Project description:Preterm infants are highly susceptible to sustained lung inflammation, which may be triggered by exposure to multiple environmental cues such as supplemental oxygen (O2) and infections. The underlying mechanisms are still poorly understood. The hypothesis of this study is that dysregulated macrophage activation is a key feature leading to inflammation-mediated development of bronchopulmonary dysplasia (BPD) in preterm infants.
Cord blood samples of preterm infants (n=14) and term infants (n=19) as well as peripheral blood from healthy adults (n=17) were collected. Age-dependent differences in immune responses of monocyte-derived Mä from preterm infants were characterized and compared to term infants and adults after lipopolysaccharide (LPS) exposure.

Project description:Neonatal Tracheal Aspirate-derived Airway Epithelial Cells (nTAECs) were isolated utilizing fresh tracheal suction aspirates from preterm (born at 23-28wks gestational age) and term infants (born at 37-42wks gestation). Included infants (especially term infants) did not have significant lung disease at the time of collection. Isolated cells were plated in 804g-cell-derived matrix coated T25, then expanded to a T75 flask and subsequently cultured in transwell inserts (0.4μm size pore) at 105 live cells/insert under air-liquid interface (ALI). Basal nTAECs have the capacity to differentiate into the full repertoire of mature airway epithelial lineages (ciliated, secretory club and mucus secreting goblet cells) during ALI culture. Cells were exposed to 60% O2 or room air control (21% O2) for 7 days (ALI day 7 to 14). On ALI day 14, cells will be harvested from the transwells and prepared for proteomic analysis using Data-independent acquisition (DIA) proteomics analysis. The samples will undergo several processing steps including chloroform/methanol extraction with trypsin digestion, Orbitrap Exploris 480 mass spectrometry analysis and eventual library construction and bioinformatics analysis. Utilizing 5 donor samples for each group, the proteomic workflow compared 4 exposure groups: a) Term nTAECs in Normoxia, b) Term nTAECs Hyperoxia, c) Preterm nTAECs in Normoxia, d) Preterm nTAECs in Hyperoxia.

Project description:Preterm infants exhibit marked heterogeneity in the developmental stages of their immune systems at birth, which vary significantly across gestational ages. The molecular basis of immune incompetence in preterm infants and its association with immunosuppressive mechanisms have not been fully elucidated. To gain a comprehensive understanding of immune heterogeneity pertaining to the early neonatal immune system, we collected peripheral blood samples from preterm infants and full-term infants within 12 h after birth alongside peripheral blood samples from four adults for comparative single-cell transcription analysis. Using single-cell RNA sequencing, we found that myeloid-derived cells from the peripheral blood of preterm infants, especially extremely preterm infants (< 28 weeks), highly expressed IL1R2. IL1R2+ myeloid-derived cells with S100P high expression and IL1R2+ regulatory T cells with LEF1 high expression may play an important role in immunosuppression during early immune development. High expression of IL1R2 in preterm infants may lead to immune suppression, and IL1R2 may affect both myeloid-derived cells and regulatory T cells, promoting immunosuppression with high S100P and LEF1 expression and playing an important role in immune suppression during early immune development in preterm infants.

Project description:Preterm infants born <32 weeks gestation have abnormal microbial colonisation and dysregulated inflammation within the gut. Preterm infant-derived intestinal organoids (PIOs) represent a valuable model for investigating gut microbiome-host interactions and inflammatory responses. We optimised an inflammation model in PIO monolayers incubated within an anaerobic co-culture system that recreates the physiological oxygen gradient of the intestinal epithelium. We trialled multiple stimuli, including live and heat-killed pathobiont consortia, lipopolysaccharide (LPS) and flagellin. We found that a combination of apical LPS and basolateral flagellin, incubated for 3 hours, elicited the most robust response. This was characterised by enhanced pro-inflammatory cytokine secretion, the potential for chemokine-driven immune recruitment, TNFα and IL17C pathway signalling, shifts from NF-κB to AP-1-mediated responses, and signs of tissue remodelling. This provides a framework for appropriate study design to disentangle the impacts of microbiome-host interactions in health and disease using intestinal organoids.

Project description:Preterm birth is often predisposed by chorioamnionitis (CA) and CA affects the fetal gut and lungs via intra-amniotic (IA) inflammation, thus accentuating the proinflammatory effects of preterm birth. It is not known if IA inflammation also affects other perfusion-sensitive organs (e.g., kidneys) before and after preterm birth. Using preterm pigs as model for preterm infants, we hypothesized that CA induces fetal and neonatal renal dysfunctions that can intially be detected via plasma proteome, partly explaining the frequent renal dysfunction in preterm infants. Fetal pigs (88% gestation) were given an IA dose of lipopolysaccharide (LPS, 1 mg/kg, n=28), delivered preterm by cesarean section three days later, and compared with controls (CON, n=26) at birth and postnatal day five. Plasma proteome and protein markers of inflammatory pathways were evaluated.

Project description:Extreme preterm infants are a growing population in the neonatal intensive care unit. Multiple factors play a role in preterm birth, resulting in complications including severe bronchopulmonary dysplasia (sBPD) without or with and pulmonary hypertension (BPD-PH). The goal of this study was to identify biomarker signatures associated with sBPD and BPD-PH. We analyzed profiles in tracheal aspirates (TAs) from 46 extremely preterm infants receiving invasive mechanical ventilation (25 sBPD, 21 BPD-PH) . We found specific miRNA signatures in TAs that may serve as biomarkers for the two disease phenotypes.
