Immunochemical FOBT screening for colorectal cancer: yield and attendance for one- versus two-sample tests, and for second round screening with various intervals.

ABSTRACT: Interventions: 1-sample or 2-samples immunochemical FOBTs. Firstly, a representative population sample of 2.500 asymptomatic persons between 50-75 years of age in the region ‘Rijnmond’ will be invited for the 1st screening round using 1-sample iFOBT in 2008 and for a 2nd screening round after one year (group A). Secondly, the 5.000 asymptomatic persons who were invited in our first trial for a 1st-round one-sample iFOBT in 2006/2007 (NTR = 1096), will be invited for 2nd round screening. They will be pre-randomized for biennial (group B) or triennial (group C) 2nd round screening. The attendance rate of these two groups will be compared with the participation of the second screening round from group A. Furthermore, 3.200 newly randomly selected persons will be invited for a single 1st round screening using 2-sample iFOBT (group D). The attendance rate of this group will be compared with the participation of the first screening round from group A. Primary outcome(s): Adherence for second round CRC screening with 1-sample iFOBT (OC-Hemodia Latex) comparing annual, biennial, and triennial screening interval. Adherence and diagnostic yield of a first screening round comparing 1-sample versus 2-sample iFOBT screening. Colorectal Cancer Screening in Average-risk Population: Immunochemical Fecal Occult Blood Testing Versus Colonoscopy

Project description:Aims: 1. To compare the efficacy of biennial immunochemical fecal occult blood test (iFOBT) versus colonoscopy every 10 years for the reduction of colorectal cancer-related mortality at 10 years in average-risk population. 2. To determine the compliance and complications associated with both strategies. Methods: Multicenter, randomized, controlled study in 8 Spanish regions (Aragón, Canarias, Catalunya, Euskadi, Galicia, Madrid, Murcia and Valencia). Study groups: * Group I: iFOBT (OC Sensor) in one stool sample, followed by colonoscopy when a positive result. * Group II: colonoscopy. Sample-size calculation: 27,749 subjects in each study group (total: 55,498).

EP001: The interferon transcriptome in uterine NK cells is upregulated during the second trimester of pregnancy allowing extravillous trophoblast invasion

Project description:Uterine Natural Killer (uNK) cells regulate essential developmental processes at the maternal-fetal interface during early pregnancy. Uterine NK cell functions are tightly regulated during early placentation to stimulate trophoblast invasion and remodel uterine spiral arteries. Access to human 1st and 2nd trimester uterine tissues allowed us to investigate the transcriptional changes in uNK cells during the early stages of early human pregnancy. Microarray analysis identified 97 upregulated genes in 2nd compared to 1st trimester purified uNK cells of which the majority (61%) clustered as interferon-stimulated-genes (ISG), with ISG15 and ISG20 being upregulated profoundly. Type I interferons (IFNα/β), but not type II interferon (IFNɣ) increased expression of the identified interferon target genes ISG15 and ISG20 in uNK cells in vitro. Moreover, the cytokine-like protein ISG15 stimulated in vitro trophoblast invasion. Second trimester uNK cells promoted trophoblast invasion in vitro, whereas both 1st and 2nd trimester uNK cells stimulated endothelial tube formation. IFNα but not IFNβ stimulation of 1st trimester uNK cells enhanced their capacity to promote trophoblast invasion. In conclusion, the uNK cell interferon transcriptome is upregulated during the 2nd trimester allowing uNK cells to promote trophoblast invasion. Type I interferon signaling regulates uNK cell-induced trophoblast invasion via induction of effector molecules like ISG15. First trimester uNK cells can be induced to act like second trimester uNK cells by IFNα with respect to promotion of trophoblast invasion. Transcription profiling of human erythroid cell differentiation

Project description:Human mononuclear cells were cultured in 2 phases. In the 1st phase the culture medium contained cyclosporine A the 2nd phase contained SCF and erythropoietin. Cells were collected at 3 stages of differentiation; on day 6, 10, 12 and represented early erythroblasts, medium stage and normoblasts. Experiment Overall Design: RNA was extracted from each sample of cells at a different stage of differentiation and subjected to microarray studies in duplicates.
