A New Model for Caries Risk Prediction in Teenagers Using a Machine Learning Algorithm Based on Environmental and Genetic Factors

ABSTRACT: This study describes the construction of a caries risk prediction model (CRPM) based on both environmental and genetic factors, using a machine learning algorithm. This CRPM included specific patient characteristics, such as SNPs, gender, and factors like the participants being the only child of the respective families, to provide an estimate of the absolute risk of a specific caries outcome. We believe that our study makes a significant contribution to the literature because the newly constructed CRPM can accurately identify a high caries-risk population

Prognosis prediction model for Alzheimer’s disease conversion from mild cognitive impairment by integrative analysis of multi-omics data

Project description:Mild cognitive impairment (MCI) is a clinical precursor of Alzheimer’s disease (AD). Many MCI subjects convert to AD, although others remain stable as MCI or sometimes return to cognitive normal. Currently, there are no curative treatments for patients who are already in AD, and therefore biomarkers for early detection of high risk MCI-to-AD conversion subjects are rapidly required. Here, we investigated potential biomarkers from blood-based miRNA (miR) expression profiles and genomic data of 197 Japanese MCI patients. Using the candidate biomarkers, we constructed a prognosis prediction model based on a Cox proportional hazard model. The final prediction model, composed of 24 miR-eQTLs (i.e. SNP-miRNA pairs) and three clinical factors (age, sex and ApoE ε4 carriers), successfully classified MCI patients into two groups, low and high, in terms of risk of MCI-to-AD conversion (log-rank trend test P=3.44×10^(-4)), and achieved a concordance index of 0.702 on an independent test set. Network-based meta-analysis using target genes of the miR-eQTLs further revealed four important hub genes (SHC1, FOXO1, GSK3B, and PTEN) associated with the pathogenesis of AD. Statistically significant differences were observed in PTEN expression between MCI and AD and SHC1 expression between cognitively normal elder subjects (CN) and AD when examining RNA-seq data from 610 blood samples (PTEN, P=0.023; SHC1, P=0.049), although FOXO1 and GSK3B showed low levels of expression in blood. Accurate prediction of MCI-to-AD conversion enables earlier appropriate intervention for those MCI patients and can lead to a reduction of MCI patients that convert to AD with high risk. We believe that further investigation with larger sample sizes will contribute to practical clinical use of our approach in MCI-to-AD conversion in the near future.

Imporatance of substantial weight loss for altering gene expression during intensive cardiovascular lifestyle modification

Project description:The objective of this study was to examine relationships between weight loss through changes in lifestyle and peripheral blood gene expression profiles. Substantial weight loss (-15.2+3.8%) in lifestyle participants was associated with improvement in selected cardiovascular risk factors and significant changes in peripheral blood gene expression from pre- to post-intervention: 132 unique genes showed significant expression changes related to immune function and inflammatory responses involving endothelial activation. In contrast, participants losing minimal weight (-3.1+2.5%) showed only minor changes in cardiovascular risk factors and markers of inflammation, and no changes in gene expression compared to non-intervention controls after 1 year. Weight loss (>10%) during lifestyle modification is associated with down-regulation of genetic pathways governing interactions between circulating immune cells and the vascular endothelium and may be required to successfully reduce CVD risk. A prospective nonrandomized trail was conducted over 1 year in participants undergoing intensive lifestyle modification to reverse or stabilize progression of coronary artery disease. Cardiovascular risk factors, inflammatory biomarkers, and gene expression as a function of weight loss were assessed in 89 lifestyle participants and 71 retrospectively matched controls undergoing usual care.

Epigenetic Age Acceleration Enhances Prediction of Overall Survival in Early-Stage Non-Small Cell Lung Cancer

Project description:Epigenetic age, influenced by environmental factors like smoking, predicts mortality in general populations but remains under-explored in early-stage lung cancer. We analyzed blood DNA methylation from 75 Boston Lung Cancer Study participants with early-stage non-small cell lung cancer (NSCLC), of whom 63 died during follow-up. Epigenetic age acceleration (EAA) was estimated using Horvath, Hannum, PhenoAge, and GrimAge2 clocks and assessed using Cox models and random survival forests (RSF). Across clocks, EAA significantly predicted survival in early-stage NSCLC. GrimAge2 EAA achieved the highest predictive accuracy (C-index=0.73±0.04) and emerged as the dominant predictor of survival among RSF models (variable importance=5.7%), surpassing smoking and age. GrimAge2 EAA showed a non-linear association with mortality in which current smokers had >10% higher predicted mortality risk than former or never smokers at equivalent EAA, a pattern confirmed in Kaplan-Meier analyses. GrimAge2 EAA is a promising biomarker for personalizing risk stratification alongside traditional clinical factors.

