ABSTRACT:

Objective

Household food insecurity is associated with CVD risk factors in low-income adults, but research on these associations among adolescents is inconsistent. This study investigates whether household and child food insecurity is associated with CVD risk factors in lower-income adolescents.

Design

Cross-sectional. Multivariable linear regression assessed the association between household and child food security and CVD risk factors. Household and child food security was measured using the US Food Security Survey Module. The analyses were adjusted for adolescent's age, sex, race/ethnicity, smoking status, physical activity and sedentary time, as well as household income and the head-of-household's education and marital status.

Setting

The USA.

Participants

The sample was comprised of 2876 adolescents, aged 12-17 years, with household incomes at or below 300 % federal poverty line from the National Health and Nutrition Examination Survey cycles 2007-2016.

Results

The weighted prevalence of household food insecurity in the analytic sample was 33·4 %, and the weighted prevalence of child food insecurity was 17·4 %. After multivariable adjustment, there were no significant associations between household and child food insecurity and BMI-for-age Z-score, systolic and diastolic blood pressure, HDL-cholesterol, total cholesterol, fasting TAG, fasting LDL-cholesterol and fasting plasma glucose.

Conclusions

Despite observed associations in adults, household food insecurity was not associated with CVD risk factors in a national sample of lower-income adolescents. Child food insecurity was also not associated with CVD risk factors. More research should be conducted to confirm these associations.

Project description:BackgroundFood insecurity is associated with poorer nutrient intakes from food sources and lower dietary supplement use. However, its association with total usual nutrient intakes, inclusive of dietary supplements, and biomarkers of nutritional status among US children remains unknown.ObjectiveThe objective was to assess total usual nutrient intakes, Healthy Eating Index-2015 (HEI-2015) scores, and nutritional biomarkers by food security status, sex, and age among US children.MethodsCross-sectional data from 9147 children aged 1-18 y from the 2011-2016 NHANES were analyzed. Usual energy and total nutrient intakes and HEI-2015 scores were estimated using the National Cancer Institute method from 24-h dietary recalls.ResultsOverall diet quality was poor, and intakes of sodium, added sugars, and saturated fat were higher than recommended limits, regardless of food security status. Food-insecure girls and boys were at higher risk of inadequate intakes for vitamin D and magnesium, and girls also had higher risk for inadequate calcium intakes compared with their food-secure counterparts, when total intakes were examined. Choline intakes of food-insecure children were less likely to meet the adequate intake than those of their food-secure peers. No differences by food security status were noted for folate, vitamin C, iron, zinc, potassium, and sodium intakes. Food-insecure adolescent girls aged 14-18 y were at higher risk of micronutrient inadequacies than any other subgroup, with 92.8% (SE: 3.6%) at risk of inadequate intakes for vitamin D. No differences in biomarkers for vitamin D, folate, iron, and zinc were observed by food security status. The prevalence of iron deficiency was 12.7% in food-secure and 12.0% in food-insecure adolescent girls.ConclusionsFood insecurity was associated with compromised intake of some micronutrients, especially among adolescent girls. These results highlight a need for targeted interventions to improve children's overall diet quality, including the reduction of specific nutrient inadequacies, especially among food-insecure children. Project description:BackgroundPrevalence and factors associated with obstructive and restrictive lung function in people with chronic kidney disease (CKD) are unknown.Study designCross-sectional and longitudinal analyses.Setting & participantsParticipants aged 40 to 79 years from NHANES (National Health and Nutrition Examination Survey) 2007 to 2012 who underwent spirometry testing.PredictorCKD (estimated glomerular filtration rate [eGFR] >15-<60mL/min/1.73m(2) or urinary albumin-creatinine ratio ≥ 30mg/g).OutcomesRestrictive lung function (defined as FEV1/FVC≥0.70 and baseline FVC<80% predicted), obstructive lung function (defined as FEV1/FVC<0.70 based on postbronchodilator spirometric results), and mortality data (available for 2007-2008 and 2009-2010 survey periods).Results7,610 participants (CKD=1,338; non-CKD=6,272) were included. Prevalences of obstructive lung function adjusted to the mean age of 55 years and 50% men in the CKD and non-CKD groups were 15.6% and 13.3%, respectively (P=0.2). Similarly, adjusted prevalences of restrictive lung function in the CKD and non-CKD groups were 9.8% and 6.7%, respectively (P=0.01). Presence of albumin-creatinine ratio ≥ 30mg/g was associated with obstructive (OR, 1.42; 95% CI, 1.07-1.88) and restrictive lung function (OR, 1.43; 95% CI, 1.01-2.03) in the entire study cohort. eGFR<60mL/min/1.73m(2) was associated with higher odds of obstructive lung function. Project description:Hysterectomy is the most common type of gynecological operation in the United States. However, complications can occur during or after the operation. Some studies suggest that hysterectomy may increase the risk of stroke. However, other studies have found different conclusions on this matter. This inconsistent conclusion may be due to small sample sizes or limited covariates. So, we sought to further investigate the correlation between hysterectomy and stroke. Our analysis was based on the data from 2007-2018 National Health and Nutrition Examination Survey (NHANES). We excluded participants with missing hysterectomy data (Question "Have you had a hysterectomy, including a partial hysterectomy, that is, surgery to remove {your/her} uterus or womb?"), participants with missing stroke data (Question "Has a doctor or other health professional ever told you that you had a stroke?), a total of 15,241 participants were included in our analysis. To estimate the correlation between hysterectomy and stroke, logistic regression models were used after adjusting for sociodemographic and health-related factors, including age, race, education level, marital status, annual family income, body mass index (BMI), alcohol consumption in the past 12 months, having smoked at least 100 cigarettes in a lifetime, hypertension, hypercholesterolemia, and diabetes. The unadjusted model suggests that women who had undergone a hysterectomy were 3.15 [95% confidence interval (CI): 2.67-3.71] times more likely to have a stroke than women who had not undergone a hysterectomy. In the crude and fully-adjusted models, the correlation between hysterectomy and stroke was consistent [odds ratio (OR)crude-adjusted =1.55 (95% CI: 1.30-1.85), ORfully-adjusted =1.36 (95% CI: 1.14-1.63)]. In the subgroup analysis stratified by age, hysterectomy seemed to have more risk for stroke occurrence regardless of subgroup, even after adjusting sociodemographic and health-related factors. Interestingly, the women who were less than or equal to 50 years old had greater odds of stroke (ORfully-adjusted =1.96) compared with women who were aged older than 50 (ORfully-adjusted =1.42). In our study, we concluded hysterectomy may increase the risk of stroke. However, as our study is a cross-sectional study and unmeasured covariates may still exist, more researches are required to confirm this conclusion.

