Clinical Trial Evaluating External Evaluation and Training on the Incomplete Resection (POLIPEVA Study)

ABSTRACT: Introduction It has been shown that some quality indicators in endoscopy can be improved through educational interventions. There are marked differences in the proportion of incomplete polypectomies among endoscopists. The effectiveness of measures to improve it has not been evaluated. Objective The main objective is to evaluate whether a training intervention or the notification of the individual proportion of incomplete polypectomies (those in which post-polypectomy biopsies of the resection margin show tissue other than normal mucosa) can improve this proportion. As secondary objectives, we will compare the proportion of fragmented polypectomies and adverse events. We will evaluate the factors associated with incomplete excision or failed cold polypectomy, as well as the individual evolution of the participants. Methods Non-pharmacological clinical trial involving endoscopists with> 1 year of experience and patients scheduled for colonoscopy. After each polypectomy, 2 additional biopsies will be taken and evaluated centrally by a blind pathologist. In a first phase, the basal rate of the participants will be evaluated. After it, the endoscopists will receive a course on endoscopic polypectomy and the other their rate of complete resection. The number of polyps required will vary depending on the number of endoscopists The primary objective will be compared using logistic regression models based on generalized estimating equations (GEE), taking into account the within-subject correlation.

The effect of cortex/medulla proportions on molecular diagnoses in kidney transplant biopsies: rejection and injury can be assessed in medulla

Project description:Histologic assessment of kidney transplant biopsies relies on cortex rather than medulla, but for microarray studies, the proportion cortex in a biopsy is typically unknown and could affect the molecular readings. The present study aimed to develop a molecular estimate of proportion cortex in biopsies and examine its effect on molecular diagnoses. Microarrays from 26 kidney transplant biopsies divided into cortex and medulla components and processed separately showed that many of the most significant differences were in glomerular genes e.g. NPHS2, NPHS1, CLIC5, PTPRO, PLA2R1, PLCE1, PODXL and REN. Using NPHS2 (podocin) to estimate proportion cortex, we examined whether proportion cortex influenced molecular assessment in the Molecular Microscope Diagnostic System. In 1190 unselected kidney transplant indication biopsies (Clinicaltrials.govNCT01299168), only 11% had <50% cortex. Molecular scores for ABMR, TCMR, and injury were independent of proportion cortex. Rejection was diagnosed in many biopsies that were mostly or all medulla. Agreement in molecular diagnoses in paired cortex/medulla samples (23/26) was similar to biological replicates (32/37). We conclude that NPHS2 expression can estimate proportion cortex; that proportion cortex has little influence on molecular diagnosis of rejection, and that, although histology cannot assess medulla, rejection does occur in medulla as well as cortex.

Expression data from gastroesphageal cancers and adjascent normal tissue

Project description:The primary objective of this study was to determine the effectiveness of bortezomib alone or in combination with irinotecan in patients with advanced gastric and gastroesphageal cancer. A secondary objecitve was to determine whether treatment was associated with changes in gene expression in the tumor and normal adjascent tissue. Tumor biopsies and biopsies of normal adjascent tissue were obtained before therapy and 24 hours after therapy. Differences in gene expression were evaluated between tumor and normal tissue (N=8 patients), and between post-treatment and pretreatment specimens for bortezomib alone (N=2 patients) and bortezomib plus irinotecan (N=10 patients).

Complete resection rate of polyps: long follow-up study

Project description:Interventions: Daily care: All included patients will undergo a colonoscopy. During colonoscopy all polyps will be removed, but per segment only one polyp will be included in the study: the most proximal polyp. Surveillance will be performed at intervals based on the Dutch gastroenterology guidelines (3 or 5 years). Study purpose: For the most proximal polyp detected, a single tattoo will be placed contralateral to the polypectomy site. During surveillance, location of previous polypectomy will be detected with help of the tattoo. If a residual polyp is detected at this polypectomy-site, this will be treated and tissue sent for histopathology. If no residual tissue is detected, several biopsies will be taken at the polypectomy site and sent for histopathology. Histopathologic evaluation of the residual tissue will be compared with histopathology of the original polyp.
