Project description:Surgical treatment of colorectal diseases typically involves excising the diseased portion of the bowel and anastomosing the remaining sections to re-establish continuity. Surgical suturing has limitations in preventing anastomotic leakage and postoperative adhesion. To address these challenges, a tissue-adhesive, antibacterial, and antioxidant hydrogel was designed to cover and seal colorectal anastomotic wounds. The hydrogel was formed in situ by simply mixing oxidized hyaluronic acid (OHA), adipic acid dihydrazide-modified hyaluronic acid (HA-ADH), ε-poly-L-lysine (ε-PLL), and tannic acid (TA). The hydrogel exhibited a rapid gelation rate and self-healing ability. Compared with commercial fibrin glue, the hydrogel had superior tissue adhesive strength and wound sealing performance. The hydrogel displayed potent reactive oxygen species scavenging ability and antibacterial activity against both gram-positive and gram-negative bacteria. The hydrogel also exhibited good biodegradation and biocompatibility. In a cecum-abdominal wall adhesion model in rats, the hydrogel attached firmly to the injured tissues and served as a physical barrier to prevent adhesion formation. In anastomotic leakage models after colon resection in rats and rabbits, the hydrogel effectively sealed the anastomotic leakage, prevented postoperative adhesion, and promoted anastomotic healing. Thus, this multifunctional hydrogel has strong clinical potential for preventing anastomotic leakage and adhesion formation after colorectal surgery.

Project description:AIMS: To explore and validate the utility of rumen endoscopy for collection of rumen papillae for gene expression measurement. METHODS: Four adult Coopworth ewes were fasted for either 4 or 24 hours. Animals were sedated, placed in a dorsally recumbent position at 45 degrees with the head upright, and an endoscope inserted via a tube inserted into the mouth. Biopsies of rumen papillae were taken from the ventral surface of the rumen atrium under visual guidance. Two biopsies were collected from one of the animals that had been fasted for 4 hours, and three from one of the animals that had been fasted for 24 hours. Video of the rumen atrium and reticulum was also collected. The animals recovered uneventfully. Biopsies were subsequently used for extraction and sequencing of mRNA. RESULTS: The ventral surface of the rumen atrium was accessible after 4 hours off pasture, but a larger region was accessible after 24 hours of fasting. Sedation allowed access for endoscope use for around 5 to 10 minutes after which increased saliva flow was noted. Rumen papillae biopsies were easily collected, with samples from a variety of sites collected in the ∼10 minute time window. High quality RNA was obtained for stranded mRNA sequencing. Of the resulting reads, 69–70% mapped uniquely to version 3.1 of the ovine genome, and 48–49% to a known gene. The rumen mRNA profiles were consistent with a previously reported study. CONCLUSIONS: This method for obtaining rumenal tissue was found to be rapid and resulted in no apparent short or long term effects on the animal. High quality RNA was successfully extracted and amplified from the rumen papillae biopsies, indicating that this technique could be used for future gene expression studies. The use of rumen endoscopy could be extended to collection of a variety of rumen and reticulum anatomical measurements and deposition and retrieval of small sensors from the rumen. Rumen endoscopy offers an attractive and cost effective approach to repeated rumen biopsies compared with serial slaughter or use of cannulated animals.

Project description:Anastomotic leakage is a major complication after gastrointestinal surgery, yet early detection remains limited by nonspecific clinical signs and narrow biomarker panels. Here, we used longitudinal data-independent acquisition proteomics to profile postoperative drain fluid from two surgical cohorts, comprising 34 pancreatic surgery patients with 222 drain-fluid samples and 34 colorectal surgery patients with 146 drain-fluid samples. We aimed to define procedure-specific leakage signatures, evaluate established biomarkers across postoperative time, and identify additional candidate markers beyond routinely measured proteins. In pancreatic surgery, established enzyme markers, particularly AMY2A and PNLIP, showed strong early discrimination between leak and non-leak patients. In colorectal surgery, the same predefined marker strategy performed inconsistently, indicating that leakage-associated biology may differ between surgical procedures. Discovery analysis revealed divergent temporal patterns. Pancreatic leakage was dominated by early enzyme enrichment followed by inflammatory, antimicrobial, oxidative stress, and matrix-remodelling-associated proteins. Colorectal leakage showed strongest divergence around POD3 and was driven largely by reduced abundance of proteins linked to uncomplicated recovery, including endothelial integrity, extracellular matrix organization, complement regulation, and coagulation balance. From these procedure-specific patterns, PRSS2 and AZU1 were selected for pancreatic surgery, and FBLN1 and CDH5 for colorectal surgery, as novel candidate markers for targeted follow-up. Targeted ELISA measurements confirmed detectability of selected candidates in drain fluid, supporting their further development as markers for novel leakage detection strategies. Overall, these data show that postoperative leakage follows procedure-specific proteomic trajectories and support drain-fluid proteomics as a framework for biologically informed biomarker discovery beyond routinely measured markers.

