CyberKnife for Hepatic Metastases From Colorectal Cancer

ABSTRACT: This prospective, multicenter study is intended to establish the efficacy and toxicity of treating unresectable colorectal liver metastases with accurately administered radiation using the CyberKnife stereotactic radiosurgery system.

Type I IFN Drives ILC2 to ILC1 Plasticity in Radiation-Induced Lung Injury

Project description:Radiation-induced lung injury (RILI) is a common dose-limiting toxicity after radiotherapy for lung cancer, especially after ablative radiotherapy such as Stereotactic Body Radiation Therapy (SBRT). However, the immunological mechanisms driving its pathogenesis remain poorly understood. Here we show that innate lymphoid cells (ILCs) orchestrate early immune remodeling after ablative irradiation. Flow cytometry and cytokine profiling revealed a radiation-induced shift from lung-resident ILC2s toward pro-inflammatory ILC1s. Single cell RNA sequencing analysis of lung ILC populations after radiation suggested that type I interferon (IFN) promoted radiation-induced ILC2 to ILC1 plasticity. In addition, IFN-β stimulation of ILC2s in vitro led to increased expression of T-bet and IFN-γ. Inhibition of type I IFN signaling attenuated lung damage in irradiated mice, suggesting a potential therapeutic strategy for mitigating RILI. Our findings highlight a previously unrecognized role of type I IFN in radiation-induced ILC plasticity that contributes to RILI pathogenesis. Importantly, this work provides a framework for developing immune-targeted interventions to reduce normal tissue toxicity during radiotherapy.

Project description:Abstract Purpose: There is a recognised need to develop new methods of high throughput, rapid and minimally invasive individual dose assessment for radiation exposure. The aim of this work is to establish a panel of highly radiation responsive genes suitable for biological dosimetry and to explore inter-individual variation in response to ionising radiation exposure.Materials and method: Analysis of gene expression in response to radiation was carried out using three independent techniques (microarray, Multiplex Quantitative RT-PCR and nCounter Analysis System) in human lymphocytes in culture and circulating blood exposed ex vivo from the same donors. Results: Variations in transcriptional response to exposure to ionising radiation analysed by microarray allowed the identification of genes which can be validated and measured accurately as biomarkers of radiation exposure using other techniques. We have identified genes which are consistently up-regulated following exposure at different time points to either 2 or 4 Gy of X-rays, for all individuals in blood and cultured lymphocytes. Most down-regulated genes including cyclins, centromeric and mitotic checkpoints proteins, particularly those associated with chromosome instability and cancer can only be detected in dividing cells. Conclusions: The data provides evidence that there are a number of genes which seem suitable for biological dosimetry, like SESN1, GADD45A, CDKN1A, CCNG1, FDXR, BBC3 and MDM2. ÊThese biomarkers could potentially be used for triage after large-scale radiological incidents. Variations in transcriptional response accurately measured by MQRT-PCRæ may allow the identification of biomarkers of radiation sensitivity and individual susceptibility and therefore being useful in radiation oncology.

Genomic Analysis of Human Brain Metastases Treated with Stereotactic Radiosurgery Reveals Unique Signature Based on Treatment Failure. [WES]

Project description:Stereotactic radiosurgery (SRS) has been shown to be efficacious for treatment of limited brain metastasis (BM), however, the effects of SRS on human brain metastases have yet to be studied. We performed genomic analysis of resected brain metastases from patients whose resected lesion was previously treated with SRS. Our analyses demonstrated for the first time that patients possess a distinct genomic signature based on type of treatment failure including local failure, leptomeningeal spread, and radio-necrosis. Examination of the center and peripheral edge of the tumors treated with SRS indicated differential DNA damage distribution and an enrichment for tumor suppressor mutations and DNA damage repair pathways along the peripheral edge. Furthermore, the two clinical modalities used to deliver SRS, LINAC and GK, demonstrated differential effects on the tumor landscape even between controlled primary sites. Our study provides, in humans, biological evidence of differential effects of SRS across BM’s.

A Prospective Pilot Study of Urine microRNA as a Biomarker of Urinary Toxicity Following MRI-Guided Prostate Stereotactic Body Radiotherapy

Project description:Urinary toxicities are common following prostate radiotherapy and can significantly impact quality of life. There is a need for biomarkers that can predict the risk of urinary toxicity following treatment using MRI guided stereotactic body radiotherapy (MR-SBRT) to assist with selection of the most appropriate radiotherapy treatment approach and to potentially provide early intervention for toxicities. Many studies demonstrated the applicability of miRNA from bodily fluids as biomarkers; thus, we decided to check if urine-derived miRNA can serve as radiation toxicity predictors. Both sequencing and confirmatory targeted qPCR showed that expressions of miR-26a-5p and miR-95-3p decrease during MR-SBRT and begin to increase during early follow up. Levels of miR-204-3p and miR-192-5p behave differently in patients with and without urinary toxicity. Expression of miR-375-3p is lower in patients with higher risk category at all timepoints and tends to drop during and after MR-SBRT, especially in patients with lower risk category. These results warrant further studies on urine miRNAs as non-invasively measured biomarkers.
