The influence of two policies of closure of the laparotomy wound in the rate of surgical site infection in colorectal cancer surgery: the New Operation trial

ABSTRACT: Study hypothesis: A decreased faecal load in the surgical wound diminished the incidence of surgical site infection. Primary outcome(s): Incidence of surgical site infection. The criteria to define surgical site infection are described by Disease Control and Prevention National Healthcare Safety Network (Spain). For the purposes of this study superficial and deep wound infections would be considered as a unique entity condition. This will be measured 30 days after the operation.

Infection induced inflammation impairs wound healing through IL-1b signaling

Project description:Wound healing is impaired by infection; however, how microbe-induced inflammation modulates tissue repair remains unclear. We took advantage of the optical transparency of zebrafish and a genetically tractable microbe, Listeria monocytogenes, to probe the role of infection and inflammation in wound healing. We found a critical window of microbial clearance necessary to limit persistent inflammation and enable efficient wound repair. Infection with bacteria engineered to activate the inflammasome, Lm-Pyro, induced persistent inflammation and impaired healing despite low bacterial burden. In contrast, infection with an anti-inflammatory, apoptosis inducing strain, Lm-Apo, had similar infectious burden but was associated with rapid wound repair. Inflammatory infections induced il-1b expression and blocking IL-1R signaling partially rescued wound healing in the presence of persistent infection. Taken together, our findings suggest that the dynamics of microbe-induced tissue inflammation impacts repair in complex tissue damage independent of bacterial load, with a critical early window for efficient tissue repair.

The Effect of Wound Problems Wound Dressing in Patients With Colorectal Cancer Surgery

Project description:This study was conducted as a randomized controlled trial in order to determine the effect of prophylactic negative pressure wound therapy for the prevention of surgical site complications in high-risk colorectal cancer surgery. Hypothesis: Prophylactic negative pressure wound therapy applied after open colorectal cancer surgery to high-risk patients affects surgical wound complications. pNBYT group: The study was completed with a total of 50 patients, 24 of intervention group anda 26 of the control group, who met the inclusion criteria at the surgical oncology service of a university hospital. The data were collected using Patient Identification Form, Surgical Procedure Form, Wound Follow-up Chart and ASEPSİS Wound Scoring System. Ethics committee approval and written informed consent of the individuals was taken in the research. The data were analyzed in SPSS Statistics 24.0 program using Shapiro Wilk test and Q-Q graphs, Independent Sample t test, Mann Whitney U test, Chi-square, Cochran’s Q and Friedman test. The value of p<0.05 was accepted for the statistical significance level. It was determined that the groups were similar in terms of identification and surgical procedure characteristics.

Peptidomic analysis of bacterial protease activity in acute wound fluids

Project description:The clinical assessment of wound infection and related complications is challenging and the development of objective methods to measure wound status and predict healing outcomes is therefore essential. As bacterial protease activities play important roles in infection, analysis of changes in the wound peptidome by individual bacterial enzymes could provide insight into proteolytic events occurring in infected wounds, and may aid in the discovery of biomarkers. Wound infection was mimicked by incubating sterile acute wound fluids ex vivo with the bacterial proteases Pseudomonas aeruginosa LasB and Staphyloccocus aureus V8. Using a peptidomics approach, we characterized the low-molecular-weight peptidome of the digested samples and identified over 100 protein targets for each enzyme. Additionally, we found enzyme-specific peptides and digestion patterns.

The impact of bacterial binding dressings following colorectal surgery on surgical wound complications in an adult population.

Project description:Interventions: Trial participants will be allocated to either the intervention or control group. There is currently only one DACC dressing (Leukomed Sorbact) available in the wound care dressing. The intervention group will receive the DACC containing dressings following surgery (Leukokmed Sorbact). The DACC dressing is a cover dressing applied over the incision site The control group will receive a dressing that does not contain DACC and has the same dressing properties without the interventional dressing characteristics. Participants allocated to the control group will receive a non DACC dressing. The wear time for both groups will be 14 days, unless otherwise clinical indicated for a dressing change and assessment by the lead PI. Dressing application and wear time is to be conducted according to the manufacturer specifications. Dressings will be applied as per standard of post operative care. Dressings will be visually checked daily by the study lead wound care clinician. Primary outcome(s): The primary outcome will be determined by clinical assessment and diagnosis as well as patient reported outcome. Post discharge surveillance using telemedicine and a phone call at 3o days will be conducted on all trial participants. The primary outcome measurement is defined as the occurrence of surgical site infection as per Centres of Disease Control definition and coding ICD-11 T81.49. [Day 30 postoperative ];The primary outcome measurement is defined as the occurrence of surgical wound dehiscence as defined by World Union of Wound Healing Societies definition and coding ICD 11: T81.3. Post discharge surveillance will be conducted using telemedicine and a phone call at 30 days after surgery.
