Personalized Self-Management Training Compared to Standardized Self-Management Training in Colorectal Cancer Patients.

ABSTRACT: This is a Phase 2 prospective, randomized, controlled, double-arm study to assess personalized self-management training (PSMT) intervention efficacy and patient experiences compared to standardized self-management training (SSMT). A total of 120 colorectal cancer (CRC) patients will be enrolled and randomized 1:1 to complete a 6-week self-management training program (either PSMT or SSMT) to be carried out by licensed occupational therapists with doctoral training. This study aims to examine whether PSMT is more effective in increasing adherence to healthy behavior recommendations compared to SSMT in CRC patients.

Self-management support in colorectal cancer care pilot study

Project description:Interventions: The intervention involves administration of The Flinders Program at the Wellington Blood & Cancer Centre. Approximately fifty eligible patients diagnosed with colorectal cancer will be randomised into intervention and control groups after completing an initial assessment. Those in the intervention group will be given three cancer coaching sessions with a trained coach over 3-months during regularly scheduled outpatient treatment appointments. The Flinders Program is a set of generic tools and processes enabling health professionals and health consumers to undertake a structured process that allows for assessment of self-management behaviours, collaborative identification of problems and goal setting leading to the development of individualised care plans (Flinders Human Behaviour & Health Research Unit, 2005). Each 30-60 minute cancer coaching session involves discussions centred around the person, not the disease. Support provided attempts to encourage collaboration and problem-solving; ideally resulting in increased resilience, information sharing, referral (if appropriate) and care planning. Remodeling of the human skeletal muscle proteome found after long-term endurance training but not after strength training

Project description:Proteomic characterization of skeletal muscle biopsies collected from the vastus lateralis of 44 male and female human subjects. Research subjects were divided in three different groups based on their individual exercise backgrounds and physical performance testing: 1) endurance trained (males; ME, n = 9 and females; FE, n = 9, with at least 15 years’ of regular training experience), 2) strength-trained males (MS, n = 9, with at least 15 years’ of regular training experience), and 3) age-matched healthy untrained controls (males, MC, n = 9 and females FC, n = 8, with a self-reported history of <2 exercise bouts per week over the past 15 years).

Self-Management and Activation to Reduce Treatment-Related Toxicities (SMARTCare)

Project description:The investigators will undertake a multi-centre, randomized controlled trial to implement and evaluate a proactive model of care (SMARTCare) during active cancer treatment that incorporates self-management support (SMS). Patients allocated to the control arm will receive care from ambulatory clinic nurses trained in SMS. Patients allocated to the intervention arm will will receive care from ambulatory clinic nurses trained in SMS, in addition to being given access to a web-based, self-management education program and nurse-led health coaching during the first four months following the first systemic therapy administration.

Evaluation of a brain training programme in cancer survivors self reporting cognitive changes

Project description:Interventions: A home based, computerised cognitive rehabilitation programme to be used for 4x forty-minute sessions per week for a total of 15 weeks (i.e. 40 hours). There are five training exercises that form the core of the program, each targeting a different area of brain functioning. These cognitive targets include visual precision, divided attention, working memory, useful field of view and visual processing speed. Each exercise has an accompanying assessment that sets an improvement goal and measures progress against that goal. The exercises adapt to an individual’s performance and all patients will complete a recommended schedule of training as per the programme design. Primary outcome(s): The primary endpoint is self reported cognitive function as assessed by the Functional Assessment of Cancer Therapy - Cognitive Function (FACT-COG). The primary outcome will be the difference between the intervention group and the control group in the change in FACT-COG score on completion of the cognitive rehabilitation program compared to the baseline score.[Self reported cognitive function as measured by FACT-COG will be evaluated at 3 time points. Self-management Support for Colorectal Cancer Survivors Colorectal Cancer Survivors: A Mixed-methods Study

Project description:Background: Survivors of colorectal cancer have to face long-term consequences of the disease and its treatment side effects, which in turn affect mood and psychological well-being. Self-management support may help colorectal cancer survivors to achieve healthy lifestyle and better adjustment. However, there is little research evidence to support it and also no theory-based self-management support interventions specifically designed for colorectal cancer survivors in Taiwan. Aim: The study aims is to test the efficacy of the Acceptance and Commitment Therapy -based self-management support program on the primary outcome, quality of life, and secondary outcomes, physical activity, fruit and vegetative intake, body mass index, sleep quality, emotion distress, and fatigue in colorectal cancer survivors . Design: An experimental design with repeated measures will be used to test the intervention efficacy. A convenient sample of 250 colorectal cancer (stage I-III) survivors who has completed initial treatments will be recruited and randomized to the control or intervention group. The intervention includes a colorectal cancer self-management information booklet, a DVD, two individual skill trainings and 12 follow-up telephone calls. These are to establish participants’ self-management skills and healthy lifestyle, including physical activity and healthy eating fruits and vegetables. The control group will receive health education leaflets. Outcome variables will be assessed on the baseline, 2th, 4th, and 6th month in both groups. Descriptive analysis will be used to describe patients’ demographics, disease variables, and outcome variables. The Chi-square, t-test, and General Linear Mix-effect Model will be used to test the efficacy of the study interventions.

Strength training with suppressed testosterone level

Project description:Human subjects were randomized for treatment with a GnRH-analogue, Goserelin, which suppresses endogenous testosterone or placebo for 12 weeks. Strength training was performed during the last 8 weeks. The suppression of testosterone resulted in an attenuation of the normal muscle adaptation to strength training (increased muscle mass and strength). To identify molecular signals involved in the response to testosterone levels, biopsies were obtained 4 hours after the last training session and gene expression compared with Affymetrix 3' microarrays. This timepoint should capture goserelin effect on both constitutive expression, training induced changes as well as acute exercise induced (4 hours) differences in mRNA levels.
