Project description:Patients with metastatic ovarian cancer (OvCa) have a 5-year survival rate of less than 30% due to persisting dissemination of chemoresistant cells in the peritoneal fluid and the immunosuppressive microenvironment in the peritoneal cavity. Here, we report that intraperitoneal administration of β-glucan and IFNγ (BI) induced robust tumor regression in clinically relevant models of metastatic OvCa. BI induced tumor regression by controlling fluid tumor burden and activating localized antitumor immunity. β-glucan alone cleared ascites and eliminated fluid tumor cells by inducing intraperitoneal clotting in the fluid and Dectin-1-Syk-dependent NETosis in the omentum. In omentum tumors, BI expanded a novel subset of immunostimulatory IL27+ macrophages and neutralizing IL27 impaired BI efficacy in vivo. Moreover, BI directly induced IL27 secretion in macrophages where single agent treatment did not. Finally, BI extended mouse survival in a chemoresistant model and significantly improved chemotherapy response in a chemo-sensitive model. In summary, we propose a new therapeutic strategy for the treatment of metastatic OvCa.

Project description:Peritoneal metastasis (PM) has a suppressive tumor immune microenvironment (TIME), which limits the effects of immunotherapy. This study aims to investigate the immunomodulatory effects of intraperitoneal administration of IL-33 on PM-associated TIME. Immunocompetent mice were used to investigate the role of IL-33 in development of abdominal dissemination and host outcome. Murine (m) and human (h) gastric cancer cells were tested for their response to IL-33 by qRT-PCR, flow cytometry, and immunofluorescence. Survival was significantly prolonged in patients with high Il-33 mRNA expression. Intraperitoneal administration of IL-33 could induce the celiac inflammatory environment, activate immunologic effector cells and reverse the immunosuppressive tumor microenvironment, which delayed tumor progression and peritoneal metastasis of gastric cancer. Mechanistically, IL-33 could induce M2 polarization by activating p38-GATA-binding protein 3 (GATA3) signaling pathway. IL-33 combined with anti-CSF1R or p38 inhibitor to regulate tumor-associated macrophages (TAMs) showed synergistic anti-tumor effect. Intraperitoneal administration of IL-33 inducing local inflammatory milieu provided a novel approach for the treatment of metastatic peritoneal malignancies, which combined with TAMs reprogramming to reshape TIME could achieve better treatment efficacy.

Project description:Peritoneal dissemination of ovarian cancer (OC) correlates with poor prognosis, but the mechanisms underlying the escape of OC cells from the intraperitoneal immune system have remained unknown. We here identify pigment epithelium–derived factor (PEDF) as a regulator of OC cell dissemination, which functions through induction of CD206+ IL-10–producing macrophages. High PEDF gene expression was associated with poor prognosis in OC patients. Concentrations of PEDF in ascites and serum were significantly higher in OC patients than those with more benign tumors and correlated with early recurrence of OC patients, suggesting that PEDF might serve as a prognostic biomarker. Bromodomain and extraterminal (BET) inhibitors were found to reduce PEDF expression and limit both OC cell survival and CD206+ macrophage induction in the peritoneal cavity. Our results thus implicate PEDF as a driver of OC dissemination and identify a BET protein–PEDF–IL-10 axis as a promising therapeutic target for OC.

Project description:Overweight/obesity is an established risk factor for ovarian cancer onset, yet its impact on relapses after first-line chemotherapy remains unclear, complicating therapeutic strategies. Peritoneal macrophages, as the most abundant immune population in the peritoneal cavity, serve as the first line of host defense, eliminating pathogens or disseminating tumor cells through phagocytosis in the peritoneal cave. In this study, we found that Impaired peritoneal macrophage phagocytosis correlates with tumor recurrence in the peritoneal cavity of obese mice or overweight patients. To investigate how HFD affects the anti-tumor capacity of peritoneal macrophages, we isolated peritoneal macrophages by flow cytometry from ID8 tumor-bearing mice one week after chemotherapy, when tumor burdens were comparable between groups and performed RNA sequencing to systemic address the underlying mechanisms.

