Population testing of an internet-based Personalised Decision Support system for Colorectal Cancer screening

ABSTRACT: Interventions: Participants in the intervention conditions receive ONE of ethier an internet-based tailored decision support intervention, or internet-based non-tailored decision support intervention. In each intervention, participants answer questions concerning their perceived susceptibility to Colorectal cancer (CRC) and attitudes towards CRC screening. Tailored responses are then generated for the ‘tailored group’ which are designed to correct misaprehensions where they exist. For the non-tailored group participants do not receive tailored messages but rather generic information brochures concerning Colorectal cancer. This intervention is accessed once only and participants work through it at their own pace. The overall duration of the study will be approximately 12 months. Primary outcome(s): Number of participants completing a colorectal cancer screening test per group. All participants receive a free Colorectal Cancer screening test and the number of kits returned per study arm is recorded upon return of the kit[approximately one year after randomisation] Study Design: Purpose: Prevention; Allocation: Randomised controlled trial

An inhaled dose of budesonide induces genes involved in transcription and signaling in human airways

Project description:Inhaled corticosteroids (ICS) control airway inflammation in mild to moderate asthma by reducing inflammatory gene expression. However, incomplete understanding of the molecular mechanisms underpinning corticosteroid action hinders development of improved therapies for more severe disease. Microarray analysis was performed on RNA from biopsies taken from healthy individuals after receiving single dose of ICS to characterize corticosteroid-induced modulation of gene expression in the human airways. Healthy male, non-smoker, non-allergic volunteers (age 18-50 years) with normal lung function were recruited into a prospective, double-blind, placebo-controlled, randomized, two-period crossover study involving an initial screening visit, followed by two intervention visits. Participants were screened at visit 1 for fulfilment of the study eligibility criteria. At visit 2, volunteers were randomized to receive inhaled budesonide (1600 Âµg) or placebo, both via Turbuhaler. Two to three weeks later, at visit 3, participants received either budesonide or placebo, as appropriate to complete both study arms. Six hours after placebo or a budesonide inhalation, bronchial biopsies were obtained via bronchoscopy.

Shifts in the Immunoepigenomic Landscape of Monocytes in Response to a Diabetes-Specific Social Support Intervention: A Pilot Study Among Native Hawaiian Adults with Diabetes

Project description:Genome-wide DNA methylation profiling of bead-enriched total monocytes collected from Native Hawaiian participants with known type 2 diabetes mellitus enrolled in a 3 month diabetes-specific social support education intervention. DNA methylation profiling was performed across ~450,000 CpGs from monocytes using the Illumina Infinium HumanMethylation450 BeadChip. Samples included 8 participants with paired DNA methylation data collected at pre-intervention and post-intervention (3 months), and 2 non-diabetic donors.

Comparing Interventions to Increase Colorectal Cancer Screening

Project description:The purpose of this study is to test different approaches to help people understand the purpose of colorectal cancer (CRC) screening, two screening test options available, and the barriers to screening so they can make informed decisions about CRC screening. Participants will be randomly assigned to one of three groups: (1) one group will receive a tailored digital video disc (DVD) in the mail; (2) another group will receive the mailed DVD plus telephone calls from a patient navigator; and (3) the third group will receive the care normally provided by the healthcare system’s endoscopy department. The investigators hypothesize the following: (1) participants who receive the tailored DVD plus the patient navigation intervention will have higher rates of CRC screening with the fecal immunochemical test (FIT), colonoscopy, or either screening test compared to those who receive the tailored DVD alone; (2) participants who receive either intervention (DVD only or DVD plus patient navigation) will have higher rates of CRC screening with FIT, colonoscopy, or either screening test than those who receive usual care; and (3) participants who receive either intervention who complete colonoscopy will have better quality of bowel preparation, less anxiety about the procedure, and greater satisfaction with the colonoscopy experience than those who receive usual care.

ICIBS Trial - Improving Patient Information About Bowel Cancer Screening - a Decision Aid Trial

Project description:This project has been completed and consisted of a randomised trial of six tailored decision aids giving patients evidence-based information about faecal occult blood test screening for bowel cancer. 314 Australians aged between 50-74 years were recruited from five general practices and randomised to received either the tailored decision aid with age-gender and family history specific information and values clarification exercise or a standard government information sheet. The decision aid significantly increased the proportion of people who were informed participants in the screening program.

Increasing Colorectal Cancer (CRC) Screening Among Hispanic Primary Care Patients

Project description:This study is designed to test a standard mailed intervention (SI) versus a novel decision support and navigation intervention (DSNI). Working with patients and stakeholders, this randomized trial will include 400 Hispanic men and women who are 50 to 75 years of age, patients in community based primary care practices, and are eligible for CRC screening. We will consent, survey, and randomize participants either to the SI Group (n=200) or the DSNI Group (n=200). Study specific aims are to: (1) Assess intervention (DSNI versus SI) impact on overall CRC screening adherence; (2)Assess intervention (DNSI versus SI) impact on CRC screening decision stage; (3) Assess intervention (DNSI versus SI) impact on test-specific CRC screening adherence; and (4) Assess intervention (DNSI versus SI) impact on CRC and knowledge and perceptions.

Participants are identified who reported having had an immunisation with Boostrix within the past 5 years (characteristics: recent boostrix = 1).
