Project description:Background: In EDTA-induced pseudothrombopenia, citrate or MgSO4 are recommended for platelet counting. Pre-analytical conditions are poorly defined for tubes containing MgSO4 or citrate. In this study, we analyzed the impact of agitation of these tubes on platelet counts. Methods: K2EDTA, citrate, and MgSO4 tubes from 70 patients were gently agitated on a wheel rotating at 20 rpm. Platelets were analyzed on the Sysmex XN analyzer at different times, and the percentage of platelet deviation from T0 was assessed and compared with the desirable bias of the EFLM. Results: at 180 min in fluorescence, the relative variation of platelets after shaking is 1.17% for K2EDTA, -29.76% for citrate, and -33.18% for MgSO4, while for unshaken MgSO4 platelets the variation is -1.3%. The reduction in platelet numbers when citrate or MgSO4 tubes are shaken is linked to the appearance of platelet clusters. Conclusions: agitation of MgSO4 and especially citrate tubes led to a decrease in platelet counts due to the formation of platelet aggregates; on the other hand, platelet counts on EDTA are virtually stable. During transport, we recommend putting sodium citrate and MgSO4 tubes in an upright position and avoiding shaking them to avoid giving an erroneous platelet result.

Project description:Platelets primarily participate in hemostasis and antimicrobial host defense. The present study aimed to investigate the effects of leucine-rich repeat flightless-interacting protein-1 (LRRFIP1) on platelet agglutination. The bacterial strain of LRRFIP1 was used to synthesize the recombinant protein and a mouse model of LRRFIP1 gene knockout was established. Platelets were isolated from the mice and divided into the different trial groups according to their treatment with collagen, thrombin receptor SFLLRN, anti-wild-type (w)LRRFIP1monoclonal antibodies and the model of LRRFIP1 gene knockout. The platelets were prepared and platelet agglutination was examined using platelet aggregation apparatus. The active αIIbβ3 integrin was examined by flow cytometry. The results revealed that the combined wLRRFIP1 protein was successfully expressed. wLRRFIP1 treatment significantly triggered platelet agglutination of collagen, thrombin and monoclonal antibody treated platelets. wLRRFIP1 knockout significantly decreased αIIbβ3 levels compared with the wild-type. Platelet agglutination was also significantly inhibited in the LRRFIP1-/-mouse model compared with the wild-type. LRRFIP1 knockout significantly decreased the αIIbβ3 levels in platelets undergoing convulxin treatment. In conclusion, LRRFIP1 treatment triggered platelet agglutination and LRRFIP1 gene knockout inhibited platelet agglutination. In addition, LRRFIP1 gene knockout significantly decreased the levels of αIIbβ3. This suggests that LRRFIP1 my be applied to patients in a clinical setting to trigger platelet agglutination in inflammatory diseases and atherothrombotic diseases.

Project description:AbstractClinical features in patients with the B-cell lymphoma, Waldenström macroglobulinemia (WM), include cytopenias, immunoglobulin M (IgM)-mediated hyperviscosity, fatigue, bleeding, and bruising. Therapeutics such as Bruton's tyrosine kinase inhibitors (BTKis) exacerbate bleeding risk. Abnormal hemostasis arising from platelet dysfunction, altered coagulation or vascular impairment have not yet been investigated in patients with WM. Therefore, this study aimed to evaluate hemostatic dysfunction in samples from these patients. Whole blood (WB) samples were collected from 14 patients with WM not receiving therapy, 5 patients receiving BTKis and 15 healthy donors (HDs). Platelet receptor levels and reticulation were measured by flow cytometry, plasma thrombin generation with or without platelets by fluorescence resonance energy transfer assay, WB clotting potential by rotational thromboelastometry, and plasma soluble glycoprotein VI (sGPVI) and serum thrombopoietin (TPO) by enzyme-linked immunosorbent assay. Donor platelet spreading, aggregation, and ability to accelerate thrombin generation in the presence of WM-derived IgM were assessed. WM platelet receptor levels, responses to physiological agonists, and plasma sGPVI were within normal ranges. WM platelets had reduced reticulation (P = .0012) whereas serum TPO levels were increased (P = .0040). WM plasma displayed slower thrombin generation (P = .0080) and WM platelets contributed less to endogenous thrombin potential (ETP; P = .0312). HD plasma or platelets incubated with IgM (50-60 mg/mL) displayed reduced spreading (P = .0002), aggregation (P < .0001), and ETP (P = .0081). Thus, alterations to thrombin potential and WB coagulation were detected in WM samples. WM IgM significantly impaired hemostasis in vitro. Platelet and coagulation properties are disturbed in patients with well-managed WM.

Project description:BackgroundMycoplasma hominis infection is common in urinary tract. 18F-FDG-PET/CT is a valuable tool for tumor and infection diagnosis. Few studies have shown the 18F-FDG-PET/CT images after mycoplasma infection.Case presentationHere we described a case of Waldenstrom macroglobulinemia with thickened bladder wall. The 18F-FDG-PET/CT showed the SUVmax up to 36.1 mimicking bladder cancer. The results of histopathological examination and metagenomic sequencing of the blood and urinary revealed the Mycoplasma hominis infection.ConclusionThe full consideration should be given to the possibility of infection besides tumor in lesions with high SUV value in 18F-FDG-PET/CT, especially in immunodeficiency patients.