Project description:BACKGROUND:Human papillomavirus (HPV) vaccination has been routinely recommended at age 11-12 years in the United States for females since 2006 and males since 2011. Coverage can be estimated using self/parent-reported HPV vaccination collected in the National Health and Nutrition Examination Survey (NHANES) for a wider age range than other national surveys. We assessed vaccination coverage in 2015-2016, temporal trends by age, and the validity of self/parent-reported vaccination status. METHODS:Participants aged 9-59 years completed an interview collecting demographic and vaccination information. Weighted coverage was estimated for two-year NHANES cycles by age group for 2007-2008 to 2015-2016 for females (N = 14318) and 2011-2012 to 2015-2016 for males (N = 7847). Temporal trends in coverage were assessed from 2007-2008 to 2011-2012 for females and from 2011-2012 to 2015-2016 for both sexes. Sensitivity and specificity of self/parent-reported vaccination were assessed using provider-verified vaccination records from a pilot study in 14-29 year-olds. RESULTS:In 2015-2016, ≥1 dose coverage among females was highest in 14-19 (54.7%) and 20-24 (56.0%) year-olds and lower in successively older age groups. Among males, ≥1 dose coverage was highest in 14-19 year-olds (39.5%) and lower at older ages. Coverage was similar in 9-13 year-old females and males. Between 2007-2008 and 2011-2012, there were increases among females younger than 30 years. Between 2011-2012 and 2015-2016, there were increases among female age groups including 20-39 year-olds; male coverage increased among ages 9-13, 14-19, and 20-24 years. Self/parent-reported receipt of ≥1 dose had a sensitivity and specificity of 87.0% and 83.3%. Performance was lower for 3 doses. CONCLUSIONS:While overall HPV vaccination coverage remains low, it is higher in females than males, except in 9-13 year-olds. There have been increases in coverage among many age groups, but coverage has stalled in younger females. Adequate validity was demonstrated for self/parent-reported vaccination of ≥1 dose, but not 3 doses, in a pilot study.

Project description:BackgroundCluster analyses have proposed different prediabetes phenotypes using glycemic parameters, body fat distribution, liver fat content, and insulin sensitivity. We aimed at classifying the subjects with prediabetes using cluster analysis and exploring the associations between prediabetes clusters with hypertension and kidney function.MethodsPatients with prediabetes in the National Health and Nutrition Examination Survey (NHANES) underwent comprehensive phenotyping and physical and laboratory variable assessment. We identified six clusters using consensus clustering analysis based on the measurements representing the body fat, glycemic status, pancreatic islet function, blood lipids, and liver function. Differences in the characteristics and prevalence of hypertension, decreased estimated glomerular filtration rate (eGFR), and increased albumin-to-creatinine ratio (ACR) were compared between clusters.ResultsA total of 4,385 subjects with prediabetes were classified into six clusters of distinctive patterns by manifesting higher or lower levels of certain metabolic parameters in each cluster. Subjects with prediabetes in cluster 1 had the lowest prevalence of hypertension, decreased eGFR, and increased ACR, whereas these were much higher in cluster 5 and cluster 6. Except for cluster 3, all the other clusters had significantly increased odds ratio (OR) of hypertension as compared with cluster 1. Compared with cluster 1, all the other clusters presented significantly increased ORs of decreased eGFR. There were also significantly higher ORs of increased ACR for cluster 5 (OR 1.95, 95% confidence interval [CI] 1.09-3.51) and cluster 6 (OR 2.02, 95%CI = 1.15-3.52) compared with cluster 1.ConclusionWe stratified subjects with prediabetes into six subgroups with different characteristics. With further development and validation, such approaches might guide early intervention on the risk factors for the subjects with prediabetes who would benefit most.
