The Genomic Medicine at VA Study

ABSTRACT: This trial will determine the clinical effectiveness of polygenic risk score testing among patients at high genetic risk for at least one of six diseases (coronary artery disease, atrial fibrillation, type 2 diabetes mellitus, colorectal cancer, breast cancer, or prostate cancer), measured by time-to-diagnosis of prevalent or incident disease over 24 months.

HDL Proteomics in Coronary Artery Calcification in Type 1 Diabetes (CACTI) Study

Project description:Atherosclerotic cardiovascular disease (CVD) is the major cause of death in patients with type 1 diabetes mellitus (T1DM). Alterations in the HDL proteome associate with prevalent CVD in T1DM. We therefore determined which proteins carried by HDL might predict incident CVD in T1DM patients. Using targeted MS/MS, we quantified 50 proteins in HDL from 181 T1DM subjects enrolled in the prospective Coronary Artery Calcification in Type 1 Diabetes study (CACTI). We used Cox proportional regression analysis and a case-cohort design to test associations of HDL proteins with incident CVD (myocardial infarction, coronary artery bypass grafting, angioplasty, or death from coronary heart disease). Only one HDL protein—SFTPB (pulmonary surfactant protein B)—predicted incident CVD in all of the models tested. In a fully adjusted model that controlled for lipids and other risk factors, the hazard ratio was 2.17 per SD increase of SFTPB (95% confidence interval, 1.12-4.21, P=0.022). Plasma fractionation demonstrated that plasma SFTPB is nearly quantitatively bound to HDL. Although previous studies have shown that high plasma levels of SFTPB associate with prevalent atherosclerosis only in smokers, we found that SFTPB predicted incident CVD in T1DM independently of smoking status. Elevated levels of SFTPB in HDL strongly predicted incident CVD in CACTI subjects independently of a wide range of confounding factors, including smoking status, HDL-C, LDL-C, and triglyceride levels. Because SFTPB is almost quantitatively bound to plasma HDL, our observations support the proposal that SFTPB carried by HDL is a marker—and perhaps mediator—of CVD risk in patients with T1DM.

Serum Alpha-2-Macroglobulin Links Type 1 Diabetes Status and Incident Cardiovascular Disease

Project description:Background: Cardiovascular disease (CVD) is a leading cause of mortality in individuals with type 1 diabetes mellitus (T1DM), with conventional risk factors not fully explaining the heightened risk. Study Design: We conducted a cross-sectional study comparing serum protein levels in 134 participants with T1DM from the Coronary Artery Calcification in Type 1 Diabetes study to 47 healthy controls, followed by a case-cohort study among 181 T1DM participants, including 47 with CVD events. Linear regression analyzed serum protein levels by diabetes status, while Cox proportional hazards regression assessed associations with incident CVD. Findings: Among 28 serum proteins tested, five showed significant differences between T1DM patients and controls, with alpha-2-macroglobulin (A2M) being notably 1.6-fold higher in T1DM. Adjusted models revealed that A2M (mean hazard ratio [HR] 2.5, 95% confidence interval [CI] 1.5-4.1) and apolipoprotein C3 (APOC3, mean HR 1.7, 95% CI 1.1-2.7) were independently associated with incident CVD in T1DM. Conclusions: Elevated serum levels of A2M in T1DM suggest its potential role as a marker—and possibly a mediator—of CVD risk in these patients, alongside APOC3. These findings underscore the significance of specific serum proteins in understanding and managing CVD risk in T1DM.

Project description:Gene expression in peripheral blood is shown sufficient to differentiate patients with metabolic disorders from control. The signatures of metabolic syndrome, coronary artery disease and type 2 diabetes also have significant overlap. Microarrays were performed on 35 subjects in the following groups: control (n=9), rheumatoid arthritis (n=6), metabolic syndrome (n= 6), coronary artery disease (n=6) and type 2 diabetes (n=8). Intensity values for each array were normalized to a sum of 10,000.

Global gene expression profile of coronary artery disease in Asian Indians

Project description:Global gene expression profile of whole blood in patients with coronary artery disease (CAD) showed significant upregulation of 343 genes and down regulation of 151 genes as compared to controls (p<0.05). There was predominant differential regulation of inflammatory and immune response genes as well as early growth response genes in our dataset. Of the ten candidate genes selected for validation by real time PCR in an independent cohort, CXCL1, EGR3, IL8, PTGS2 and CD69 genes were up regulated and IFNG and FASLG down regulated in cases relative to controls. We selected 13 patients with angiographically confirmed coronary artery disease (CAD) between ages 40 - 55 years and 11 population-based asymptomatic controls with normal ECG and matched for age, gendre and common risk factors such as diabetes and hypertension to that of the cases. Global gene expression profiling was performed on the Agilent microarray platform.

Whole genome transcriptomics of pre-access veins and hemodialysis arteriovenous fistula (AVF) samples from two-stage AVF patients with different maturation outcomes (matured vs failed)

Project description:We report the RNA expression profiles of native veins used for AVF creation and of remodeled AVF samples obtained 7-16 weeks later at the time of transposition (if vein matured) or salvage procedure (if vein failed). We perfomed RNA-seq on native veins and AVFs with different maturation outcomes (matured vs. failed). The "matured" and "failed" subgroups were similar in terms of demographics (age range, sex distribution, ethnicity distribution), clinical characteristics (proportion of diabetes mellitus and coronary artery disease, previous hemodialysis access history), and time interval between first-stage and second-stage surgeries.
