Lifestyle Intervention Among Participants of the French Colorectal Cancer Screening Program

ABSTRACT: The investigators propose a randomized controlled trial to develop and evaluate the impact of promoting advice on diet and lifestyle recommendations for cancer prevention at colorectal cancer screening among individuals who may be at higher risk for colorectal cancer (CRC). The specific objectives of this study are to 1) develop a lifestyle intervention based on evidenced-based diet and lifestyle recommendations (i.e. those proposed by the World Cancer Research Fund (WCRF) and the French National Cancer Institute); 2) evaluate the effect of the intervention on the adherence to the target diet and lifestyle recommendations as well as other subjective health measures, including quality of life; 3) evaluate the effect of the intervention on biological pathways linked to colorectal cancer development including physical fitness, anthropometrics, biomarkers for nutrition, and metabolic health.

Project description:Interventions: This is a pilot 3-arm randomised controlled trial with the primary aim of investigating the efficacy of an enhanced eHealth nutrition intervention at reducing lifestyle-related colorectal cancer risk (as determined by the 2018 World Cancer Research Fund (WCRF)/American Institute for Cancer Research (AICR) score) at 3 months, within patients undergoing colonoscopy. This will be compared with the efficacy of a standard eHealth nutrition intervention, and no intervention, at reducing lifestyle-related colorectal cancer risk at 3 months, within patients undergoing colonoscopy. Enhanced intervention group: Enhanced group participants will receive one 30 minute online consultation via Zoom with an Accredited Practising Dietitian, and a personal nutrition report generated through completion of the online Australian Eating Survey (AES), at baseline. The one-on-one online consultation with the dietitian, using a standardised format and informed by results of the AES nutrition report, will provide participants with personalised dietary advice, strategies, and resources to improve dietary intake over 3 months, in line with the 2018 WCRF/AICR Recommendations for Cancer Prevention. The personal nutrition report will contain specific information and feedback about the participants’ current dietary intake, including energy intake from core and non-core foods and drinks, macronutrient and micronutrient intake, and diet quality. The resources provided will include resources designed specifically for this study by a team of dietitians, as well as resources readily available from organisations such as the World Cancer Research Fund, Australian Government, Cancer Council, Cancer Institute NSW, and Nutrition Australia. Project description:This ordinary differential equation model of the interactions between tumor and normal cells is based on the publication: S. A. Alharbi and A. S. Rambely, "Dynamic Simulation for Analyzing the Effects of the Intervention of Vitamins on Delaying the Growth of Tumor Cells," in IEEE Access, vol. 7, pp. 128816-128827, 2019 doi: 10.1109/ACCESS.2019.2940060. Comment: This TNM model is described by the system of equations presented in (2), in the publication manuscript. Abstract: The natural sources of the vitamins, which come from a balanced diet (as recommended by the World Cancer Research Fund and the American Institute for Cancer Research) contribute to protecting the body from advancing progressive of cancer stages. Thus, in this study, we analyze the effect of the intervention of vitamins on delaying the growth of cancer cells based on the dynamics of a normal cell cycle when the tumor cells appear in a tissue as a resulting for progressing abnormal cells due to the weak response of the immune system. We developed a mathematical model, called tumor-normal-vitamins model (TNVM), which is governed by a system of ordinary differential equations and refers to two main populations normal cells and tumor cells. This model considers the intervention of vitamins as a moderating factor within thirty days. The models are discussed analytically and numerically by utilizing the Runge-Kutta method to simulate them. The results of the analysis and simulation of free model illustrate that the model will be stable if the tumor cells succeed in eliminating normal cells in the tissue. Whereas, the analysis and simulation of the TNVM showed a case of coexistence between normal cells and tumor cells occur if an individual consumes a regular rate of vitamins that have been simulated to be 87% per day from a natural food source. Even though the response of the immune system is weak, the daily consumption of enough vitamins can play an essential role in delaying the development of an early stage of cancer. This study contributes to the increasing awareness regarding a healthy diet to reduce the risk of some deadly diseases, especially cancer.

Project description:Lifestyle intervention can improve insulin sensitivity in obese youth yet few studies have examined the biological mechanisms underlying improvements. Therefore, the purpose of this study was to explore biological pathways associated with intervention-induced improvements in insulin sensitivity. Fifteen (7M/8F) overweight/obese (BMI percentile=96.3M-BM-11.1) Latino adolescents (15.0M-BM-10.9 years) completed a 12-week lifestyle intervention that included weekly nutrition education and 180 minutes of moderate-vigorous exercise per week. Insulin sensitivity, estimated by an oral glucose tolerance test and the Matsuda Index, increased 29.2% post intervention (2.4M-BM-10.3 to 3.1M-BM-10.3, p=0.01). Global microarray analysis profiling from whole blood was performed to examine changes in gene expression and to explore biological pathways that were significantly changed in response to the intervention. A total of 1,459 probes corresponding to mRNA transcripts (717 up, 742 down) were differentially expressed with a fold changeM-bM-^IM-%1.2 and P<0.05. Among the genes identified were hexokinase 3 (HK3), ATPase, H+ transporting V0 subunit e2 (ATPV0E), and sterol regulatory element binding transcription factor 1 (SREBF1), and endothelial cell adhesion molecule (ESAM). There were 8 pathways identified that met the criteria for significance, including insulin signaling, type 1 diabetes, and glycerophospholipid metabolism. Participants that increased insulin sensitivity exhibited five times the number of significant genes altered compared to non-responders (1,144 vs. 230). These findings offer insight into the molecular mechanisms underlying health improvements among high-risk Latino youth. Lifestyle interventions may contribute to improved insulin sensitivity through pathways related to insulin signaling and immune response. Further, genetic factors may mediate response to lifestyle intervention. Fifteen (7M/8F) overweight/obese Latino Youth Whole blood RNA samples evaluated pre and post intervention.
