MicroRNAs associated with reverse left ventricular remodeling in humans identify pathways of heart failure progression

ABSTRACT: Plasma miRNAs associated with LVRR in humans are dysregulated in animal HF and target clusters of genes involved in mechanisms implicated in HF. A translational approach integrating human HF, bioinformatics, and model systems may uncover novel pathways involved in HF.

Diabetic ischaemic cardiomyopathy: Characterising the left ventricular myocardial molecular profile in advanced human heart failure

Project description:Ischaemic cardiomyopathy (ICM) is the most common cause of heart failure (HF) and often coexists with diabetes mellitus (DM). Yet, their combined effects are seldom investigated and are poorly understood. We performed multi-omic analyses of end-stage ICM with DM (ICM-DM), ICM-No DM, non-ischaemic cardiomyopathy with DM (NICM-DM), and NICM-No DM against healthy age-matched donors (AMD). Tissue was sourced from pre-mortem human left ventricular myocardium. Though fatty acid oxidation proteins were down-regulated in ICM-DM relative to AMD and other HF conditions, the unique ICM-DM down-regulation of acylcarnitines, perilipin, and ketone body, amino acid, and glucose metabolising proteins indicated lipid metabolism may not be entirely impaired. Oxidative phosphorylation appeared exacerbated in ICM-DM, consistent with purportedly increased oxidative stress. Extracellular matrix proteins were up-regulated principally in ICM-DM despite the absence of scar tissue. These findings indicate DM confounds the pathological phenotype in HF, and the need for a paradigm shift regarding lipid metabolism.

Load-Dependent Changes in Left Ventricular Gene Expression in a Pathophysiologically Relevant Murine Model of Reversible Heart Failure (2 weeks heart failure, 4 weeks reversal)

Project description:Wild type, female, C57BL/6 mice were subjected to sham (n=6) surgery, or TAC + MI to cause progressive LV remodeling (n=12). At 2wks post-TAC, one group of mice underwent de-banding (HF-DB, n=6), whereas in a second group of mice the band remained intact (HF; n = 6). LV remodeling was evaluated by 2D echocardiography at 14 days post-TAC+MI , and 4 wks post-debanding. At 6 wks the hearts were excised and analyzed for changes in gene expression using transcriptional profiling. e-banding in the HF-DB mice resulted in normalization of LV volumes and LV mass, and normalization of cardiac myocyte hypertrophy at 6wks, without significant changes in myofibrillar collagen in the HF and HF-DB mice. Both LV ejection fraction (LVEF) and LV radial strain improved numerically following de-banding; however, both measurements remained significantly depressed in the HF-DB mice compared to sham, and were not significantly different from HF mice at 6wks. Reverse LV remodeling in the HF-DB mouse hearts was accompanied by a partial (80%) normalization of the genes that became dysregulated during LV remodeling, whereas 20% of genes remained persistently dysregulated following reverse LV remodeling.

Breast Cancer Progression in the Presence of Treated and Untreated Left Ventricular Dysfunction

Project description:We hypothesized that treating heart failure mitigates its impact on cancer progression and that different treatments (carvedilol, enalapril, empagliflozin) may have distinct effects. For this reason we have created an mouse model combining both heart failure (LAD ligation) and 4T1 metastatic breast cancer to study the effects of these treatments on breast cancer development at different levels: primary tumor growth in vivo, metastasis development and RNA-sequencing of the primary tumor. Twenty samples divided in five different groups were analyzed with RNA sequencing: group 1= Ctrl; group 2= HF/veh; group 3= HF/car; group 4= HF/emp group 5 HF/ena. HF medications with different mechanisms of action had distinct effects on 4T1 progression when the two diseases coexisted. These effects varied from isolated effects on the tumor transcriptome, to effects on the expansion of the primary tumor and on metastatic spread in the lungs.

Prevalence of transthyretin amyloidosis in patients with heart failure and no left ventricular hypertrophy

Project description:The study prospectively enrolled patients admitted for HF with LV ejection fraction (LVEF) Ã¢Â‰Â¥ 50% and LV wall thickness <12 mm. TTR cardiac amyloidosis was diagnosed according to accepted criteria, which include positive cardiac 99-Tc-DPD scintigraphy in the absence of monoclonal protein expansion in blood. In a cohort of patients with HFpEF without LVH, the prevalence of TTR cardiac amyloidosis was 5%. Transthyretin gene sequencing was performed in positive patients.

Dynamic adaptation of myocardial proteome during heart failure development after induction of left ventricular pressure overload in mice

Project description:We analyzed time dependent global proteomic adaptations during heart failure (HF) progression in a mouse model, suffering from left ventricular pressure overload due to transverse aortic constriction (TAC), to gain deeper insights in the disease development and identify new biomarker candidates. The hearts from TAC and sham mice were examined by cardiac MRI on either day 4, 14, 21, 28, 42, and 56 after surgery (n=6 group/time point). At each time point, proteomes of the left (LV) and right ventricles (RV) of TAC and sham mice were analyzed by mass spectrometry (MS).
