Reducing Fear of Cancer Recurrence in Danish Colorectal Cancer Survivors

ABSTRACT: Cognitive therapy has been shown to reduce fear of cancer recurrence (FCR), mainly in breast cancer survivors. The accessibility of cognitive behavioural interventions could be further improved by Internet delivery, but self-guided interventions have shown limited efficacy. The aim of this study is to test the efficacy of a therapist guided internet-delivered intervention (TG-iConquerFear) vs. augmented treatment as usual (aTAU) in Danish colorectal cancer survivors.

Extracellular vesicle proteome of breast cancer patients with and without cognitive impairment following anthracycline-based chemotherapy

Project description:Cognitive impairment due to cancer and its therapy is a major concern among cancer survivors. Extracellular vesicles (EVs) composition altered by cancer and chemotherapy may affect neurological processes such as neuroplasticity, potentially impacting the cognitive abilities of cancer survivors. We investigated the EV proteome of breast cancer patients with and without cognitive impairment following anthracycline-based chemotherapy from longitudinally collected plasma. EVs were cup-shaped and positive for Flotillin-1 and TSG-101. We identified 517 differentially expressed EV proteins between the cognitive impaired and non-impaired groups during and post-chemotherapy. The observed decreased expression of p2X purinoceptor, cofilin-1, ADAM 10, and dynamin-1 in the plasma EVs of cognitive impaired group may suggest alterations in mechanisms underlying synaptic plasticity. The reduced expression of tight junction proteins among cognitive-impaired patients may imply weakening of the blood-brain barrier. These EV protein signatures may serve as a fingerprint that underscore the mechanisms underlying cognitive impairment in cancer survivors.

The SWORD-study: Managing fear of cancer recurrence.

Project description:Interventions: The intervention is designed as blended care, combining traditional face-to-face cognitive behavior therapy (CBT) with online activities (psycho-education, assignments, assessments and therapist contact). If access to the website is not possible a workbook will be supplied instead. The CBT lasts three months and comprises five individual hourly face to face sessions, supplemented with three (15 minute) electronic consultations (‘e-consults’) or contact by telephone. The therapy protocol is based on a theoretical model by Lee-Jones (1997) which hypothesizes that an emotional reaction (fear) can be the result of interpretations and cognitions of the threat of cancer, triggered by perceptions of internal and/or external cues, leading to dysfunctional behavior and subsequently an increased fear response. The CBT is directed at change of the cognitions and behaviors managing FCR. It contains techniques commonly used in psychotherapy, such as cognitive reframing, exposure- and response prevention, psycho-education, mindfulness and relaxation exercises. Transcriptional, behavioural and biochemical profiling in the 3xTg-AD mouse model reveals a specific signature of amyloid deposition and functional decline in Alzheimer’s disease

Project description:Alzheimer’s disease (AD)-related degenerative decline is associated to the presence of amyloid beta (Aβ) plaque lesions and neuro fibrillary tangles (NFT). However, the precise molecular mechanisms linking Aβ deposition and neurological decline are still unclear. Here we combine genome-wide transcriptional profiling of the insular cortex of 3xTg-AD mice and control littermates from early through to late adulthood (2-14 months of age), with behavioural and biochemical profiling in the same animals to identify transcriptional determinants of functional decline specifically associated to build-up of Aβ deposits. Differential expression analysis revealed differentially expressed genes (DEGs) in the cortex long before observed onset of behavioural symptoms in this model. Using behavioural and biochemical data derived from the same mice and samples, we found that down but not up-regulated DEGs show a stronger average association with learning performance than random background genes in control not seen in AD mice. Conversely, these same genes were found to have a stronger association with Aβ deposition than background genes in AD but not in control mice, thereby identifying these genes as potential intermediaries between abnormal Aβ/NFT deposition and functional decline. Using a complementary approach, gene ontology analysis revealed a highly significant enrichment of learning and memory, associative, memory and cognitive functions only among down-regulated, but not up-regulated, DEGs. Our results demonstrate wider transcriptional changes triggered by the abnormal deposition of Aβ/NFT occurring well before behavioural decline and identify a distinct set of genes specifically associated to abnormal Aβ protein deposition and cognitive decline.

Host whole genome variations are associated with neurocognitive outcome in survivors of pediatric medulloblastoma

Project description:Host whole genome analysis is a promising source of predictive information for long-term morbidity in cancer survivors. However, studies on genetic predictors of long-term outcome, particularly neurocognitive function following chemoradiation in pediatric oncology are limited. In the present study, we evaluated variations in host whole genome single nucleotide polymorphisms (SNPs) and their association with cognitive outcome. Whole-genome SNP analysis of host peripheral blood was conducted on 22 medulloblastoma long-term survivors, of whom 18 completed neuropsychological testing. First, unsupervised consensus clustering of the most variable SNPs within 409 genes involved in DNA repair was performed. Discrete variant groups were identified, although they were not associated with cognitive outcome, suggesting that variations in genes corresponding to a single functional group may be insufficient to predict long-term outcome alone. In support of this interpretation, unsupervised hierarchical clustering analysis using disease-associated gene variants by cognitive impairment status yielded two distinct variant clusters comprised of 36 variants, 34 of which were in noncoding regions. These findings illustrate for the first time that cognitively impaired survivors have a distinct variant profile compared to other medulloblastoma survivors. Future research in larger cohorts is needed to validate host genome predictors of cognitive impairment that may impact clinical management.
