Meta-analysis of job-exposure matrix data from multiple sources.

ABSTRACT: The objectives of this study were to determine the heterogeneity of data sources used to construct a job-exposure matrix (JEM) for occupational noise, and to calculate pooled exposure estimates for different job titles using different sources. The JEM was populated with measurements from government databases, private industry, and the published literature. Data were organized by job title using the US standard occupational classification (SOC). Using data from the literature as prior information, adjusted mean exposure was calculated for both the government and industry data following a simple Bayesian approach. A meta-analysis was conducted to measure data heterogeneity across sources and to calculate a pooled exposure estimate for each SOC and SOC group. In total, 715,867 measurements across 259 SOCs were analyzed. Using the data from literature as a prior, 15 of 28 applicable SOCs in the government and industry data had adjusted mean exposures above the OSHA action level (85 dBA). The meta-analysis showed that 63% of SOCs, and 78% of SOC groups, had moderate to high heterogeneity. Fifty-one percent of SOCs and 43% of SOC groups had pooled estimated exposures >85 dBA. The pooled estimates suggested that workers in 131 of 259 SOCs (51%) were exposed beyond the threshold of 85 dBA.

Project description:ObjectivesThe use of a job-exposure matrix (JEM) to assess exposure to potential health hazards in occupational epidemiological studies requires coding each participant's job history to a standard occupation and/or industry classification system recognized by the JEM. The objectives of this study were to assess the impact of inter-coder variability in job coding on reliability in exposure estimates derived from linking the job codes to the Canadian job-exposure matrix (CANJEM) and to identify influent parameters.MethodTwo trained coders independently coded 1000 jobs sampled from a population-based case-control study to the ISCO-1968 occupation classification at the five-digit resolution level, of which 859 could be linked to CANJEM using both assigned codes. Each of the two sets of codes was separately linked to CANJEM and thereby generated, for each of the 258 occupational agents available in CANJEM, two exposure estimates: exposure status (yes/no) and intensity of exposure (low, medium, and high) for exposed jobs only. Then, inter-rater reliability (IRR) was computed (i) after stratifying agents in 4 classes depending, for each, on the proportion of occupation codes in CANJEM defined as 'exposed' and (ii) for two additional scenarios restricted to jobs coded differently: the first one using experts' codes, the other one using codes randomly selected. IRR was computed using Cohen's kappa, PABAK and Gwet's AC1 index for exposure status, and weighted kappa and Gwet's AC2 for exposure intensity.ResultsAcross all agents and based on all jobs, median (Q1, Q3; Nagents) values were 0.68 (0.59, 0.75; 220) for kappa, 0.99 (0.95, 1.00; 258) for PABAK, and 0.99 (0.97, 1.00; 258) for AC1. For the additional scenarios, median kappa was 0.28 (0.00, 0.45; 209) and -0.01 (-0.02, 00; 233) restricted to jobs coded differently using experts' and random codes, respectively. A similar decreasing pattern was observed for PABAK and AC1 albeit with higher absolute values. Median kappa remained stable across exposure prevalence classes but was more variable for low prevalent agents. PABAK and AC1 decreased with increasing prevalence. Considering exposure intensity and all exposed jobs, median values were 0.79 (0.68, 0.91; 96) for weighted kappa, and 0.95 (0.89, 0.99; 102) for AC2. For the additional scenarios, median kappa was, respectively, 0.28 (-0.04, 0.42) and -0.05 (-0.18, 0.09) restricted to jobs coded differently using experts' and random codes, with a similar though attenuated pattern for AC2.ConclusionDespite reassuring overall reliability results, our study clearly demonstrated the loss of information associated with jobs coded differently. Especially, in cases of low exposure prevalence, efforts should be made to reliably code potentially exposed jobs.

Project description:ObjectivesThe aim of this study was to create a quantitative job-exposure matrix (JEM) for noise including a large set of measurements for the Swedish workforce, a detailed exposure-level assessment, spanning over an extensive time period from 1970 to 2014.MethodsThe JEM was developed by 2 teams, each with an experienced occupational hygienist and an occupational safety engineer. Each pair assessed the exposure using measurements performed and reported by occupational hygienists, occupational safety engineers, or similar, from 1970 to 2014. The measurements included either the original LAeq(8h) measurements or an LAeq(8h) levels calculated from partial measurements of the working day, provided that the measurement targeted a regular task usually performed during a full workday. The collection of measurement reports was done in 2008 and 2012 by contacting clinics working in the area of occupational health or occupational safety engineers and their submitted reports were added to our own material. Noise exposure assessments were inserted at the appropriate time period for the relevant job family. The final matrix was developed in a consensus procedure and the validity was investigated by comparison of the 2 team's individual results.ResultsThe noise JEM contains 321 job families with information regarding occupational noise from 1970 to 2014. The time-period label has a 5-yr scale starting in 1970. The estimated average 8 h (TWA) noise level in decibels [dB(A)] for every job family and 5-yr period was coded as 1: <70 dB(A), 2: 70 to 74 dB(A), 3: 75 to 79 dB(A), 4: 80 to 84 dB(A) or 5: 85(+) dB(A). The validation showed no systematic difference in relative position and very high agreement in the ordering of paired ordinal classifications. The JEM has also successfully been applied in several epidemiological studies.ConclusionsWe present a JEM for occupational noise using Swedish data from 1970 to 2014 with a higher degree of sensitivity in assessed noise exposure compared with the previously existing version. Repeated application of the JEM, in epidemiological studies, has shown consistent results and contributed to yielding important findings.
