reduced-dose DLIR, reported confidence, and scored image quality. Contrast-to-noise ratios for liver metastases were recorded. Summary statistics were reported, and a generalized linear mixed model was used. Results Fifty-one participants (mean age ± standard deviation, 57 years ± 13; 31 men) were evaluated. The mean volume CT dose index was 65.1% lower with reduced-dose CT (12.2 mGy) than with standard-dose CT (34.9 mGy). A total of 161 lesions (127 metastases, 34 benign lesions) with a mean size of 0.7 cm ± 0.3 were identified. Subjective image quality of reduced-dose DLIR was superior to that of standard-dose FBP (P < .001). The mean contrast-to-noise ratio for liver metastases of reduced-dose DLIR (3.9 ± 1.7) was higher than that of standard-dose FBP (3.5 ± 1.4) (P < .001). Differences in detection were identified only for lesions 0.5 cm or smaller: 63 of 65 lesions detected with standard-dose FBP (96.9%; 95% CI: 89.3, 99.6) and 47 lesions with reduced-dose DLIR (72.3%; 95% CI: 59.8, 82.7). Lesion accuracy with standard-dose FBP and reduced-dose DLIR was 80.1% (95% CI: 73.1, 86.0; 129 of 161 lesions) and 67.1% (95% CI: 59.3, 74.3; 108 of 161 lesions), respectively (P = .01). Lower lesion confidence was reported with a reduced dose (P < .001). Conclusion Deep learning image reconstruction (DLIR) improved CT image quality at 65% radiation dose reduction while preserving detection of liver lesions larger than 0.5 cm. Reduced-dose DLIR demonstrated overall inferior characterization of liver lesions and reader confidence. Clinical trial registration no.