Project description:Background: For the majority of patients of metastatic colorectal cancer (mCRC), systematic chemotherapy has been a choice for palliative treatment. When first-line chemotherapies do not work, targeted therapies are administered. Because of inter-individual genetic and epigenetic variations, patients with mCRC who are treated with these therapies may respond differently. It is of great clinical interest to elucidate response mechanisms that could be utilized in predicting individual response to these treatments. Methods: A highly sensitive technique, the 5hmC-Seal was applied to profile genome-wide 5-hydroxymethylcytosines (5hmC) in 155 cfDNA samples from patients with mCRC recruited for two clinical trials at Zhongshan Hospital, Fudan University in China. Response to systematic chemotherapy and targeted therapy (becacizumab or cetuximab) (CR, complete response; PR, partial response; SD, stable disease; PD, progressive disease) was measured and a 5hmC-based predictive model in cfDNA was developed using a machine learning approach in the PROVE Study (NCT03679039) (n = 100), followed by validation in the METHOD Study (NCT03599947) (n = 55). Results: Genome-wide mapping of 5hmC in patient-derived cfDNA suggested a distinct 5hmC landscape showing tissue origin and gene regulatory relevance. A eleven-gene model involving relevant pathways such as Rap1 signaling pathway, cAMP signaling pathway, Phospholipase D signaling pathway and cGMP-PKG signaling pathway, distinguished patients with mCRC who responded to chemotherapy, regardless of targeted therapy, resectability, or mutation status in the validation samples (AUC=0.77; 95% CI, 0.64-0.90), outperforming RAS mutation status (AUC=0.70; 95% CI, 0.57-0.83). In addition, the 5hmC model also showed a trend of capacity for predicting progression-free survival (PFS) for mCRC. Conclusions: Genome-wide mapping reflected relevant pathways and functional interaction networks implicated in the determination of treatment response for patients with mCRC. The 5hmC-Seal model in cfDNA provided a promising non-invasive method for predicting response to systematic chemotherapy in patients with mCRC, who will be essential for personalized medicine.

Project description:High-grade serous (tubo-ovarian) carcinoma (HGSC) is the most lethal gynecological malignancy and the most common form of ovarian cancer. Most patients (75%) are diagnosed at advanced disease stages, with widespread peritoneal dissemination leading to poor prognosis. Cancer-associated fibroblasts (CAFs) were shown to modulate the immune microenvironment in multiple cancer types. However, their interactions with immune cells and their role in facilitating peritoneal metastasis in ovarian cancer are largely unresolved. Here, we characterized the interactions between fibroblasts and immune cells that facilitate the formation of a hospitable niche in metastatic peritoneal ovarian cancer. We isolated and characterized CAFs from HGSC patients and mouse models, and investigated the events that promote peritoneal metastases. Our findings reveal the reciprocal communication between fibroblasts and T cells in distinct peritoneal metastatic niches, positioning the TIGIT-CD155 immune checkpoint signaling axis as a target for therapeutic intervention in metastatic HGSC. Our study provides a potential novel avenue for therapeutic intervention to inhibit metastatic spread of ovarian carcinoma.

This study aimed to establish an orthotopic model of PDAC peritoneal dissemination and to uncover the transcriptional and regulatory programs underlying this process. METHODS: Organoids were established from primary pancreatic tumors and malignant effusions of patients with PDAC and orthotopically transplanted into the pancreas of immunodeficient mice to generate patient-derived orthotopic xenograft (PDOX) models. Subsequently, the organoids were rederived from pancreatic and peritoneal lesions of a representative model (PDOX12) and orthotopically reimplanted to assess the dissemination capacity. Single-nucleus RNA sequencing (snRNA-seq) and single-cell ATAC sequencing (scATAC-seq) were performed to analyze the tumors from these models. RESULTS: The organoids that were derived from malignant effusions reproducibly generated peritoneal metastases after orthotopic implantation. To dissect this process more precisely, we focused on one representative model (PDOX12) and rederived organoids from its pancreatic and peritoneal lesions. These organoids generated matched PDOX models that differed only in dissemination potential when reimplanted orthotopically. The results of snRNA-seq revealed a distinct subpopulation enriched in the high-dissemination model, which was characterized by the coordinated activation of genes involved in cytoskeletal dynamics, extracellular matrix remodeling, and plasticity-related signaling—suggesting a dissemination-primed state. Integration with scATAC-seq identified STAT3, SMAD3, and SOX2 as potential upstream regulators of this gene program. CONCLUSIONS: This study established PDOX models that isolate the peritoneal dissemination phenotype and reveal the transcriptional and regulatory programs driving this process.
