ABSTRACT: Primary objectives: The first primary objective of the study is to compare the postoperative complication rate according to Clavien score (> grade 1) of a perioperative chemotherapy with a postoperative regimen. A second primary objective of the study is to compare for the patient subgroup with >3 liver metastases or at least one metastasis ≥ 5 cm in diameter the median disease free survival. Primary endpoints: The first primary objective of the study is to compare the postoperative complication rate according to the Clavien score (> grade 1) (Appendix 2) of a perioperative chemotherapy with a postoperative regimen. A second primary objective of the study is to compare for the patient subgroup with > 3 liver metastases or at least one metastasis ≥ 5 cm in diameter the median disease free survival between the two treatment arms

Self-Packed Capillary 300 µm Columns for Deep and Sensitive Proteome Analysis.

Project description:Nanoflow chromatography remains the standard workflow for proteomics. Capillary flow chromatography applications are mainly limited to fast acquisitions, likely due to the absence of commercially available long (> 15 cm), 300 µm inner diameter columns. We present a methodology for packing 300 µm diameter columns of any desired length and systematically optimising several capLC parameters. Utilizing CSH chemistry, known for its ability to maintain excellent peak shapes even under high loading conditions, we systematically tested various bead sizes and column lengths. Specifically, we examined 30 cm columns packed with 1.7 µm beads, 55 cm columns with 2.5 µm beads, and 1 m columns using 3.5 µm beads. We also varied the gradient lengths from 30 minutes to 10 hours. Our results demonstrated outstanding performance with the 30 cm columns packed with 1.7 µm beads on a 2-hour gradient, achieving a median full width at half maximum of 7.4 seconds and a peak capacity of 530. We also show that the columns can handle high sample amounts in 0.5-8 µg range without loss of separation power. Finally, we systematically investigated the effect of gradient length and loading and demonstrate that we can quantify close to 9000 proteins and 124 thousand unique peptides on Exploris 240 system, analysing 4 µg of HeLa digest material on a three hour gradient. We demonstrate that the performance of the presented columns approaches or surpasses the most efficient commercially available columns.

INCREASED HEAT SHOCK PROTEIN 70 EXPRESSION BY CM-695 ASSOCIATES WITH STRONG ANTITROMBOTIC EFFECT WITHOUT INCREASING BLEEDING

Project description:BACKGROUND Antithrombotic medications target coagulation factors. Their use is associated with an increased bleeding risk. Safer drugs are needed. We described that the heat shock protein 70 (Hsp70) exhibits antithrombotic properties that do not influence bleeding. OBJECTIVES By using murine models, we want to test the hypothesis that overexpressing Hsp70 with CM-695, a dual inhibitor of HDAC6 and phosphodiesterase 9, protects against thrombosis while leaves bleeding tendency unaltered. METHODS CM-695 was used to induce Hsp70 overexpression. Hsp70 overexpressing mice were submitted to three thrombosis-triggering procedures. The ferric chloride carotid artery model was used to compare the antithrombotic role of CM-695 and rivaroxaban, a direct oral anticoagulant. The mouse tail transection model was used to compare the bleeding tendency upon CM-695 or rivaroxaban administration. RESULTS Intraperitoneal (i.p.) 20 mg/kg CM-695 increased Hsp70 expression markedly in the murine aortic tissue. This treatment delayed thrombosis in the collagen/epinephrine [P=0.04 (Log-Rank test), n=10], Rose Bengal/laser [median vessel occlusion time (OT): 58.6 vs. 39.0 minutes (min) in the control group (CG), P=0.008, n≥10] and ferric chloride (OT: 14.7 vs. 9.2 min in the CG, P=0.032, n≥10) models. I.p. 80 mg/kg CM-695 (n≥9) and intravenous 3 mg/kg rivaroxaban (n≥8) significantly delayed thrombosis. CM-695 did not induce bleeding [median bleeding time (BT): 8.5 vs. 7.5 min in the CG, n≥10]. However, this was dramatically increased by rivaroxaban (BT: 30.0 vs. 13.7 min in the CG, P=0.001, n=10). CONCLUSIONS CM-695 is a new antithrombotic drug devoid of bleeding risk that may be envisioned as a useful clinical tool.

Longitudinal Profiling in Patients Undergoing Cardiac Surgery Reveals Postoperative Changes in DNA Methylation

Project description:Cardiac surgery and cardiopulmonary bypass induce a substantial immune and inflammatory response, the overactivation of which is associated with significant complications. Longitudinal DNA methylation profiling allows the potential to identify changes in gene regulatory mechanisms that are secondary to surgery and to identify molecular processes that predict and/or cause postoperative complications. In this study, we measure DNA methylation in preoperative and postoperative whole blood samples from 96 patients undergoing cardiac surgery on cardiopulmonary bypass. We identify several loci with statistically significant postoperative changes in methylation. Additionally, two of these loci are associated with new-onset postoperative atrial fibrillation, a significant complication after cardiac surgery. This research establishes that there are statistically significant changes in DNA methylation that occur immediately after cardiac surgery and that these acute alterations in DNA methylation have the granularity to identify processes associated with major postoperative complications.

Gene expression profiling of early primary breast cancer to identify prognostic markers and associated pathways

Project description:Retrospective series of primary breast cancer patients who received surgery between 1989 and 1992. Patients received adjuvant chemotherapy and/or adjuvant hormone therapy, or no adjuvant treatment. Tamoxifen was used as endocrine therapy for 5 years in ER+ BC patients. Patients who were <50 years of age, with lymph node positive tumors, or ER– and/or >3 cm in diameter, received adjuvant cyclophosphamide, methotrexate, and 5-fluorouracil (CMF) for six cycles, in a thrice weekly intravenous regimen. Patients >50 years of age with ER–, lymph node–positive tumors also received CMF.

Human Femoral Artery Proteome and 2-Hydroxyisobutyrylation Analysis in Metabolic Syndrome Induced Restenosis

Project description:Restenosis is the primary complication following stenting for coronary and peripheral arterial disease (PAD), posing an ongoing clinical challenge. Metabolic syndrome (MetS), characterized by metabolic disturbances, has been identified as an independent predictor for postoperative restenosis in coronary and carotid arteries, potentially due to endothelial dysfunction and augmented oxidative stress in cells, while its specific regulatory mechanism is still largely unknown. Lysine 2-hydroxyisobutyrylation (Khib), a recently identified post-translational modification, plays a crucial role in transcriptional regulation and cellular metabolism. However, there is a lack of comprehensive analysis of the proteome and Khib modifications within restenotic vessels in the context of MetS, as well as in the understanding of the associated pathophysiology. Here, we firstly validate changes in expression and distribution of Khib in vascular tissues in clinical tissue and cell models. Subsequently, we utilized proteomics and modifiomics to examine the protein and Khib modification characteristics in MetS-induced restenosis.
