Project description:Resistance to CDK4/6 inhibitors plus endocrine therapy, standard of care for HR+/HER2-neg metastatic breast cancer (MBC) patients, remains as a clinical problem with limited therapeutic options after disease progression. Treatment with abemaciclib after progression on a prior CDK4/6i has received increasing interest. However, molecular predictors of cross-resistance and unique mechanism of resistance to each CDK4/6i are unknown. In this study, multiomics analyses revealed ≥30 differentially altered pathways between palbociclib and abemaciclib resistant models. Mechanistic, preclinical and retrospective clinical evidence showed that HR+/HER2-neg MBC tumors refractory to palbociclib still may be responsive to abemaciclib. Based on the unique mechanisms of resistance to each CDK4/6i, we propose that patients who would benefit from abemaciclib treatment post progression on palbociclib could be selected based on the Hallmark gene set enrichment of G2/M pathway, while those who most probably would be refractory could be identified base on the gene set enrichment of OXPHOS pathway.

Project description:Lung cancer remains the leading cause of cancer-related death due to poor treatment responses and resistance arising from tumor heterogeneity. Here we show that adverse prognosis associated with epigenetic silencing of the tumour suppressor RASSF1A is due to increased deposition of extracellular matrix (ECM), tumour stiffness and metastatic dissemination in vitro and in vivo. We find that lung cancer cells with RASSF1A promoter methylation display constitutive nuclear YAP1 and expression of collagen prolylhydroxylase (P4HA2) which increases collagen deposition. Furthermore, we identify that elevated collagen creates a stiff-ECM which in turn triggers cancer stem-like programming and metastatic dissemination in vivo. Re-expression of RASSF1A or inhibition of P4HA2 activity reverse these effects and increase markers of lung differentiation (TTF-1, Mucin5B). Our study identifies RASSF1A as a clinical biomarker associated with mechanical properties of ECM which increases levels of cancer stemness and risk of metastatic progression in lung adenocarcinoma. Moreover, we highlight P4HA2 as a potential target for uncoupling ECM signals that support cancer stemness.

Project description:Exclusion of cancer patients with brain metastases from clinical trials is a major cause of the limited therapeutic options available for secondary brain tumors. Here, we report a novel drug-screening platform (METPlatform) based on organotypic cultures that allows identifying anti-metastatic compounds in a preparation that includes the tumor microenvironment. By applying this approach to brain metastasis, we identified HSP90 as a promising therapeutic target. A blood-brain barrier permeable HSP90 inhibitor showed high potency against mouse and human brain metastases from melanoma, lung and breast adenocarcinoma with distinct oncogenomic profiles at clinically relevant stages of the disease, including a novel model of local relapse after neurosurgery. Furthermore, in situ proteomic analysis of brain metastases treated with the chaperone inhibitor revealed non-canonical clients of HSP90 as potential novel mediators of brain metastasis and actionable mechanisms of resistance driven by autophagy. Our work validates METPlatform as a potent resource for metastasis research integrating drug-screening and unbiased omic approaches that is fully compatible with human samples. We envision that METPlatform could be established as a clinically relevant strategy to personalize the management of metastatic disease in the brain and elsewhere.

Project description:Some patients with localized pancreatic ductal adenocarcinoma (PDAC) undergo resection with curative intent, but most experience disease recurrence and succumb to their disease. Through examination of 743 PDAC patients in a single-center we find that the site of first recurrence is a major predictor of survival. Patients with initial liver-metastatic recurrence (LIV-MR, n = 100) had significantly shorter disease-free and overall survival compared to those with initial recurrence to the lung (LUN-MR, n = 31). We performed single-nucleus RNA-seq (snRNA-seq) and low-pass whole-genome sequencing (lpWGS) on a representative population of this cohort, including 21 primary PDACs with either LIV-MR or LUN-MR, and found that malignant cells adopt transcriptional programs that mimicked that of their ultimate metastatic target organ. We validated the LIV-MR and LUN-MR signatures in multiple data sets, including single-cell RNA-seq of normal human adult and fetal liver and lung tissues, bulk, single-cell, and spatial transcriptomics of PDAC lung and liver metastases, and in vivo data modelling metastatic organotropism in PDAC and other lineages. We reconstructed two different trajectories of malignant transformation in PDAC and find that the LIV-MR and LUN-MR are associated with these, suggesting that subsequent metastatic organotropism may be determined early in tumorigenesis. Furthermore, primary PDAC tumors recurring with LIV-MR compared to LUN-MR were deplete of infiltrating T cells and enriched with M2-like macrophages, highlighting potential contributions to subsequent outcomes. Taken together, this work provides evidence for pre-existing cellular adaptations that determine metastatic organotropism and may be useful for prognostication and development of therapeutic strategies at the time of diagnosis of resectable PDAC.

Project description:Pheochromocytomas and paragangliomas (PPGLs) arising in patients with SDHB mutations have a well-established predisposition to develop metastatic disease, but the molecular mechanisms underlying this aggressive behavior remain incompletely understood. We investigated how metastasis and SDHB mutation status contribute to the epigenetic landscape of PPGLs, aiming to disentangle their individual and combined effects on tumor progression. This dataset contains the DNA methylation data of the 34 PPGLs analysed in this study.

Project description:This study reports a rare case of endometriosis-associated intestinal tumor (EAIT) in a 53-year-old woman who presented with rectal bleeding. Imaging and histopathology confirmed endometrioid adenocarcinoma arising from rectal endometriosis with ovarian metastasis. The patient underwent posterior total pelvic exenteration followed by adjuvant chemotherapy and has remained disease-free for 3.5 years. Comprehensive proteomic profiling of the tumor identified 9,294 proteins, with data deposited in the jPOST repository. This case highlights the diagnostic challenges, clinical management strategies, and biological insights of EAIT, providing a valuable reference for future comparative and translational research.

Project description:A small percentage of ulcerative colitis patients may experience disease progression into colitis-associated colorectal cancer (CA-CRC). In the clinic, it is challenging to identify these patients. Consequently, all UC patients are followed closely by e.g. annual colonoscopy from eight years of the UC diagnosis. Although, the progression may still be difficult to discover as the neoplastic mucosa may appear with a flat structure. The need for biomarkers that early in the UC disease course can predict the predisposition to progress into CA-CRC is therefore highly wanted in the clinic. The objective of this gene expression analysis was to identify a set of genes with differential expression in inflamed mucosa of progressed UC patients compared to inflamed mucosa of non-progressed UC patients. The gene expression was examined using Agilent SurePrint G3 human gene expression 60K microarray.
