Self-Management and Activation to Reduce Treatment-Related Toxicities (SMARTCare)

ABSTRACT: The investigators will undertake a multi-centre, randomized controlled trial to implement and evaluate a proactive model of care (SMARTCare) during active cancer treatment that incorporates self-management support (SMS). Patients allocated to the control arm will receive care from ambulatory clinic nurses trained in SMS. Patients allocated to the intervention arm will will receive care from ambulatory clinic nurses trained in SMS, in addition to being given access to a web-based, self-management education program and nurse-led health coaching during the first four months following the first systemic therapy administration.

Special survivorship OPD for colorectal cancer patients to improve follow up

Project description:Intervention1: Arm B survivorship programme: Patients on Arm B will be randomized to the survivorship programme and will be followed up in a special survivorship clinic. The follow up clinic will be scheduled at a fixed time on alternate Tuesday. The team in the clinic will consist of specialist nurses trained in enterostomal therapy as well as counselling in addition to the clinicians (surgeon, oncologist, radiotherapist, gastroenterologist). The appointments will be managed by the clinic nurses. We will use telephonic reminders/ email or SMS alerts to remind patients about their follow up dates. In case of a missed appointment we will plan a telephonic interview on a day other than the routine clinic day). The follow up protocols will be similar. All patients with age less than 40 years will be referred to the genetic clinic. Patients with familial cancers will be managed as per standard guidelines. All data will be recorded in a CRF (Appendix 3) and entered into a prospective SPSS based database in the survivorship programme. Patients who are planned for treatment with a palliative intent or who are planned for Best Supportive Care (BSC) will be excluded. Control Intervention1: ARM A :standard follow up: Patients on Arm A will be randomized to standard follow up. These patients will be followed up in the regular clinic (general or private) along with other CRC patients as per current SOPs for surveillance and follow up (Appendix 1). Data will be evaluated at end of 2 years from Electronic Medical Records (EMR). Primary outcome(s): To evaluate whether a survivorship programme improves follow up in CRC patients treated with a curative intent.Timepoint: 5 years

Improving Care After Chemotherapy

Project description:The main purpose of this study is to examine changes in patient-reported symptoms during the first two cycles of neoadjuvant or adjuvant chemotherapy for non-small cell lung cancer (NSCLC), colorectal cancer (CRC) and breast cancer, among patients who receive standard care plus a proactive nursing intervention relative to patients who receive standard care alone. Interventions to improve symptom management and prevent urgent care needs in both the clinic and hospital for patients receiving chemotherapy with curative intent are needed to enhance the quality of cancer care.

Project description:We investigated genome-wide DNA methylation of newly emerged queens, newly emerged workers, adult nurses, adult foragers and adult reverted nurses using comprehensive high throughput arrays for relative methylation (CHARM) We used custom Nimblegen microarrays We isolated genomic DNA from newly emerged queens, newly emerged workers, adult continuous nurses, adult continuous foragers and adult reverted nurses hybridized to custom-designed Nimblegen microarrays (CHARM arrays). Multiple brains were pooled for each sample and used for the genome-wide DNA methylation analysis.

Collaborative Network to Take Responsibility for Oral Anticancer Therapy

Project description:This study aims to develop, implement and evaluate a care pathway for patients with cancer who are treated with oral anticancer drugs (OACD). The care pathway will be developed in six hospitals in Flanders, and will be adapted to the local setting and situation. The investigators hypothesize that the implementation of a care pathway will increase the quality of the drug therapy, the communication between health care professionals (HCPs) and patients, and will lead to an improved level of self-management and adherence. Moreover, the invesitgators hypothesize that the care pathway will facilitate the communication between HCPs in the hospital setting and in ambulatory care, and will improve counseling skills, self-efficacy and self-confidence of HCPs. At the end of the study, a roadmap for the nation-wide implementation of a similar care pathway will be constructed based on the experiences of the participating hospitals. This roadmap will certainly include an e-learning platform for healthcare professionals.

Ambulatory Cancer Pain Management: A Feasibility Study

Project description:The primary purpose of this study is to examine the feasibility of collecting pain and other symptom data in outpatient treatment centers across multiple institutions in the Texas Medical Center (TMC). We will also examine the prevalence and severity of cancer-related pain and begin to identify other commonly reported symptoms in outpatient settings. Factors that interfere with management of pain will also be examined. Additionally, the study will explore patient satisfaction with pain management and pain education in outpatient settings. The specific aims of this study include: Primary Objective: -Explore the feasibility of a collaborative effort by oncology nurses to collect pain and symptom data across multiple institutions that care for ambulatory cancer patients in the TMC. Secondary Objectives: * Describe the prevalence and severity of pain and other symptoms in persons with breast, colorectal, lung, or prostate cancer who are receiving chemotherapy in the outpatient setting. * Describe factors that interfere with adequate pain management in these patients. * Examine patient satisfaction with pain management and patient education about pain management.

Antennae transcriptomics of Varroa-hygienic and Nurse/Forager bees

Project description:The Varroa mite represents the main threat of honey bees (Apis mellifera). Bees from some colonies can limit the proliferation of this parasite by detecting and removing parasitized brood, such behavior is defined as Varroa sensitive Hygiene (VSH). This is an important issue for selecting colonies that can survive Varroa outbreaks. We therefore study the molecular meachnisms underlying this behavior by comparing the antennae transcriptomic profile of VSH and non-VSH bees. Those profiles were further compared to to the profiles of nurses and forager profiles involved in brood care and food collection, respectively.

Fetal malformations and early embryonic gene expression response in Cynomolgus monkeys maternally exposed to thalidomide

Project description:EXPERIMENT: The animal experiments were performed at Shin Nippon Biomedical Laboratories (SNBL), Ltd. (Kagoshima, Japan) in compliance with the Guideline for Animal Experimentation (1987), and in accordance with the Law Concerning the Protection and Control of Animals (1973) and the Standards Relating to the Care and Management of Experimental Animals (1980). This study was approved by the Institutional Animal Care and Use Committee of SNBL and performed in accordance with the ethics criteria contained in the bylaws of the SNBL committee. Each female monkey was paired with a male of proven fertility for one day between day 11 and day 15 of the menstrual cycle. Pregnant females, aged 5-8 years and weighing 2.84-3.76 kg on day 22 of gestation, were allocated randomly to two groups, each with three monkeys, and housed individually. The monkeys were orally dosed with (Â±)-thalidomide (Lot no. SDH7273/SDJ3347, Wako Pure Chemical Industries, Ltd., Osaka, Japan) at 0 or 20 mg/kg by oral intubation on day 26 of gestation, which was during the critical period for thalidomide-induced teratogenesis [Delahunt and Lassen, 1964; Hendrickx, 1973]. Dosage was adjusted to the body weight on day 22 of gestation and the drug was packed in a gelatin capsule. Control monkeys received the capsule only. ANIMAL MODEL: Macaca fascicularis INTERVAL: NON. Interventions to Decrease Financial Toxicity

Project description:Financial distress affects 30-70% of cancer patients and describes the burden that patients experience due to the costs of care (CoC). One reason may be because patients lack the appropriate information on CoC that would help them better plan for and manage their CoC. Therefore, the investigators plan to test a Proactive CoC intervention which includes a discussion with a trained educator on CoC information and a Cost Tracking tool to help patients deal with their CoC.
