Project description:Intervention 1: The first intervention group: patients with a serum level of 25-hydroxyvitamin D3 less than 12 ng/ml receive 8 pearls of 50000 units of cholecalciferol from Zahravi Pharmaceutical Company one pearl weekly for 8 week and then 30 Tabs of 1000 units of cholecalciferol from Omid parsina damavand pharmaceuitical company, daily for 24 days. Intervention 2: The second intervention group: patients with a serum level of 25-hydroxyvitamin D3 of 12-20 ng/ml receive a total of 90 Tabs of 1000 units of cholecalciferol from Omid parsina damavand pharmaceuitical company, one pearl daily for 90 days. Intervention 3: The third intervention group: patients with a serum level of 25-hydroxyvitamin D3 of 20-30 ng/ml will receive a total of 60 pearls of 1000 units of cholecalciferol from Omid parsina damavand pharmaceuitical company accoriding to following command: Two days one pearl daily and then one day off for 3 months. Intervention 4: The fourth intervention group: patients with a serum level of 25-hydroxyvitamin D3 above 30 ng/ml will receive a total of 30 pearls of 1000 units of cholecalciferol from Omid parsina damavand pharmaceuitical company, one pearl every 3 days for 3 months.
Primary outcome(s): Depression score in Zung questionnaire. Timepoint: Before starting intervention, 90 days after starting to take cholecalciferol. Method of measurement: Zung Self-Rating Depression Scale questionnaire.;Anxiety score in Zung questionnaire. Timepoint: Before starting intervention, 90 days after starting to take cholecalciferol. Project description:Objective. Follistatin-like protein 1 (FSTL-1) is a secreted glycoprotein that is over-expressed in certain inflammatory diseases. Our objective in this study was to correlate FSTL-1 levels with measures of clinical disease activity in systemic juvenile idiopathic arthritis (sJIA), including macrophage activation syndrome (MAS). Methods. FSTL-1 serum levels were measured by ELISA in 28 patients with sJIA, including 7 patients who developed MAS, as well as in 30 normal controls. Levels were correlated with erythrocyte sedimentation rate (ESR), ferritin and sIL-2Rα expression. Differential gene expression based on FSTL-1 levels was analyzed in peripheral blood mononuclear cells (PBMCs). Results. FSTL-1 serum levels were elevated at time of presentation (pre-treatment) of sJIA (mean 200.7 ng/ml) and decreased to normal (mean 133.7 ng/ml) over 24 months (p<0.01). FSTL-1 levels were markedly elevated during acute MAS (mean 279.8 ng/ml) and decreased to normal (mean 120.3 ng/ml) following treatment (p<0.001). FSTL-1 levels correlated with serum markers of inflammation, including sIL-2Rα and ferritin, in patients with MAS. PBMCs from patients with FSTL-1 levels >200 ng/ml showed an increase in expression of genes related to innate immunity and erythropoiesis, and decrease in NK cell function. Patients with the highest FSTL-1 levels at disease onset (>300 ng/ml) ultimately required cyclosporine treatment. Conclusion. In patients with sJIA, serum FSTL-1 is a biomarker for MAS and FSTL-1 levels at presentation may predict subsequent disease course. Patients with FSTL-1 levels >200 ng/ml had altered gene expression patterns suggestive of subclinical MAS and may represent a subgroup of sJIA with more severe disease.

Project description:Glucocorticoids used in pharmacological doses for the treatment of a variety of medical conditions, and endogenous glucocorticoid excess – Cushing’s syndrome, may result in several adverse effects, but currently there is no clinically useful biomarker of glucocorticoid activity. A three-phase protein biomarker discovery strategy was used. Proteomic biomarker discovery and qualification was conducted on the secretome of ex vivo-stimulated peripheral blood mononuclear cells (PBMC) isolated from 6 volunteers, incubated ± dexamethasone 100 ng/mL for 4h and 24h. Untargeted proteomics with label-free quantification (LFQ) was conducted to discover candidate proteins which were quantified using targeted proteomics by a custom multiple reaction monitoring mass spectrometry (MRM-MS) assay. Five proteins were selected for serum measurement by immunoassay in 20 healthy volunteers, with blood drawn at baseline and 12h after 4 mg oral dexamethasone. Paired analysis of the discovery proteomics data (576 and 280 proteins for the 4h and 24h secretomes, respectively) generated a shortlist of candidates which were qualified using MRM-MS to obtain protein level intensity data for 39 proteins. In the validation cohort, 3/5 proteins were dexamethasone-responsive, two significantly decreased: lysozyme C (mean±SEM) – 101±5.5 vs 67±4.4 ng/mL, (P<0.0001); nucleophosmin-1 (median (interquartile range)) – 16.6 (14.4-18.4) vs 14.2 (11.1-17.4) ng/mL, (P<0.01), while high mobility group box 2 (mean±SEM) significantly increased – 819±34 vs 984±60 pg/mL (P<0.01). Using an ex vivo proteomic approach in PBMC, we have identified and verified circulating glucocorticoid-responsive proteins which may have potential as serum biomarkers.

Serum samples were trypsin-digested and fractionated by SCX chromatography prior to SRM and PRM measurements. PRM data obtained by high-resolution mass spectrometry correlated better with ELISA measurements than SRM data measured on a triple quadrupole mass spectrometer. While all three methods (SRM, PRM, ELISA) were able to quantify HSP90 alpha in serum at the ng/mL level, the use of PRM on a high-resolution mass spectrometer reduced variation. To rule out that the observed differences in SRM and PRM are due to different mass spectrometry systems, the SCX fractions were measured on the same high-resolution instrument (Orbitrap) in the ion trap mode (IT-PRM); such measurements showed that intense co-eluting signals were present in the SRM method, but these interfering peaks were eliminated in the high-resolution PRM mode. Thus, this report shows that it is possible to measure ng/mL levels of HSP90 alpha in a reproducible, selective and sensitive way using PRM. This opens up the possibility to quantify low levels of multiple proteins in complex samples based on a fractionation strategy on tryptic peptides followed by SRM and PRM.
