Hi-C for genome-wide detection of enhancer-hijacking rearrangements in routine clinical biopsies of lymphoid cancers

Project description:We performed Hi-C retrospectively on archival formalin-fixed paraffin embedded (FFPE) tissue biopsies of lymphoid cancers to explore the utility of Hi-C for comprehensively identifying enhancer-hijacking oncogene rearrangements in routine clinical samples. Our data shows that FFPE-compatible Hi-C retains state-specific topological features of tumor cells at gene loci associated with germinal center B cell or plasma cell state and comprehensively detects rearrangements genome-wide with high concordance against prior clinical testing. Hi-C additionally reveals enhancer-promoter interactions consistent with enhancer-hijacking across rearrangement breakpoints at recurrent, uncommon, and diversely-rearranging loci, and provides topological information to support the interpretation of these events that is not provided by alternative routine clinical assays such as FISH.

JAK2V617F Impairs Lymphoid Differentiation in Myeloproliferative Neoplasms

Project description:Myeloproliferative neoplasms (MPNs) arise via the acquisition of a driver mutation in a single hematopoietic stem cell (HSC), often decades prior to the development of a clinical phenotype. The most common MPN driver mutation, JAK2V617F, activates aberrant JAK/STAT signaling via cytokine receptors critical for myelopoiesis. Over time, this MPN HSC clone outcompetes its normal counterparts, leading to excessive myeloid cell production and contributes to lymphopenia in patients with MPNs and leades to elevated neutrophil-to-lymphocyte ratio (NLR), which is predictive of disease-related complications including thrombosis and mortality. We conducted this study to learn how hematopoiesis from the JAK2V617F clone affects lymphopoiesis in patients with MPNs. Although myeloid proliferation via aberrant JAK2 signaling is the most apparent mechanistic link between JAK2V617F and MPN phenotypes, our findings demonstrate that impaired lymphoid differentiation is an additional feature of JAK2V617F hematopoiesis, leading to the rarity of JAK2V617F lymphocytes despite the dominance of JAK2V617F HSCs in patients with MPNs. The combination of prolific myelopoiesis and defective lymphopoiesis from the JAK2V617F clone is a potential connection between MPN pathology and the surrogate markers, including NLR and lymphopenia, which hold prognostic significance. Based on our data, we speculate that, defective JAK2V617F lymphopoiesis and the consequent increased burden of lymphopoiesis from residual normal HSC clones drives the appearance of abnormal lymphocyte subsets, lymphoproliferative disease or T cell exhaustion in MPNs. Further study of MPN lymphopoiesis provides an opportunity to define the immune deficits underlying the myriad complications that affect patients with MPNs.

Leucine, Autoimmune Diseases, and Circulating Mediators Shape the Causal Landscape of Lymphoid Malignancies

Project description:This study investigated environmental and metabolic determinants of lymphoid malignancies (LM) using a systematic Mendelian randomization analysis of 10,090 GWAS traits from the IEU OpenGWAS database. Autoimmune diseases, including hyperthyroidism and membranous nephropathy, were identified as risk factors for LM, and mediation analysis suggested circulating promotilin, monounsaturated fatty acids, and glucose as potential mediators. Leucine was identified as a protective factor. Findings were evaluated in large epidemiological cohorts and UK Biobank multi-omics data, and further examined in lymphoid malignancy cell lines and an MH/NRASG12D B-cell precursor acute lymphoblastic leukemia mouse model. High-leucine conditions suppressed cell proliferation and induced apoptosis in vitro, while a high-leucine diet prolonged survival in vivo. Higher plasma leucine levels were also associated with improved clinical outcomes in B-ALL and CAR-T-treated B-cell lymphoma cohorts.

A Study of Repotrectinib (TPX-0005) in Patients With Advanced Solid Tumors Harboring ALK, ROS1, or NTRK1-3 Rearrangements

Project description:Phase 1 dose escalation will determine the first cycle dose-limiting toxicities (DLTs), the maximum tolerated dose (MTD), the biologically effective dose and recommended Phase 2 dose (RP2D) of repotrectinib given to adult subjects with advanced solid malignancies harboring an ALK, ROS1, NTRK1, NTRK2, or NTRK3 gene rearrangement. Midazolam DDI substudy will examine effect of of repotrectinib on CYP3A induction. Phase 2 will determine the confirmed Overall Response Rate (ORR) as assessed by Blinded Independent Central Review (BICR) of repotrectinib in each subject population expansion cohort of advanced solid tumors that harbor a ROS1, NTRK1, NTRK2, or NTRK3 gene rearrangement. The secondary objective will include the duration of response (DOR), time to response (TTR), progression-free survival (PFS), overall survival (OS) and clinical benefit rate (CBR) of repotrectinib in each expansion cohort of advanced solid tumors that harbor a ROS1, NTRK1, NTRK2, or NTRK3 gene rearrangement.