Project description:Anastomotic leakage is a major and potentially mortal complication with an incidence of 10-13% after resection of the rectum. For patients showing no clinical signs of peritonitis, the traditional method has been a conservative treatment with transrectal rinsing. This treatment is often associated with a very protracted postoperative course with healing times of up to a year or more for the anastomotic leakage.
Treatment with vacuum drainage (VD) is a new method primarily developed for wound therapy.
The objective of this study is to investigate the effects of transrectal vacuum treatment on the healing of anastomotic leakage after rectum resection in a prospective, randomized, controlled multicentre trial in 60 patients found to develop clinically significant anastomotic leakages after elective rectal resection.

Project description:In colorectal surgery, hand suturing and stapling are routine methods for performing intestinal anastomoses, and these methods appear to be similar in terms of clinical safety. Despite several years of experience with surgical procedures as well as improvements to the medical devices, problems with disturbed anastomotic healing leading to leakage and stenosis after colorectal surgery remain a significant challenge for surgeons. A frequency of anastomotic leakage after stapling or suturing has been reported to range from 3% to 20%. In addition, preoperative radiotherapy has been shown to increase the risk of anastomotic leakage even further.
The methods that are used today to detect leakages are unfortunately inaccurately and limited to monitoring symptoms, temperature, CRP-levels, and performing abdominal examinations and CT-scans. These clinical signs and parameters usually become apparent several days after onset of the leakage, which leads to a delayed diagnosis. Anastomotic leakage is not only a significant cause of increased morbidity of complications and mortality in patients, but also associated with increased risk of local recurrence and poor prognosis. Moreover, when reoperation is required to fix the leakage, a permanent stoma may be made at the level of the sigmoid colon and this procedure impacts live quality of patients negatively.
Based on the above considerations, a novel, adaptive anastomotic method has been developed by CarpoNovum to achieve a safer anastomosis. The method’s working name is C-REX Ring-locking Procedure. C-REX is referred to our Colorectal anastomotic rings for Re-join the intestinal ends and validate the anastomosis, with function of Extract samples for analysis and conduct X-ray through connected catheters.
The novel adaptive anastomotic medical devices, C-REX LapAid and C-REX DMH/DMHC are easy to use, with unique possibility to control the anastomosis during and after surgery. The previous successful preclinical study encourages a clinical verification in patients undergoing colonic resection to evaluate the safety and performance of C-REX Ring-locking Procedure by use of C-REX LapAid and C-REX DMH/DMHC.

Project description:Anastomotic leakage is serious morbidity that can develop in patients operated on for colorectal cancer and can reach potentially life-threatening dimensions. Many international studies have been conducted to reduce and eliminate this postoperative complication that may have a mortal course. In these studies, preoperative, perioperative and postoperative factors of the patient, operation techniques, structure of the material used in the operation and multiple factors belonging to the surgeon were held responsible. Intraabdominal sepsis secondary to late anastomotic leakage and subsequent multiorgan failure can cost the patient’s life. Anastomotic leaks that develop in patients who have been operated for colorectal cancer; In order to detect patients’ postoperative clinical findings, laboratory examinations, imaging tests, and to eliminate them before intraabdominal sepsis develops, studies including many different laboratory and imaging methods have been carried out. Although previous studies have shown that there are many laboratory examinations and imaging methods that can predict anastomotic leaks early, they have many advantages over each other in terms of efficiency, sensitivity, specificity, and cost.
The investigators aimed to investigate the effectiveness of C reactive protein and blood sodium value, as well as their superiority, among the tests that can predict postoperative anastomotic leakage, especially in patients who have undergone a single anastomosis following resection for non-metastatic colorectal cancer.
