Project description:Multiple myeloma (MM) is a hematological cancer which arises from abnormal antibody producing white blood plasma cells. MM affected approximately 500 000 people and results in ~100 000 deaths annually, being currently considered treatable but uncurable. There are several MM chemotherapy treatment regimens, among which ten include bortezomib, a proteasome-targeted drug. Patients with MM respond differently on bortezomib, and new prognostic biomarkers are needed to personalize treatments. However, there is a shortage of clinically annotated molecular profiles of MM that could be used to establish novel molecular diagnostics. In this study we report new molecular profiles for 58 MM patients annotated with data on two chemotherapy regimens which include bortezomib, doxorubicin, and dexamethasone (PAD) or bortezomib, cyclophosphamide, and dexamethasone (VCD). MM biosamples were taken prior to first line chemotherapy treatments and subjected to RNA sequencing.

Project description:Current chemotherapy regimens have significantly improved overall survival for children with cancer. However, these curative treatments are associated with detrimental side effects such as chemotherapy-induced cognitive impairment (CICI), or “chemobrain.” Measurable deficits in cognitive function may persist years after chemotherapy treatments, significantly reducing quality of life. Specifically, doxorubicin (DOXO), a commonly used chemotherapeutic agent in curative regimens for children with cancer, plays a pivotal role in the development of CICI, even though it does not cross the blood brain barrier (BBB). We propose to address the poorly understood mechanism of DOXO-related CICI by studying changes induced by DOXO in BBB integrity in vitro, using human cerebral microvascular endothelial cells (hCMEC/D3). Our findings provide critical insights on how DOXO impacts the BBB and builds a foundation for developing preventative measures and therapeutic interventions that may improve the quality of life for patients.

Project description:n cancer treatment regimens using chemotherapy, adverse effects can outweigh the benefits. Various chemotherapeutic drugs are linked to skeletal muscle wasting, drastically reducing the chance of survivability of cancer patients. Insights into the molecular basis of chemotherapy-induced cachexia is an unmet need to improve the treatment strategies. Here, we investigated tyrosine kinase inhibitor (TKI) class of chemotherapeutic agents for their effects on muscle function. Sorafenib (Sor), but not Nilotinib (Nilo) and Imatinib (Ima), triggered cachexia. Our system-wide analyses revealed that Sor alters the global transcription program and proteostasis in muscle cells. Mechanistically, Sor impeded chromatin association of SET1/MLL histone methyltransferase on distinct muscle-specific genes. Thus, it reduced H3K4 methylation and rendered the chromatin transcriptionally incompetent, as characterized by diminished association of RNA polymerase II. This transcriptional reorientation resulted in disruption of sarcomere organization, drastically perturbed calcium homeostasis, and mitochondrial respiration in muscle cells. Consequently, Sor-treatment severely compromised the contractile ability of muscle cells. Collectively, we identified an unanticipated epigenetic process affected by Sor that led to cachexia. Our findings hold the potential to strategize therapy regimens to minimize chemotherapy-induced muscle wasting and improve treatment outcomes.

Project description:This is a Phase 2, multicenter, multiple-arm, open-label study to evaluate the efficacy, safety, and tolerability of EZN-2208. EZN-2208 will be administered as a single agent in patients with K-RAS mutations in the tumors. Patients with wild type K-RAS in tumors will be randomized to EZN-2208 + cetuximab or to standard of care (Camptosar + cetuximab), patients must have failed regimens containing irinotecan (Camptosar, CPT-11), oxaliplatin (Eloxatin), and fluoropyrimidine.
After discontinuation of study treatment, patients will receive care as considered appropriate by the investigator. Patients will continue to be followed for disease progression, subsequent anticancer therapy, and survival.

