Project description:Traumatic brain injury (TBI) is the principal cause of invalidity and death in the population under 45 years of age worldwide. This mini-review aims to systematize the forensic approach in neuropathological studies, highlighting the proper elements to be noted during external, radiological, autoptical, and histological examinations with particular attention paid to immunohistochemistry and molecular biology. In the light of the results of this mini-review, an accurate forensic approach can be considered mandatory in the examination of suspected TBI with medico-legal importance, in order to gather all the possible evidence to corroborate the diagnosis of a lesion that may have caused, or contributed to, death. From this point of view, only the use of an evidence-based protocol can reach a suitable diagnosis, especially in those cases in which there are other neuropathological conditions (ischemia, neurodegeneration, neuro-inflammation, dementia) that may have played a role in death. This is even more relevant when corpses, in an advanced state of decomposition, are studied, where the radiological, macroscopic and histological analyses fail to give meaningful answers. In these cases, immune-histochemical and molecular biology diagnostics are of fundamental importance and a forensic neuropathologist has to know them. Particularly, MiRNAs are promising biomarkers for TBI both for brain damage identification and for medico-legal aspects, even if further investigations are required to validate the first experimental studies. In the same way, the genetic substrate should be examined during any forensic examination, considering its importance in the outcome of TBI.

Project description:BackgroundCognitive dysfunction after traumatic brain injury (TBI) significantly reduces quality of life and imposes a heavy burden on society. A detailed examination of research trends of cognitive dysfunction following TBI has not yet been conducted. This study aimed to examine the bibliometric analysis of cognitive dysfunction after traumatic brain injury over the past 20 years.MethodsLiterature on bibliometric analysis was retrieved from the Web of Science Core Collection (WoSCC) and Science Citation Index Expanded (SCI-E) from 2004 to 2023. The type of literature and the language were refined. A total of 1,902 articles were used for bibliometric analysis, including 1,543 (81.1%) original articles and 359 (18.9%) review articles. Data were retrieved on June 5, 2024.ResultsThe publication volume of articles was increasing year by year, with articles published in 537 journals. The Journal of Neurotrauma, with 130 articles, was the most productive and influential journal. The University of California System led in the number of articles published. There were 9,002 authors from 62 countries/regions. The USA and China were the top-ranked countries by article count. Pandharipande PP authored the highly cited article. Pick CG, as the author with the highest h-index. The top three of author keywords were traumatic brain injury, cognitive impairment, and mild traumatic brain injury. The topics of cognitive dysfunction after TBI were ferroptosis, cognitive decline, spinal cord injury, and prognosis.ConclusionOur findings provide valuable insights into cognitive dysfunction following TBI and highlight emerging trends for future research.

Project description:Traumatic brain injury remains a leading cause of death and disability across the globe. Substantial uncertainty in outcome prediction continues to be the rule notwithstanding the existing prediction models. Additionally, despite very promising preclinical data, randomized clinical trials (RCTs) of neuroprotective strategies in moderate and severe TBI have failed to demonstrate significant treatment effects. Better predictive models are needed, as the existing validated ones are more useful in prognosticating poor outcome and do not include biomarkers, genomics, proteonomics, metabolomics, etc. Invasive neuromonitoring long believed to be a "game changer" in the care of TBI patients have shown mixed results, and the level of evidence to support its widespread use remains insufficient. This is due in part to the extremely heterogenous nature of the disease regarding its etiology, pathology and severity. Currently, the diagnosis of traumatic brain injury (TBI) in the acute setting is centered on neurological examination and neuroimaging tools such as CT scanning and MRI, and its treatment has been largely confronted using a "one-size-fits-all" approach, that has left us with many unanswered questions. Precision medicine is an innovative approach for TBI treatment that considers individual variability in genes, environment, and lifestyle and has expanded across the medical fields. In this article, we briefly explore the field of precision medicine in TBI including biomarkers for therapeutic decision-making, multimodal neuromonitoring, and genomics.

Project description:Traumatic brain injury remains a global health crisis that spans all demographics, yet there exist limited treatment options that may effectively curtail its lingering symptoms. Traumatic brain injury pathology entails a progression from primary injury to inflammation-mediated secondary cell death. Sequestering this inflammation as a means of ameliorating the greater symptomology of traumatic brain injury has emerged as an attractive treatment prospect. In this review, we recapitulate and evaluate the important developments relating to regulating traumatic brain injury-induced neuroinflammation, edema, and blood-brain barrier disintegration through pharmacotherapy and stem cell transplants. Although these studies of stand-alone treatments have yielded some positive results, more therapeutic outcomes have been documented from the promising area of combined drug and stem cell therapy. Harnessing the facilitatory properties of certain pharmaceuticals with the anti-inflammatory and regenerative effects of stem cell transplants creates a synergistic effect greater than the sum of its parts. The burgeoning evidence in favor of combined drug and stem cell therapies warrants more elaborate preclinical studies on this topic in order to pave the way for later clinical trials.

