Optimizing Personalized Therapy for Rheumatoid Arthritis Using a Novel Drug Combination of Bioactive Chemical Constituents Derived from a Classic Chinese Herbal Formula

ABSTRACT: Personalized therapy of rheumatoid arthritis (RA) based on traditional Chinese medicine cold and hot syndromes is selection of the best treatment for an individual patient. Wutou Decoction (WTD) is one of the classic Chinese herbal formulae which achieve favorable therapeutic response in treating RA-cold syndrome. Microarray analysis based on the adjuvant induced arthritis model combined with characteristics of RA and cold/hot syndromes was performed to screen RA-cold and RA-hot-syndrome-related genes, as well as WTD effect genes.

Project description:Rheumatoid arthritis (RA) is a chronic and disabling disease. TNF-α inhibitors have demonstrated an outstanding performance in relieving joint inflammation and retarding bone erosion involved in RA. However, there is still about one-thirds of RA patients had a poor response to TNF α inhibitors. Currently the personalized biological treatment is the research hotspot. Recent studies focuses on exploring biomarkers predictive of drug response. The research methods such as genomics, transcriptomics, proteomics, metabolomics and immunocytology, have been applied, but they are not successfully integrated. The related studies in China are still at an initial stage, which necessitates an in-depth study in this area. Our preliminary study showed that TNF-α-308 gene polymorphisms existed in Chinese RA patients and PI3K/Akt signal pathway was ativated in proliferated synovial fibroblasts stimulated by TNF-α. Therefore, for the first attempt in China, we intend to screen for differential proteins by using iTRAQ technique in RA patients receiving anti-TNF-α therapy, and then verify the predictive effects of selected differential proteins from the upstream gene polymorphism to the downstream protein expression. We will also investigate the mechanisms of differential proteins involved in TNF-α related signal pathway by using in vitro gene transfer, siRNA interference, and RA animal models. Through our study we hope to discover a prediction protein with a domestic genetic background and finally establish a prediction system with Chinese characteristics.

Project description:Traditional Chinese medicine emphasizes treatment based on syndrome differentiation. This study aimed to clarify the characteristics of DNA methylation expression profiles in peripheral blood mononuclear cells in patients with psoriasis and analyze the differences in these profiles among different traditional Chinese medicine syndromes of psoriasis in order to provide a material basis for the diversity of these syndromes. Blood samples were collected from 32 participants, including 13 patients with psoriatic blood heat syndrome, 13 patients with psoriatic blood stasis syndrome, and 6 healthy controls. Peripheral blood mononuclear cells were extracted and subjected to DNA quality inspection. An Illumina Human Methylation 850k chip was used to sequence each group of samples. After screening out differentially methylated sites, the distribution of the corresponding differentially methylated genes on the chromatin was determined. According to gene annotation classification and CpG island annotation classification, the differentially methylated regions between sample groups were screened, and Gene Ontology and KEGG pathway analyses were used to perform functional analysis of differentially methylated genes. Finally, the differentially methylated genes closely correlating with psoriasis severity were screened using Spearman’s correlation analysis.

Topologically Associated Domains Delineate Susceptibility to Somatic Hypermutation

Project description:Somatic hypermutation (SHM) introduces point mutations into immunoglobulin (Ig) genes of activated B cells to support the process of antibody affinity maturation but also causes "off-target" mutations in other parts of the genome. We have used sensitive lentiviral SHM reporter vectors and a mutationally active human B cell line to identify dozens of regions of the genome that are intrinsically susceptible to SHM ("hot" regions) and many hundreds of regions that are resistant to SHM ("cold" regions). Hot and cold regions are frequently contained within topologically associated domains (TADs). Comparison of hot and cold TADs reveals that while overall levels of transcription are equal, hot TADs are enriched for NIPBL (a component of the cohesin loader), super enhancers, markers of paused/stalled RNA polymerase 2, and multiple transcription factors implicated in B cell development and targeting of SHM. We demonstrate that at least some hot TADs contain enhancer elements that possess SHM targeting activity and that insertion of a strong Ig SHM-targeting element into a cold TAD renders it hot. Our findings lead to a model for SHM susceptibility involving the cooperative action of cis-acting SHM targeting elements and the dynamic and architectural properties of TADs.

The Gene Expression Analysis of Peripheral Blood Monocytes from Psoriasis Vulgaris Patients with Different Traditional Chinese Medicine (TCM) Syndromes

Project description:Psoriasis is chronic skin disease and an important health concern. Traditional Chinese Medicine (TCM) has shown great promise in the treatment of psoriasis. However, the correlation between TCM Syndromes and genomics of psoriasis has not been evaluated. Here, we analyzed gene expression profiling of monocytes from psoriasis vulgaris patients with different TCM syndrome types to reveal the molecular basis of different psoriasis syndromes. Of the 62 cases of psoriasis vulgaris recruited, 16, 23, and 23 cases were of blood-heat syndrome, blood stasis syndrome, and blood-dryness syndrome, respectively; 10 healthy controls were recruited as controls. Affymertix's Gene Chip ®clariom D gene chip was used to detect the gene expression profile of peripheral blood monocytes collected from recruited individuals. Compared with the healthy control group, 1570 genes were up-regulated and 977 genes were down-regulated in the psoriasis vulgaris patients group; 798 genes and 108 genes were up- and down-regulated in the blood-heat syndrome group respectively; 319 and 433 genes were up- and down-regulated in the blood-dryness syndrome group, respectively; and 502 and 179 genes were up-and down-regulated in the blood-stasis syndrome group. Our analyses indicated not only common differential genes and pathways between psoriasis syndrome groups and healthy controls, but also syndrome-specific genes and pathways. The results of this study link the three syndromes at the gene level and will be useful for clarifying the molecular basis of TCM syndromes of psoriasis.
