E-Motivación: Developing and Pilot Testing an App to Improve Latinos’ Screening Colonoscopy Rates

ABSTRACT: Among Latinos, a fast growing and underserved population, Colorectal Cancer (CRC) is the third leading cause of cancer death in men and in women. Compared to non-Latino whites, Latinos are less likely to be diagnosed with localized CRC and more likely to be diagnosed with advanced stage disease. Of the recommended CRC screening tests, a colonoscopy allows for both the detection and removal of precancerous and cancerous polyps. Although screening colonoscopies can detect and prevent CRC, more than half of Latinos have not received a screening colonoscopy within the recommended time frame (one screening colonoscopy per ten years). The purpose of this study is to develop and begin to test an electronically-delivered motivational interviewing progressive web application (e-MI app), called Motívate. The Motívate app will be offered in both English and Spanish to patients who self-identify as Latino/a/x, after they receive a referral for a screening colonoscopy.

Internet-Based Motivational Interviewing for Colonoscopy

Project description:Compared to other racial groups, African Americans have the highest colorectal cancer (CRC) morbidity and mortality rates. Although colonoscopies can prevent CRC, nearly one third of African Americans have not received a screening colonoscopy within the recommended time frame (one colonoscopy per ten years). It is critical to increase African Americans’ screening colonoscopy rates in order to reduce racial inequities in CRC morbidity and mortality. Previous research suggests that a motivational interviewing based intervention may help improve screening colonoscopy uptake. This study will conduct a randomized clinical trial examining the efficacy of a motivational interviewing informed tablet app, called e-Motivate, to improve African Americans’ screening colonoscopy uptake. Participants will be African Americans who receive a referral for a screening colonoscopy. Participants (N=200) will be randomly assigned to one of two groups: (1) usual care group (N=100); or (2) e-Motivate app group (N=100). Participants in the usual care group will receive standard clinical care which includes patient navigation (e.g., scheduling, reminder calls). Participants in the e-Motivate group will complete the e-Motivate app in the clinic immediately after they receive a referral for the screening colonoscopy. The participants in the e-Motivate app group will also receive usual care. Six months following the initial referral, participants’ medical charts will be reviewed to determine whether the participants completed the recommended screening colonoscopy. Secondary outcomes (e.g., bowel prep quality, number of cancellations) will also be analyzed.

Data-Independent Acquisition Analysis to Identify Serum Biomarkers and Neoantigens for Differentiating Colorectal Cancer from Inflammatory Bowel Disease

Project description:Early detection of colorectal cancer (CRC) and distinguishing inflammatory bowel diseases from cancerous conditions significantly improve survival rates. While colonoscopy is the gold standard for CRC detection, it is invasive and associated with potential complications. Therefore, there is a need for blood biomarkers that possess high sensitivity and specificity which can be used for pre-diagnosis. In this study, we screened for serum biomarkers specifically altered in colonoscopy-positive (CP) patients (cancer group) when compared to colonoscopy-negative (CN) patients (suffering from other bowel-related illnesses with symptoms similar to CRC), and healthy control (HC) individuals (n=20 each). Using Data-Independent Acquisition-Mass spectrometry (DIA-MS) analysis, we identified potential protein markers and neoantigens uniquely altered in the CP group compared to the CN group.

Time Lapse to Cancer-Defining the Transition from Polyp to Cancer

Project description:<p>In the US, CRC incidence has declined with uptake in colonoscopy for early detection and removal of polyps, the precursor lesion, can stop a cancer from developing but in spite of this, CRC remains the second leading cause of cancer death in the United States. One third of people who undergo screening colonoscopy will have adenomatous polyps, but less than 5% of the time are these polyps presumed to go on to develop into cancer. Why does one polyp develop into cancer while another one that looks very similar does not. In this proposal we will identify what molecular genetic changes in the genome, in the mRNA expression and genetic methylation patterns will distinguish a polyp that has already transformed into cancer from one that has not.</p>

Whole Blood Gene Expression Analysis of Controls and CRC Patients

Project description:Colorectal cancer is the third most common malignancy and the fourth most common cause of cancer mortality worldwide. In 2008, more than one million cases were newly diagnosed, and more than 600,000 people died from the disease. Given its slow development from removable precancerous lesions and curable early stages, screening for CRC has the potential to reduce both the incidence and mortality of the disease. However, compliance with current screening methods remains poor and there is a clear need for an accurate in vitro blood test to increase participation in colorectal cancer screening. In this study, we performed genome-wide gene expression profiling of peripheral blood samples from 100 healthy controls and 100 colorectal cancer patients using PAXgeneTM technology and Affymetrix GeneChip® microarrays. We show that monitoring gene expression in blood results in distinct transcriptional profiles between the controls and cancer patients. Thus, the microarray-based blood gene expression profiling holds great promise for developing novel biomarkers for colorectal cancer detection.

Gene expression data from very early stage lesions (T1) of colorectal cancer

Project description:There are ~1.4 million cases of colorectal cancer (CRC) annually worldwide. Due to recent implementation of bowel cancer screening (BCS), incidence of diagnosis with stage I (T1/2, N0, M0) CRC has increased nearly four-fold; from approximately 12% pre-screening to approximately 42-45%. While survival rates for patients diagnosed with stage I CRC remains above 95%, and identification of patients at this early stage provides the best opportunity to improve in cancer survival rates, recent data has provided quantitative evidence that metastatic seeding can occur from disseminated cells at the very earliest stage of CRC development. The aim of study was to give an insight into aggressive “born-to-be-bad” biology through generation and transcriptional profiling of a novel case-controlled retrospective T1 CRC discovery cohort. We performed transcriptional and histological profiling to identify molecular pathology features of the high-risk metastatic phenotypes, discriminating between relapse and non-relapse T1 lesions and found that a cohort of aggressive early disseminating stage 1 CRC is associated with high cell intrinsic TGFB signalling and not stromal signalling.

Expression data for tumor and normal tissue samples from deceased sepsis and non-sepsis colorectal cancer patients

Project description:The mortality risk from cancer-associated sepsis is differentially affected by individual cancer sites and host responses to sepsis are heterogenous. Native Hawaiians have a 2-fold higher mortality risk from cancer-associated sepsis than Whites and also higher mortality risk from colorectal cancer (CRC). We investigated this disparity by examining ethnic variation in the transcriptome of patients with CRC-associated sepsis and its relation to survival and genetic diversity. We conducted transcriptomic profiling of CRC tumors and adjacent non-tumor tissue obtained from adult patients of Native Hawaiian and Japanese ethnicity who died from cancer-associated sepsis. We examined differential gene expression in relation to patient survival and sepsis disease etiology.