Project description:Traumatic brain injury (TBI) is a serious condition in which trauma to the head causes damage to the brain, leading to a disruption in brain function. This is a significant health issue worldwide, with around 69 million people suffering from TBI each year. Immediately following the trauma, damage occurs in the acute phase of injury that leads to the primary outcomes of the TBI. In the hours-to-days that follow, secondary damage can also occur, leading to chronic outcomes. TBIs can range in severity from mild to severe, and can be complicated by the fact that some individuals sustain multiple TBIs, a risk factor for worse long-term outcomes. Although our knowledge about the pathophysiology of TBI has increased in recent years, unfortunately this has not been translated into effective clinical therapies. The U.S. Food and Drug Administration has yet to approve any drugs for the treatment of TBI; current clinical treatment guidelines merely offer supportive care. Outcomes between individuals greatly vary, which makes the treatment for TBI so challenging. A blow of similar force can have only mild, primary outcomes in one individual and yet cause severe, chronic outcomes in another. One of the reasons that have been proposed for this differential response to TBI is the underlying genetic differences across the population. Due to this, many researchers have begun to investigate the possibility of using precision medicine techniques to address TBI treatment. In this review, we will discuss the research detailing the identification of genetic risk factors for worse outcomes after TBI, and the work investigating personalized treatments for these higher-risk individuals. We highlight the need for further research into the identification of higher-risk individuals and the development of personalized therapies for TBI.

Project description:Background Traumatic brain injury (TBI) is a major public health concern with far-reaching consequences on individuals’ lives. Despite the abundance of works published on TBI rehabilitation, few studies have bibliometrically analyzed the published TBI rehabilitation research. This study aims to characterize current international trends and global productivity by analyzing articles on TBI rehabilitation using bibliometric approaches and visualization methods. Methods We conducted a bibliometric analysis of data retrieved and extracted from the Web of Science Core Collection database to examine the evolution and thematic trends in TBI rehabilitation research up until December 31, 2022. The specific characteristics of the research articles on TBI rehabilitation were evaluated, such as publication year, countries/regions, institutions, authors, journals, research fields, references, and keywords. Results Our analysis identified 5,541 research articles on TBI rehabilitation and observed a progressive increase in publications and citations over the years. The United States (US, 2,833, 51.13%), Australia (727, 13.12%), and Canada (525, 9.47%) were the most prolific countries/regions. The University of Washington (226, 4.08%) and Hammond FM (114, 2.06%) were the most productive institution and author, respectively. The top three productive journals were Brain Injury (862; 15.56%), Archives of Physical Medicine and Rehabilitation (630; 11.37%), and Journal of Head Trauma Rehabilitation (405, 7.31%). The most frequent research fields were Rehabilitation, Neurosciences, and Clinical Neurology. Co-citation references primarily addressed “outcome assessment,” “community integration” and “TBI management,” and “injury chronicity” and “sequelae” have gained more attention in recent years. “Mild TBI,” “outcome,” “stroke” and “children” were the commonly used keywords. Additionally, the analysis unveiled emerging research frontiers, including “return to work,” “disorder of consciousness,” “veterans,” “mild TBI,” “pediatric,” “executive function” and “acquired brain injury.” Conclusion This study provides valuable insights into the current state of TBI rehabilitation research, which has experienced a rapid increase in attention and exponential growth in publications and citations in the last three decades. TBI rehabilitation research is characterized by its multi-disciplinary approach, involving fields such as Rehabilitation, Neurosciences, and Clinical Neurology. The analysis revealed emerging research subjects that could inform future research directions.

Project description:Police forensic investigations are not immune to our society's ubiquitous search for better predictive ability. In the particular and very topical case of Traumatic Brain Injury (TBI), police forensic investigations aim at evaluating whether a given impact or assault scenario led to the clinically observed TBI. This question is traditionally answered by means of forensic biomechanics and neurosurgical expertise which cannot provide a fully objective probabilistic measure. To this end, we propose here a numerical framework-based solution coupling biomechanical simulations of a variety of injurious impacts to machine learning training of police reports provided by the UK's Thames Valley Police and the National Crime Agency's National Injury Database. In this approach, the biomechanical predictions of mechanical metrics such as strain and stress distributions are interpreted by the machine learning model by additionally considering assault specific metadata to predict brain injury outcomes. The framework, only taking as input information typically available in police reports, reaches prediction accuracies exceeding 94% for skull fracture, 79% for loss of consciousness and intracranial haemorrhage, and is able to identify the best predictive features for each targeted injury. Overall, the proposed framework offers new avenues for the prediction, directly from police reports, of any TBI related symptom as required by forensic law enforcement investigations.

