Investigation of continence, sexual- and bowel function after radical surgery for bladder cancer

Project description:Background and study aims Bladder cancer is one of the most common types of cancer worldwide. If the cancer has spread particularly aggressively or keeps coming back after initial treatment, a radical cystectomy may be required. A radical cystectomy is an operation where the entire bladder is removed, as well as nearby organs that may contain cancer cells, such as the prostate and vas deferens (tubes that transport sperm) in men, and the uterus and ovaries in women. Although total removal of the bladder is an effective treatment against the cancer, patients often experience long-term side effects from the surgery. One of the most pronounced after effects are problems with functional problems, namely sexual problems and urinary incontinence. Another common problem following a radical cystectomy is difficulties with bowel movements, such as difficulty passing stools or needing to go urgently. The aim of this study is to look at defecation (rectal function) in patients before and after their radical cystectomy surgery. Who can participate? Adults suffering from bladder cancer, with a planned radical cystectomy. What does the study involve? Before the planned surgery, participants are asked to complete questionnaires regarding their sexual function and continence. Additionally, pressure measurements in the rectum are taken so that rectal function can be determined. Twelve months after the patients have had their radical cystectomy, the questionnaires and function tests are repeated to find out whether the results have changed.

Discovery and validation of a novel expression signature for recurrence in high-risk bladder cancer post-cystectomy

Project description:Purpose: Selecting muscle-invasive bladder cancer patients for adjuvant therapy is currently based on clinical variables with limited power. We hypothesized that genomic-based signatures can outperform clinical models to identify patients at higher risk. Method:Transcriptome-wide expression profiles were generated using 1.4 million feature-arrays on archival tumors from 225 patients who underwent radical cystectomy and had muscle-invasive and/or node-positive bladder cancer. A 15-feature GC was developed on the discovery set with area under curve (AUC) of 0.77 in the validation set. A cohort comprised of 225 patients with organ-confined, muscle-invasive (pT2N0M0),extravesical (pT3-4aN0M0), and node-positive (pTanyN1-3M0) UCB who underwent radicalcystectomy at the University of Southern California between 1998 and 2004 was used. Each patient had aminimum two-year follow-up post-cystectomy unless they recurred prior to that date. Patients receiving neoadjuvant chemotherapy, and those with clinical evidence of lymphadenopathy or distant metastasis at diagnosis were excluded.

Divergent biological response to neoadjuvant chemotherapy in muscle-invasive bladder cancer

Project description:Despite cisplatin-based neoadjuvant chemotherapy (NAC) 60% of patients with muscle-invasive bladder cancer still have residual invasive disease at radical cystectomy (RC). The NAC-induced biological alterations in these cisplatin-resistant tumors remain largely unstudied. We analyzed gene expression from 133 patients with residual invasive disease after cisplatin-based NAC. H&E and immunohistochemistry were used to confirm tissue sampling and gene expression analysis. Established molecular subtyping models proved to be inconsistent in their classification of the post-NAC samples. Unsupervised consensus clustering revealed four distinct consensus clusters (CC). The CC1-Basal and CC2-Luminal subtypes expressed genes consistent with a basal-like and a luminal-like phenotype, respectively. The CC3-Immune subtype had the highest immune activity, including T-cell infiltration and checkpoint molecule expression, but lacked both basal and luminal markers. The CC4-Scar-like subtype expressed genes associated with wound-healing/ scarring and was associated with favorable prognosis.

Molecular changes during progression from early stage to advanced urothelial carcinoma

Project description:It is unclear how urothelial carcinoma of the urinary bladder change, genetically and phenotypically, as patients progress from recurrent non muscle-invasive (NMI) to muscle-invasive (MI) disease or disease that require radical cystectomy. This data set contains gene expression data from 200 samples in a longitudinal study of such patients. Overlap with previously GEO-published cohorts from our lab is described in the metadata.

Glutathione dynamics is a potential diagnostic and therapeutic trait determining the neoadjuvant chemotherapy response to bladder cancer

Project description:Radical cystectomy with preoperative cisplatin-based neoadjuvant chemotherapy (NAC) is a current standard care for muscle-invasive bladder cancers (MIBCs). However, the response rate remains relatively low, and current clinico-pathologic and molecular classifications are inadequate to prospectively stratify MBIC patients regarding the NAC response. Here, we showed that dysregulation in glutathione (GSH) pathway is fundamental for the NAC resistance of MBICs. Comprehensive analysis of transcriptome profiles from four independent cohorts revealed that GSH metabolism and immune response genes were significantly enriched in NAC resistant and sensitive MIBC tumors, respectively. A machine learning based tumor/stroma classifier was applied for high-throughput digitalized immunohistochemistry analysis, identifying GSH dynamic proteins including glutaminase-1 (GLS1) were associated with the NAC resistance and unfavorable clinical characteristics. Consistently, GSH dynamics was activated in cisplatin resistant human MIBC cells, stimulating their proliferation, stemness features, and invasion. Combination treatment of GSH dynamics modulators and cisplatin significantly suppressed tumor growth in an orthotopic xenograft animal model. Our results demonstrate the prognostic and therapeutic values of GSH dynamic mediators in determining the clinical features and response to NAC of patients with MIBCs.
