9 Minutes for Tandem Colonoscopy Withdrawal

ABSTRACT: A mean withdrawal time of at least 6 minutes has been considered to be one of the critical quality criterions of colonoscopy. Recently, our group completed a multicenter randomized controlled trial, which proved that prolonging the withdrawal time to 9 minutes could significantly improve the adenoma detection rate of colonoscopists, especially for young colonoscopists and proximal colon. However, it has some limitations in included participates (mixed indications for colonoscopy) and cannot illustrate the impact of withdrawal time on adenoma miss rate in a parallel randomized design. It is necessary to include tandem colonoscopy and adopt strict criteria of the screening population to confirm the effect of the 9-minute withdrawal time on the adenoma miss rate. Therefore, the investigators plan to conduct a multicenter, randomized controlled trial of tandem colonoscopy to compare adenoma miss rate of 6-minute and 9-minute withdrawal in screening population.

Assessing Effect of Withdrawal Time on Adenoma Detection Rate for Screening Colonoscopy

Project description:The investigators’ null hypothesis is that a withdrawal time of 9 to 10 minutes is non-inferior to a withdrawal time of 12 minutes or greater. Thus, the goal of this tandem design trial is to compare the additional diagnostic yield (# of missed lesions) for withdrawal times exceeding 10 minutes for screening/surveillance colonoscopies. Although withdrawal times longer than the standard 6-minute recommendation have been shown to be beneficial, there is limited prospective evidence investigating the benefit or lack thereof for withdrawal times greater than 9-10 minutes.

Transcriptomic profiles of advanced colorectal adenomas from 40 Korean patients

Project description:We report the RNA-seq data of 40 advanced colorectal adenoma patients form Dongguk University Ilsan International Hospital. The polyps with a diameter of 1cm or greater were regarded as advenced colorectal adenoma and obtained through colonoscopy. The data consist of 22 tublar adenoma, 6 tublovillous adenoma, 5 sessile serrated adenoma/polyp, 1 traditional serrated adenoma, intramucosal adenocarcinoma, neuroendocrine tumor, hyperplastic polyp, inflammatory polyp, high grade dysplasia, and atypical glands with adjacent hyperplastic mucosa.

Does Artificial Intelligence Improve Polyp Detection at Colonoscopy?

Project description:Interventions: Usual standard of care from the endoscopist doctor, with addition of Endo-AID (Olympus Corporation) AI module to be on during withdrawal phase of colonoscopy. -Usual standard of care: single colonoscopy, which may take between 3-30 minutes. During withdrawal phase, which may take between 6 and 60 minutes, the AI module will be activated. It is not known if use of this module will increase the withdrawal time (compared to standard of care, without it). -The nurses will advise the doctor of the randomisation (AI or standard of care), and turn on the AI device once withdrawal has commenced. -The AI will analyse the video output from the scope to automatically detect suspected polyps in real-time during withdrawal. Polyps would then be further analysed and resected as per usual standard of care. Primary outcome(s): ADR (Adenoma Detection Rate) - proportion of patients with at least one histologically proven adenoma or carcinoma. Histology will be assessed using standard procedure in the laboratory. Project description:The tubular adenoma sample cohort (accrued by Janssen Pharmaceuticals) consisted of 127 high-risk baseline adenoma samples acquired retrospectively by Avaden Biosciences. High risk patients are defined here as presenting a tubular adenoma ? 10mm, 3 or more adenomas, adenomas with villous histology or adenomas showing high-grade dysplasia based on Lieberman et al. Patients with familial adenomatous polyposis (FAP) and Lynch syndrome were excluded. All patients received at least one baseline colonoscopy for which clinical and pathological records were available. Adenoma tissue was available as archival, formalin-fixed, paraffin-embedded (FFPE) blocks.