Project description:Metastatic pancreatic cancer (mPC) has a dismal prognosis, with first line systemic therapy relying primarily on FOLFIRINOX (5FU/irinotecan/oxaliplatin) or AG (Gemcitabine/Nab-Paclitaxel). Therapeutic options for mPC refractory to these regimens remain poorly-defined, and data on later lines options are scare. This prospective, single-arm study (NCT05481463) evaluated the safety and preliminary efficacy of combining the anti-angiogenic agent surufatinib with the cytotoxic drug TAS-102 (Trifluridine/Tipiracil) in mPC patients who had progressed on ≥ 2 prior lines of therapy. Primary and secondary endpoints included progression-free survival (PFS), overall survival (OS), objective response rate (ORR), disease control rate (DCR), and safety. Among 20 patients analyzed for efficacy, median PFS was 2.35 months (95% CI: 1.91-3.94) and median OS was 6.34 months (95% CI: 3.81-10.09). The ORR and DCR were 20% (4/20; all partial response) and 30% (6/20), respectively. All patients experienced treatment emergent adverse events (TEAEs), with anemia (59.1%), neutropenia (54.6%), leukopenia (50.0%), and lymphopenia (45.5%) as the most common any-grade events. Grade ≥3 TEAEs including neutropenia (31.8%), lymphopenia (13.6%), and anemia (9.1%), were observed in 50.0% (11/22) of patients. Subgroup analysis identified metastases involving > 2 organs or hepatic sites as potential predicative biomarkers for inferior efficacy. Proteomic screening revealed that overexpressed OCIAD2 correlated with poor prognosis, a finding validated in two publicly available external cohorts (CPTAC database and RuiJin cohort). In conclusion, combination of TAS-102 and surufatinib demonstrates clinically meaningful efficacy and manageable toxicity as a later line therapeutic option for refractory mPC. The biomarkers identified in this study may hold the potential to guide patient stratification and warrant further investigation to optimize precision application of this regimen.

Project description:This phase I/II neoadjuvant trial determined maximally-tolerated doses (MTD), dose-limiting toxicities (DLT), response-to-therapy, and explored the role of new response biomarkers. The combination regimens were delivered with acceptable toxicity, good clinical response, inducing changes in tumor RNA content and integrity. Pre-treatment gene expressions impacted clinical response, including several near 17q12, which with ERBB2, may better identify chemoresponsiveness. The NCIC Clinical Trials Group MA.22 phase I/II clinical trial (ClinicalTrials.gov identifier NCT00066443) sought to determine the optimal dosing regimens for docetaxel/epirubicin combination chemotherapy in women with locally advanced (over 95% of patients) or inflammatory breast cancer. The protocol was approved by Health Canada and local Ethics Review Boards, and patients provided written, informed consent. Various doses of epirubicin and docetaxel were administered to patients in either a standard q3 weekly (Schedule A) or dose dense q2 weekly (Schedule B) regimen. Doses for Schedule A were 75 mg/m2 IV of docetaxel and 75, 90, 105, or 120 mg/m2 IV of epirubicin (with 6 mg of pegfilgrastim per cycle on day 2 to prevent neutropenia). Doses for both docetaxel and epirubicin in Schedule B were 50, 60, and 70 mg/m2 IV (with identical pegfilgrastim support). For each schedule, phase I was dose finding for phase II. Patients were allocated to the various phases and dosing regimens of the trial without randomization. None of the patients received trastuzumab in the initial years of the study and HER2+ patients were not enrolled on study, once trastuzumab funding became available. Six tumor biopsy cores were obtained pre-, mid-, and post-treatment: 3 cores for pathologic assessment; 3 cores for microarray studies. Total RNA was isolated from these cores and RNA integrity was assessed using Agilent Bioanalyzer. One of the RNA samples with the best quality was used for microarray study. One colour microarray of Agilent whole human genome nucleotide arrays was conducted with one array per patient. The current data set represents pre-treatment set. MA.22 clinical trial accrued T3N0, any N2 or N3, and T4 breast cancer patients (according to the classification described at; http://emedicine.medscape.com/article/2007112-overview). However, since the current study does not focus on the tumor grade/stage, the information was not provided in the current records.

Project description:Pancreatic cancer is an aggressive disease with a low 5-year survival rate and poor response to therapy. Here, we demonstrate that inhibition of the myeloid-specific SRC family-kinase HCK impairs pancreatic tumor growth and metastasis by enhancing the infiltration of cytotoxic effector cells, stimulating the activation of myeloid cells, and by reducing the desmoplastic microenvironment. Genetic ablation of HCK also maximizes the therapeutic efficacy of chemotherapy and immunotherapy, and improves progression-free survival in mice. Collectively, our results demonstrate that targeting HCK can overcome barriers that limit responses to therapy, and provide a compelling rationale for HCK to be considered as a drug target to improve the responsiveness of pancreatic tumors to chemotherapy and/or immune checkpoint blockade.