Project description:BackgroundImmature platelets (IP) are the youngest circulating platelets, released from megakaryocytes, and demonstrating increased dimensions, significant RNA content, and enhanced activity. Immature platelet research focuses on a differential diagnostic help in patients with thrombocytopenia. The objectives of this study were to compare the variability of IP in citrate and EDTA samples, and to determine stability over time.MethodsFifty-six patients were included for comparison between EDTA and citrate whole blood sample collection. Among the patients, 28 had thrombocytopenia (platelet count < 150G/L). Platelet measurement impedancemetry and fluorimetry were performed with Sysmex XN-9000. The immature platelet fraction (IPF) and absolute immature platelet count (A-IPC) were determined with a fluorescent method.ResultsThe mean value of platelet count with fluorescence was, in EDTA sample, 215 ± 171 and, in citrate sample, 153 ± 118 G/L. No significant difference was observed between IPF between EDTA and citrate (7.74 ± 6.68% vs. 8.45 ± 7.37%, p = 0.69), respectively. With the Bland-Altman analysis, the mean difference in the EDTA sample, between 1 and 24 h, was 8.06 ± 6.96% and 8.73 ± 7.12% for IPF, whereas in the citrate sample, between 1 and 6 h, it was 8.60 ± 7.29% and 7.54 ± 6.97%, for IPF. Comparing 1 h EDTA sample with 6 h citrate sample, the variance ratio was 0.974 (95% CI: 0.864-1.084) in IPF.ConclusionsWe confirmed the potential to conduct IP measurements up to 24 h in the EDTA sample and IPF measurements in the citrate sample for up to 6 h. These results may be useful for the use of IPF, which is a promising parameter whose interest in clinical practice and standardization is not yet well defined.

Project description:BackgroundEDTA-dependent pseudothrombocytopenia (EDTA-PTCP) is an in vitro phenomenon that may lead to expensive, time-consuming, and invasive diagnostic procedures as well as unnecessary patient treatment. The purpose of this study was to explore the effects of time, anticoagulant and detection channel on the platelet (PLT) count of EDTA-PTCP samples, and to suggest a better method for correcting spurious low PLT counts.MethodsIn this study, 43 identified EDTA-PTCP samples were collected. The Sysmex XN-9100, Mindray BC-6900 and Mindray BC-5390 haematology analysers were used to test these EDTA-PTCP samples on the following detection channels at different time points: PLT count by impedance method (PLT-I), PLT count by optical method (PLT-O) and PLT count by fluorescent staining (PLT-F).ResultsEDTA-PTCP was time-dependent and small PLT agglutination occurred in most of the corresponding citrate-treated samples. Our results further demonstrated that the detection channel significantly affected the PLT count of the EDTA-PTCP samples. The XN-9100 PLT-F channel exhibited a greater dissociative effect than the XN-9100 PLT-I and PLT-O channels. Moreover, blood samples processed in the PLT-O channel of the Mindray hematology analyzer showed the highest PLT count in EDTA-K2 tubes compared to the other detection channels.ConclusionOur data showed that time, anticoagulant and detection channel significantly affected the PLT count in the EDTA-PTCP samples. For the EDTA-PTCP samples, the simplest retest method was to use the PLT-O channel of the Mindray automatic blood analyser within 30 min. In addition, changing the sodium citrate anticoagulant and using the XN-9100 PLT-F channel within 15 min were also suitable for correcting the spurious low PLT of the EDTA-PTCP samples.

Project description:BackgroundEDTA-induced pseudothrombocytopenia (PTCP) during whole blood collection requires significant laboratory resources to obtain accurate results. We evaluated platelet-deaggregation function in EDTA-induced PTCP and platelet-clump flagging by the BC-6800Plus hematology analyzer using integrated digital image analysis.MethodsWe prospectively collected 132 whole blood samples suspected of platelet clumping (102 in EDTA and 30 in sodium citrate) from 88 individuals. We compared platelet counts determined using the platelet count by impedance (PLT-I) function of the DxH 900 hematology analyzer and the PLT-I or optical platelet count (PLT-O) function of the BC-6800Plus. Platelet clumping was verified through manual inspection and the MC-80 digital image analyzer.ResultsAmong the 132 whole blood samples, 43 EDTA samples showed platelet clumping. The DxH 900 PLT-I and BC-6800Plus PLT-I results demonstrated a strong correlation (r=0.711) for the EDTA samples but only a moderate correlation with the BC-6800Plus PLT-O results (r=0.506 and 0.545, respectively). The BC-6800Plus PLT-O results were consistent with the sodium citrate platelet counts, with a median dissociation rate of 102.5% (range, 74.9%-123.1%). The DxH 900 and BC-6800Plus analyzers had sensitivity values of 0.79 and 0.72, respectively, for platelet-clump flagging. When integrating the MC-80 digital image analysis results, the sensitivity of BC-6800Plus improved to 0.89 (standard mode) or 1.0 (PLT-Pro mode).ConclusionsBC-6800Plus PLT-O measurement results are close to the actual values obtained by platelet deaggregation with PTCP samples. Integrating the BC-6800Plus with a digital imaging analyzer effectively improved the diagnosis of PTCP and reduced the requirement for additional laboratory procedures.
