A clinical study to see the effects of two different methods of fluid administration (Fluid volumeestimated by anaesthetist compared with volume estimated by a machine) during Cytoreductive Surgery with Hyperthermic Intraperitoneal Chemotherapy

Project description:Intervention1: Stroke volume variation (SVV) (using Flotrac) guided fluid therapy: The CI will be maintained at values greater than or equal to 2.5 l/min/m2 , stroke volume variation 12%, Stroke Volume Index (SVI) 35ml/m2/ beat, Systemic Vascular Resistance Index (SVRI) more than equal to 1900 dynes-sec/cm-5/m2 . Basal infusion of crystalloids will be at 4 ml/kg/h. If CI 2.5 l/min/m2, SVI 35 ml/m2 and SVV 12% crystalloid challenge and colloid bolus will be given. In the case of CI 2.5 l/min/m2 and SVI 35 ml/m2 with SVV 12% an infusion with dobutamine will be initiated alongwith colloid bolus. Control Intervention1: Fluid therapy guided by clinical parameters like mean arterial pressure (MAP), central venous pressure (CVP) and urine output: to maintain end organ perfusion, general fluid management with IV infusion of crystalloid (lactate free) will be done aiming at a Mean arterial pressure (MAP) of 65 mm Hg, urinary output of at least 0.5 (CRS phase), 2 (HIPEC phase), and 1 (post HIPEC phase) ml/ kg/ hr, respectively and CVP of 8-12 mm hg. Basal infusion of crystalloid will be at 4 ml/kg/hour, with bolus of crystalloid, colloid and use of noradrenaline depending on MAP, Urine output and CVP. Primary outcome(s): Serum Lactate levelsTimepoint: At baseline, then at one hour interval during Cytoreductive phase, half hourly interval during HIPEC phase and every 6 hourly interval post operatively for 24 hours. Randomised Trial of Doppler-Optimised Fluid Balance in Elective Colectomy

Project description:Perioperative fluid management has been historically dictated by outdated, contradictory concepts. Excessive or inadequate fluid administration around surgery is deleterious and hence goal-directed fluid therapy using Oesophageal Doppler-derived cardiac indices is preferable to optimize tissue perfusion. Previous studies in this area have been hampered by lack of standardization in other aspects of perioperative care and none have explored the impact of individualized fluid therapy on post-operative fatigue.The investigators proposed a study involving 80 patients having open/ laparoscopic colonic surgery to investigate the effect of Oesophageal Doppler guided fluid administration intraoperatively compared to current best practice of fluid restriction. The investigators have an optimized peri-operative care pathway established at the Manukau Surgical Centre (MSC), Middlemore Hospital. All patients will be cared for under the Enhanced Recovery After Surgery (ERAS) multimodal care plan therefore ensuring that all other aspects of care besides intraoperative fluid administration remain homogenous. Outcomes will include post-operative recovery, clinical outcomes as well as physiological data with follow-up to 30 days.

Tylutki2017-four-compartment PBPK heart model accounting for cardiac metabolism

Project description:In the field of cardiac drug efficacy and safety assessment, information on drug concentration in heart tissue is desirable. Because measuring drug concentrations in human cardiac tissue is challenging in healthy volunteers, mathematical models are used to cope with such limitations. With a goal of predicting drug concentration in cardiac tissue, we have developed a whole-body PBPK model consisting of seventeen perfusion-limited compartments. The proposed PBPK heart model consisted of four compartments: the epicardium, midmyocardium, endocardium, and pericardial fluid, and accounted for cardiac metabolism using CYP450. The model was written in R. The plasma:tissues partition coefficients (Kp) were calculated in Simcyp Simulator. The model was fitted to the concentrations of amitriptyline in plasma and the heart. The estimated parameters were as follows: 0.80 for the absorption rate [h-1], 52.6 for Kprest, 0.01 for the blood flow through the pericardial fluid [L/h], and 0.78 for the P-parameter describing the diffusion between the pericardial fluid and epicardium [L/h]. The total cardiac clearance of amitriptyline was calculated as 0.316 L/h. Although the model needs further improvement, the results support its feasibility, and it is a first attempt to provide an active drug concentration in various locations within heart tissue using a PBPK approach. Model is encoded by Ruby and submitted to BioModels by Ahmad Zyoud.
