CY-503 for the Treatment of Chemotherapy-refractory Metastatic Colorectal Cancer

ABSTRACT: This trial is designed as a phase II evaluation of the effect of CY-503 or placebo on progression free survival (PFS) defined as the time from start of treatment until the objective observation of progressive disease (PD) or death from any course in patients with chemotherapy-refractory metastatic colorectal cancer.

Clinical efficacy and biomarker analysis of stereotactic body radiotherapy combined with sintilimab and a bevacizumab biosimilar in anti-PD-1 refractory hepatocellular carcinoma

Project description:Immune checkpoint inhibitor (ICI) resistance in advanced hepatocellular carcinoma (HCC)poses a major therapeutic challenge. Here, we report a phase 2 trial evaluating stereotactic bodyradiotherapy (SBRT) combined with the anti-PD-1 antibody sintilimab and bevacizumabbiosimilar (lBI305) to re-sensitize ICI-refractory HCC. Twenty-one patients with progressiveHCC after prior ICI therapy received SBRT (25-50 Gy in 5 fractions) followed by sintilimab(200 mg) and IBl305 (15 mgkg) every 3 weeks. The primary endpoint, objective response rate(ORR) in non-irradiated lesions, was 33.3% (7/21), with a disease control rate of66.7%(14/21)Median progression-free survival (PFS) was 6.2 months (95% CI 3.7-15.1), and estimatedmedian overall survival (OS) was 24.4 months (95% CI 9.2-NA). SBRT achieved 100% localcontrol, while grade >3 treatment-related adverse events occurred in 33.3% of patientsLongitudinal proteomic profiling revealed that responders exhibited lower baseline IFN-y (p0.025) and elevated IL-6 (p=0.002), with post-SBRT increases in IFN-y, IL-2, and IL-6correlating with improved outcomes. These findings demonstrate that SBRT synergizes withdual PD-1/VEGF blockade to overcome ICI resistance, potentially through radiation-inducedimmune remodeling. Biomarker dynamics highlight actionable targets for optimizingcombination therapies in anti-PD-1-refractory HCC.

Decitabine-Primed CAR T-Cell Therapy for Relapsed/Refractory Non-Hodgkin's Lymphoma: A Phase 1/2 Clinical Study Demonstrating Enhanced Efficacy and Prolonged Survival

Project description:Relapsed/refractory Non-Hodgkin's lymphoma (R/R NHL) remains a therapeutic challenge despite advances in immune checkpoint inhibitors (ICIs). While chimeric antigen receptor (CAR) T-cell therapy has shown promise in hematologic malignancies, its efficacy in NHL is limited by T-cell exhaustion and tumor microenvironment (TME) immunosuppression. Preclinical studies suggest that low-dose decitabine, a DNA hypomethylating agent, can epigenetically reprogram T cells to enhance cytotoxicity and persistence. This open-label phase 1/2 trial (NCT04697940) enrolled 33 R/R NHL patients (aged 16–73 years) to evaluate the safety and efficacy of decitabine-primed CAR T cells targeting CD19/CD20 (CART19/20). Patients received lymphodepletion followed by escalating doses (0.5–5×10^6 cells/kg) of decitabine-activated CAR T cells. Primary endpoints included dose-limiting toxicities (DLTs) and objective response rate (ORR); secondary endpoints encompassed progression-free survival (PFS) and mechanistic analyses of T-cell dynamics.In the phase 1 cohort (n=18), no severe cytokine release syndrome (CRS) or neurotoxicity was observed, establishing 5×10^6 cells/kg as the recommended phase 2 dose (RP2D). Among 15 evaluable patients in phase 2, the ORR reached 86.7% (13/15), with a complete remission (CR) rate of 60% (9/15). Median PFS was 22.1 months, significantly surpassing historical controls of PD-1 monotherapy (median PFS: 12–15 months). Mechanistic studies revealed that decitabine pretreatment upregulated AP-1 transcription factor JunD in CAR T cells, promoting clonal expansion of progenitor-exhausted CD8+ T cells and enhancing tumor infiltration.

This approach enabled construction of an integrated map of subset-specific immunopathology, comparing rRA with eRA - two opposing ends of the RA disease continuum - to uncover mechanisms underlying treatment refractory disease and inform future therapeutic strategies.

Single-cell analysis reveals epithelial alarmin-associated immune architecture in pediatric refractory and rhinovirus-positive asthma

Project description:Background: Asthma in children is increasingly recognized as biologically distinct from adult disease, yet the cellular organization underlying refractory asthma and virus-associated exacerbation remains poorly defined. In particular, how chronic airway remodeling and acute viral responses are integrated within the pediatric airway microenvironment is not well understood. Methods:Children with asthma were classified into refractory asthma (defined according to Global Initiative for Asthma criteria) and rhinovirus-positive asthma (defined by qPCR-confirmed rhinovirus infection). Bronchoalveolar lavage fluid samples from children with refractory asthma (n = 5), rhinovirus-positive asthma (n = 4) and healthy controls (n = 6) were subjected to single-cell RNA sequencing, followed by integrative bioinformatic analysis and cross-age comparison with adult asthma datasets. Controls were obtained from individuals undergoing bronchoscopy for non-inflammatory indications and lacked a history of atopy or airway inflammatory disease. Results: Pediatric refractory asthma showed increased epithelial cell representation, elevated epithelial alarmin–related gene expression, enhanced chemokine and interferon-stimulated gene signatures, and increased expression of remodeling-associated genes. Cell–cell communication analysis identified enhanced macrophage-to-epithelium signaling mediated through FN1–CD44/αVβ integrin interactions. Rhinovirus-positive asthma exhibited a more pronounced inflammatory profile, characterized by increased T-cell abundance, enhanced chemokine and interferon responses, and strengthened CTL–macrophage communication via the CCL5–CCR1 axis. Cross-age comparison indicated relatively higher epithelial alarmin–associated features in pediatric asthma and stronger chemokine-associated signatures in adult asthma.
