Use of lymph-circulating extracellular vesicles as surrogate markers of melanoma progression and BRAFV600E mutation

Project description:Liquid biopsies are becoming imperative on early patient diagnosis, prognosis and evaluation of residual disease. The use of circulating extracellular vesicles (EVs) as surrogate markers of tumor progression could be a powerful tool in the clinical setting. EVs in plasma have emerged as a non-invasive option to detect metastatic outcome, however sensitivity is low. Here we have characterized the lymph obtained after postoperative lymphadenectomy as a novel biological fluid enriched in EVs. Our proteomic profiling and BRAFV600E/K status determination demonstrate for the first time that EVs from the lymph of melanoma patients are enriched in melanoma-associated proteins and are useful for BRAF mutations detection. Melanoma oncogenic pathways, immunomodulation and platelet activating proteins are enriched in lymph-derived exosomes from patients with distal lymph node spread compared to local/early spreading. Furthermore, patients positive for BRAFV600E mutation on lymph-circulating vesicles had a shorter time of relapse. These data encourage the analysis of lymph-circulating EVs for detection of residual disease and recurrence.

Project description:Background: the transcription of tumor mutations from DNA into RNA has implications for biology, epigenetics and clinical practice. It is not clear if mutations are in general transcribed and, if so, at what proportion to the wild-type allele. Here, we examined the correlation between DNA mutation allele frequency and RNA mutation allele frequency. Methods: we sequenced the exome and transcriptome of tumor cell lines with large copy number variations, identified heterozygous single nucleotide mutations and absolute DNA copy number, and determined the corresponding DNA and RNA mutation allele fraction. Results: we found that 99% of the DNA mutations in expressed genes are expressed as RNA. Moreover, we found a high correlation between the DNA and RNA mutation allele frequency. Exceptions are mutations that cause premature termination codons and therefore activate nonsense-mediated decay. Beyond this, we did not find evidence of any wide-scale mechanism, such as allele-specific epigenetic silencing, preferentially promoting mutated or wild-type alleles. Conclusion: taken together, our data strongly suggest that genes are equally transcribed from all alleles, mutated and wild-type, and thus transcribed in proportion to their DNA allele frequency.

Optimal anti-EGFR Treatment of mCRC Patients with Low-Frequency RAS Mutation

Project description:Primary objectives: The primary objective of the study is to define an optimal cut-off for anti-EGFR treatment with panitumumab in combination with FOLFIRI of patients with low-frequency RAS mutation defined by digital Next Generation Sequencing (dNGS) using the Oncomine cfDNA pan-cancer panel assay (52 genes) on an Ion Torrent S5 Prime platform. This assay combined with unique molecular identifier (UMI) or molecular barcode technique allows the exact quantification of mutation frequencies. Primary endpoints: As primary endpoint ORR according to RECIST 1.1 will be evaluated separately for each group of patients with defined low-frequency RAS mutation (Groups A, B and C).

Project description:Cancer is a DNA disease characterized by the presence of genetics alterations in cancer cells. The recent studies underline that these recurring alterations must be considered as a good molecular marker. In fact, they could use for tumor DNA detection in different biological fluids. So, the main purpose is to define the presence of circulating tumor DNA in the patients plasma with colorectal cancer, by the presence of mutation (KRAS, NRAS, BRAF, APC, TP53 and MIRCOSATELLIE instability). These molecular analysis will be done both in tumor and plasma samples, This trial allows to characterize the prognostic value of circulating tumoral DNA presence in colorectal cancer.
