Screening and treatment of psychological distress in colorectal cancer with metastasized disease: the TES-trial

ABSTRACT: Interventions: Targeted selection of patients with psychological distress. Enhanced care by the clinical nurse specialist. Stepped care: The steps include: (i) Watchful waiting. (ii) If psychological distress persists, the guided self-help program. (iii) If psychological distress persists, a problem analysis is performed and an agreement is made with the patient on the next step; treatment consists of problem solving therapy (face-to-face). (iv) If psychological distress persists, psychotherapy, medication or a referral for other services (e.g. social work) is offered. The TES intervention is compared to usual care. Single-cell transcriptome profiling implicates the psychological stress-induced disruption of spermatogenesis

Project description:Male infertility has become a global problem. And psychological stress has been proven to be an important cause of male infertility. In this work, we constructed a terrified-sound stress model in SD rats and performed single-cell RNA sequencing of the testicular tissues from the post-stressed and normal rats. Through the comparative analysis of the gene expression patterns of the 11,744 rat testicular cells, we revealed the potential mechanism by which terrified-sound induced psychological stress affects spermatogenesis and ultimately leads to reproductive impairment in male rats. This will provide a theoretical basis for the study of psychological stress on male reproductive impairments for other animals and humans, and further find the therapeutic targets for male infertility caused by psychological stress.

The enteric nervous system relays psychological stress to intestinal inflammation [ScRNASeqCD45seq]

Project description:Mental health profoundly impacts inflammatory responses in the body. This is particularly apparent in inflammatory bowel disease (IBD), where psychological stress is associated with disease flares. Here, we discover a critical role for the enteric nervous system (ENS) in mediating the aggravating effect of chronic stress on intestinal inflammation. We find that chronically elevated levels of glucocorticoids drive the generation of an inflammatory subset of enteric glia that promotes monocyte- and TNF-mediated colitis via CSF1. Additionally, glucocorticoids cause transcriptional immaturity in enteric neurons, acetylcholine deficiency, and dysmotility via TGFβ2. We verify the connection between the psychological state, intestinal inflammation, and dysmotility in two cohorts of human IBD patients. Together, these findings offer a mechanistic explanation for the impact of the brain on peripheral inflammation, define the ENS as a relay between psychological stress and gut inflammation, and suggest that stress management could serve as a valuable component of IBD care.

Project description:Individual differences in basal leukocyte gene expression profiles as a function of hedonic and eudaimonic well-being, and psychological and social sub-dimensions of eudaimonic well-being. Gene expression profiling was carried out on peripheral blood mononuclear cell RNA samples collected from 122 healthy adults measured for hedonic and eudaimonic dimensions of well-being using the Short Flourishing Scale (Keyes, C (2014). Gene expression and response to psychological therapy

Project description:This study represents the first investigation of genome-wide expression profiles with respect to psychological treatment outcome. Participants (n=102) with panic disorder or specific phobia received exposure-based CBT. Treatment outcome was defined as percentage reduction from baseline in clinician-rated severity of their primary anxiety diagnosis at post-treatment and six month follow-up. Gene expression was determined from whole blood samples at 3 time-points using the Illumina HT-12v4 BeadChip microarray. No changes in gene expression were significantly associated with treatment outcomes when correcting for multiple testing (q<0.05), although a small number of genes showed a suggestive association with treatment outcome (q<0.5, n=20). Study reports suggestive evidence for the role of a small number of genes in treatment outcome. Although preliminary, the findings contribute to a growing body of research suggesting that response to psychological therapies may be associated with changes at a biological level.