Serum Alpha-2-Macroglobulin Links Type 1 Diabetes Status and Incident Cardiovascular Disease

Project description:Background: Cardiovascular disease (CVD) is a leading cause of mortality in individuals with type 1 diabetes mellitus (T1DM), with conventional risk factors not fully explaining the heightened risk. Study Design: We conducted a cross-sectional study comparing serum protein levels in 134 participants with T1DM from the Coronary Artery Calcification in Type 1 Diabetes study to 47 healthy controls, followed by a case-cohort study among 181 T1DM participants, including 47 with CVD events. Linear regression analyzed serum protein levels by diabetes status, while Cox proportional hazards regression assessed associations with incident CVD. Findings: Among 28 serum proteins tested, five showed significant differences between T1DM patients and controls, with alpha-2-macroglobulin (A2M) being notably 1.6-fold higher in T1DM. Adjusted models revealed that A2M (mean hazard ratio [HR] 2.5, 95% confidence interval [CI] 1.5-4.1) and apolipoprotein C3 (APOC3, mean HR 1.7, 95% CI 1.1-2.7) were independently associated with incident CVD in T1DM. Conclusions: Elevated serum levels of A2M in T1DM suggest its potential role as a marker—and possibly a mediator—of CVD risk in these patients, alongside APOC3. These findings underscore the significance of specific serum proteins in understanding and managing CVD risk in T1DM.

Accurate prediction of central conventional CHondrosarcoma Risk and Outcome using MEthylation profiling: CHROME

Project description:Chondrosarcomas are malignant cartilage-forming bone tumours with heterogeneous behaviour, making prognostication and clinical management challenging. Histological grading is the primary tool for predicting clinical outcomes in conventional chondrosarcoma. However, its high interobserver variability limits reliable distinction between low- and high-risk patients, potentially leading to suboptimal clinical management. Given the increasing use of DNA methylation profiling as a valuable tool in surgical pathology for tumour classification, we investigated its prognostic value in central conventional chondrosarcoma. We generated methylation data from 69 primary central conventional chondrosarcomas profiled with Illumina’s Human MethylationEPIC Array (850k sites), and identified methylation sites individually linked to patient outcome. A LASSO Cox regression model was applied to these methylation sites to identify an optimal set of 8 informative sites. With the regression coefficients of these methylation sites, we constructed a risk score that predicts central conventional CHondrosarcoma Risk Outcome from Methylation (CHROME). CHROME stratifies patients into High or Low risk groups for disease recurrence, onset of metastasis and disease-specific mortality. Survival analysis in an independent validation cohort (n=68) demonstrated strong discriminatory performance of CHROME, with complete separation of outcomes and no adverse events observed in the Low risk group. Overview of the clinico-pathological information showed that a low grade (ACT/G1) case with an event was correctly assigned to the High risk group, while 9 high grade cases without events were classified as Low risk. CHROME provides accurate risk stratification in central conventional chondrosarcoma and may represent a valuable tool for improving its prognostication and clinical management.

Autism mutations rewire protein interaction networks to drive neurodevelopmental pathology

Project description:Translating high-confidence (hc) autism spectrum disorder (ASD) genes into viable treatment targets remains elusive. We constructed a foundational protein-protein interaction (PPI) network in HEK293T cells involving 100 hcASD risk genes, revealing over 1,800 PPIs (87% novel). Interactors, expressed in the human brain and enriched for ASD but not schizophrenia genetic risk, converged on protein complexes involved in neurogenesis, tubulin biology, transcriptional regulation, and chromatin modification. A PPI map of 54 patient-derived missense variants identified differential physical interactions, and we leveraged AlphaFold-Multimer predictions to prioritize direct PPIs and specific variants for interrogation in Xenopus tropicalis and human forebrain organoids. A mutation in the transcription factor FOXP1 led to reconfiguration of DNA binding sites and altered development of deep cortical layer neurons in forebrain organoids. This work offers new insights into molecular mechanisms underlying ASD and describes a powerful platform to develop and test therapeutic strategies for many genetically-defined conditions.