Project description:Traumatic brain injury (TBI) is the leading cause of disability and death, and the social burden of mortality and morbidity caused by TBI is significant. Under the influence of comprehensive factors, such as social environment, lifestyle, and employment type, the incidence of TBI continues to increase annually. Current pharmacotherapy of TBI mainly focuses on symptomatic supportive treatment, aiming to reduce intracranial pressure, ease pain, alleviate irritability, and fight infection. In this study, we summarized numerous studies covering the use of neuroprotective agents in different animal models and clinical trials after TBI. However, we found that no drug has been approved as specifically effective for the treatment of TBI. Effective therapeutic strategies for TBI remain an urgent need, and attention is turning toward traditional Chinese medicine. We analyzed the reasons why existing high-profile drugs had failed to show clinical benefits and offered our views on the research of traditional herbal medicine for treating TBI.

Project description:BackgroundTraumatic brain injury (TBI) is a brain function injury caused by external mechanical injury. Primary and secondary injuries cause neurological deficits that mature brain tissue cannot repair itself. Stem cells can self-renewal and differentiate, the research of stem cells in the pathogenesis and treatment of TBI has made significant progress in recent years. However, numerous articles must be summarized to analyze hot spots and predict trends. This study aims to provide a panorama of knowledge and research hotspots through bibliometrics.MethodWe searched in the Web of Science Core Collection (WoSCC) database to identify articles pertaining to TBI and stem cells published between 2000 and 2022. Visualization knowledge maps, including co-authorship, co-citation, and co-occurrence analysis were generated by VOSviewer, CiteSpace, and the R package "bibliometrix."ResultsWe retrieved a total of 459 articles from 45 countries. The United States and China contributed the majority of publications. The number of publications related to TBI and stem cells is increasing yearly. Tianjin Medical University was the most prolific institution, and Professor Charles S. Cox, Jr. from the University of Texas Health Science Center at Houston was the most influential author. The Journal of Neurotrauma has published the most research articles on TBI and stem cells. Based on the burst references, "immunomodulation," "TBI," and "cellular therapy" have been regarded as research hotspots in the field. The keywords co-occurrence analysis revealed that "exosomes," "neuroinflammation," and "microglia" were essential research directions in the future.ConclusionResearch on TBI and stem cells has shown a rapid growth trend in recent years. Existing studies mainly focus on the activation mechanism of endogenous neural stem cells and how to make exogenous stem cell therapy more effective. The combination with bioengineering technology is the trend in this field. Topics related to exosomes and immune regulation may be the future focus of TBI and stem cell research.

Project description:BackgroundTraumatic brain injury (TBI) is a catastrophic disease involving complex inflammatory processes. This study aimed to quantitatively analyze and visualize the global research trends on inflammation associated with TBI.MethodsAll publications concerning TBI and inflammation published from 2007 to 2021 were retrieved from the Web of Science Core Collection database. Key visualization and statistical analysis were calculated and evaluated using VOSviewer, CiteSpace, R package "bibliometrix," and an online bibliometric analysis platform.ResultsFrom 2007 to 2021, 15,138 authors from 2860 institutions in 77 countries/regions published 3154 articles on inflammation associated with TBI in 786 academic journals. The research output has significantly increased over the years despite a minor fluctuation. Among the countries, the United States showed the highest output (43.50%) with the most total citations (62,791). The author with the most published articles was Cox CS (30 articles with h-index = 20), and the most popular journal in the field was the Journal of Neurotrauma (190 papers, cited 6433 times). The high-frequency keywords were "post-traumatic brain injury," "brain edema," and "glial activation." Moreover, high-frequency keywords analysis indicated that various inflammatory cells contributed to neuroinflammation, neuroprotection, and oxidative stress after TBI.ConclusionThis study revealed the research trends, hotspots, and emerging topics in inflammation associated with TBI by quantitative and visualized analysis. The current research focuses on the crosstalk between various inflammatory cells and the brain and the associated mechanisms. This study presents the research landscape and inspires future research on inflammation associated with TBI.
