Project description:Liquid biopsies are becoming imperative on early patient diagnosis, prognosis and evaluation of residual disease. The use of circulating extracellular vesicles (EVs) as surrogate markers of tumor progression could be a powerful tool in the clinical setting. EVs in plasma have emerged as a non-invasive option to detect metastatic outcome, however sensitivity is low. Here we have characterized the lymph obtained after postoperative lymphadenectomy as a novel biological fluid enriched in EVs. Our proteomic profiling and BRAFV600E/K status determination demonstrate for the first time that EVs from the lymph of melanoma patients are enriched in melanoma-associated proteins and are useful for BRAF mutations detection. Melanoma oncogenic pathways, immunomodulation and platelet activating proteins are enriched in lymph-derived exosomes from patients with distal lymph node spread compared to local/early spreading. Furthermore, patients positive for BRAFV600E mutation on lymph-circulating vesicles had a shorter time of relapse. These data encourage the analysis of lymph-circulating EVs for detection of residual disease and recurrence.

Project description:Endometrial cancer is the most common gynecological cancer worldwide. In women from developed countries, endometrial cancer is usually detected after the age of 60 years. Nearly 90% of the diagnosed cases are sporadic, while 10% are attributed to genetic factors. Thus, it is necessary to develop a diagnostic platform. Currently, the lymph node status is a major prognostic factor in assessing endometrial cancer. However, despite its high sensitivity, it is still in debate. In order to increase its selectivity and specificity, in-depth proteomic study has been performed on patients sentinel lymph node (SNL) samples and on grade I to grade III endometrial carcinoma (EC) samples. We identified specific markers of bad diagnosis that clearly correlate SNL to EC i.e. PRSS3, PTX3, ASS1, YBX2, RBM25, MUC5B. These markers have also been detected in TCGA. By contrast, we also detected good prognosis markers for the overall survival such as CD74, GOLM1, SLC9A3R2, SP100 proteins. Markers for EC and SNL cancer-grading have also been identified. Taken together, it is now possible to propose a novel approach for molecular diagnosis, prognosis and patient’s stratification in EC which will be of excellent utility in therapeutic decision.

We investigated JAGGED1 (JAG1) as a promoter of lymph node metastasis and a prognostic biomarker in metastatic lymph node specimens. Experimental Design: We used mouse models to assess the role of JAG1 expression in human and mouse breast cancer cells on lymphovascular invasion, lymph node metastatic potential, transcriptional profiles, and tumor interactions with lymphatic endothelium. We examined breast and lymph node samples from 284 breast cancer patients to determine the correlative and prognostic value of tumoral JAG1 expression in the lymph node. Results: In matched human breast tumor and lymph node samples, tumor cells that invaded lymph nodes showed higher JAG1 expression than their associated primary tumors (P value < 0.0001). In multiple models, breast cancer cells with high JAG1 expression showed increased lymphovascular invasion, lymph node metastasis, lymph node metastatic outgrowth, and migration through lymphatic endothelium. Transcriptomic analysis indicated that tumoral JAG1 regulates both juxtacrine and paracrine signaling pathways that induce inflammatory and pro-metastatic genes in lymphatic endothelium. When examining patients with identical surgical treatments, patients with lymph node JAG1 H-scorelow showed increased 5-year recurrence free survival rates than patients with lymph node JAG1 H-scorehi (87% vs 70%, P=0.016). Conclusions: JAG1 expression promotes lymphovascular invasion and lymph node metastasis in murine models. In patients, high expression of JAG1 in tumor cells in lymph node metastases predicts reduced recurrence free survival.

Project description:Regional lymph node metastasis critically influences distant metastatic progression, anti-tumour immunity, and patient prognosis. This study systematically characterised immune cell dynamics across paired primary tumours, sentinel tumour-draining lymph nodes (S-TDLNs), and secondary axillary lymph nodes (ALNs) from treatment-naïve triple-negative breast cancer (TNBC) patients using integrated multi-omics approaches, including imaging mass cytometry, single-cell RNA sequencing, Visium and Xenium spatial transcriptomics, and multiplex immunofluorescence. Xenium profiling was performed on FFPE lymph node sections spanning different metastatic stages to resolve high-resolution spatial organisation, neighbourhood complexity, and immune cell co-localisation during sequential lymph node metastasis.

Project description:Both the univariate analysis and multivariate analysis showed that the status of primary lymph node was the strongest predict factor. The subgroup analysis confirmed that the lymph node metastasis was associated to worse prognosis only in stage T3 and T4, rather than in stage T1 and T2. We firstly proposed the hypothesis that the status of lymph node reflected delayed diagnose of the disease, rather than the biological behavior of “seed”. The mRNA profile showed that there was minimal difference between lymph node positive and negative subgroups.
