Phase I/II trial of encorafenib, cetuximab, and nivolumab in microsatellite stable, BRAFV600E metastatic colorectal cancer [dataset 1]

Project description:The BRAF inhibitor encorafenib and anti-EGFR antibody cetuximab modestly improve survival for patients with microsatellite stable (MSS) BRAFV600E metastatic colorectal cancer (mCRC), characterized by higher immune activation than MSS BRAFwild-type CRC. In this phase I/II study (NCT04017650) of 26 participants with MSS BRAFV600E mCRC who received encorafenib, cetuximab, and anti-PD-1 antibody nivolumab, we report an overall response rate of 50% (95% confidence interval (CI) 29-71) and median progression-free survival of 7.4 months (95% CI, 5.6-9.6). Transcriptomic profiling of pretreatment biopsies and extracellular vesicle RNA (evRNA) isolated from plasma show enrichment of non-canonical MAP kinase signaling and immune activation signatures for responders. Complement pathway activation enriches in non-responder biopsies. On serial evRNA profiling, decreased MAPK signature and increased interferon gamma response signature associate with sustained treatment benefit. MSS BRAFV600E mCRC with baseline MAP kinase activation and immune activation signatures may benefit from the triple combination but not with complement pathway activation.

Expression data for 52 microsatellite stable (MSS) primary tumors from patients with proximal colon cancer.

Project description:Samples were taken from surgically resected tumor specimens from patients with proximal colon cancer. The expression profiles were determined using the Affymetrix GeneChip Human Exon 1.0 ST Array version 2. APC gene mutation status was determined using Sanger sequencing. A classifier for APC mutation status was trained using these expression data. 52 microsatellite stable (MSS) proximal colon cancers samples were analyzed. 17 samples were APC wild-type and 35 had APC protein-truncating mutations.

Comprehensive proteogenomic analysis identifies immune targets in microsatellite stable and unstable colorectal cancers

Project description:Immunotherapy in colorectal cancer (CRC) has largely focused on microsatellite instability (MSI) tumors, based on the assumption that high mutational burden drives immunogenicity. However, most CRCs are microsatellite-stable (MSS) and remain unresponsive to checkpoint blockade. Using proteogenomic profiling of 26 primary CRC tumors, we identified 115,292 MHC-I–associated peptides across 61 HLA alleles, yielding 196 tumor-associated antigens (TAAs) and 70 aberrantly expressed tumor-specific antigens (aeTSAs).

Spatial predictors of response to chemo-immunotherapy in microsatellite stable metastatic colorectal cancer

Project description:Microsatellite-stable (MSS) colorectal cancers (CRC) are largely unresponsive to immune checkpoint inhibition (ICI). The MAYA trial used temozolomide (TMZ) in MGMT-silenced MSS mCRC, hypothesizing that TMZ-induced hypermutation could sensitize tumors to ICI; the primary endpoint was met, showing durable responses with TMZ plus ipilimumab and nivolumab. We perform integrated spatial, transcriptomic, and immune profiling of longitudinal tumor and blood samples from patients treated on the MAYA trial. Post-TMZ increases in tumor mutational burden associate with improved progression-free survival. Spatial profiling demonstrates that clinical benefit is greatest in permissive tumor microenvironments. Responders exhibit enrichment of cytotoxic T cells across tumor and stromal compartments, whereas non-responders display heterogeneous cellular neighborhoods, with fibroblasts in close spatial proximity to T cells, consistent with barriers to immune mediated clearance. Longitudinal peripheral immune profiling shows that early upregulation of TIGIT and PD-1 following TMZ exposure predicts resistance. Together, these findings indicate that both mutational evolution and spatial immune architecture contribute to immune sensitization in MGMT-silenced MSS CRC.

Single cell RNA-seq of Panc02-H7-Fluc model with vehicle tobemstomig, nivolumab and relatlimab-like treatment

Project description:We used transgenic mice challenged subcutaneously with Panc02-H7-Fluc tumors to compare the efficacy of tobemstomig with nivolumab and relatlimab-like monotherapies, as well as their combination, and further investigated their mechanism of action. We used a murinized version of tobemstomig, nivolumab and relatlimab-like anti-LAG-3 to prevent the formation of anti-drug antibodies and to prolong the treatment to 3 weeks. We performed single cell RNA sequencing on the tumor samples collected at day 28. We found both qualitative and quantitative differences between vehicle, mu-tobemstomig, mu-nivolumab monotherapy and in combination with mu-relatlimab-like anti-LAG-3.

Transcription profiling by array of microsatellite stable stage III Colorectal Adenocarcinoma (MSS-CRC) with wild type or mutant p53 to study the prognostic significance of the mutation

Project description:Clinical Significance: Understanding the differences in colorectal cancer (CRC) aggressiveness and clinical outcomes in relation to tumor stage and different molecular subsets is at most important for designing treatment regimens. However, molecular signatures for specific phenotypic subsets that predict the aggressiveness and clinical outcomes of CRC, specifically in advanced disease stage are lacking. Therefore, for the first time, the current study has identified a set of molecular markers that are associated with aggressive Stage III CRCs that exhibited microsatellite stable and mutant p53 phenotypic features. These findings might aid in designing aggressive treatment regimens and help to provide insights into the development of novel therapeutic targets. Results: Increased incidence of p53 mutations in MSS CRCs (58%) was associated with higher CRC-specific mortality than MSS-p53 wild-type phenotypes (log-rank, P=0.025; and hazard ratio, 2.52; 95% confidence interval, 1.25-5.08). Of 49 down-regulated genes, LPAR6, PDLIM3 and PLAT and of 35 up-regulated genes, TRIM29, FUT3, IQGAP3, and SLC6A8, were confirmed by qNPA, qRT-PCR, and IHC platforms.
