Next generation sequencing to find new medical targets for the treatment of conduit stenosis

ABSTRACT: To find new medical tagets for the treatment of conduit stenosis, 11 pulmonary bioartificial valved conduits were explanted from patients with a congenital heart defect and next generation sequencing was performed using material from the thickened stenosis-causing conduit wall and the respective leaflet. Deregulated genes between 9 samples from the thickened conduit-wall and 8 leaflets were determined and all receptor tyrosin kinases were selected for further experiments.

Project description:Vein graft failure (VGF) following cardiovascular bypass surgery results in significant patient morbidity and cost to the healthcare system. Vein graft injury can occur during autogenous vein harvest and preparation, as well as after implantation into the arterial system, leading to the development of intimal hyperplasia, vein graft stenosis, and, ultimately, bypass graft failure. While previous studies have identified maladaptive pathways that occur shortly after implantation, the specific signaling pathways that occur during vein graft preparation are not well defined and may result in a cumulative impact on VGF. We, therefore, aimed to elucidate the response of the vein conduit wall during harvest and following implantation, probing the key maladaptive pathways driving graft failure with the overarching goal of identifying therapeutic targets for biologic intervention to minimize these natural responses to surgical vein graft injury. Employing a novel approach to investigating vascular pathologies, we harnessed both single-nuclei RNA-sequencing (snRNA-seq) and spatial transcriptomics (ST) analyses to profile the genomic effects of vein grafts after harvest and distension, then compared these findings to vein grafts obtained 24 hours after carotid-cartoid vein bypass implantation in a canine model (n=4). Collectively, we find that vein conduit harvest and distension elicit a prompt genomic response facilitated by distinct cellular subpopulations heterogeneously distributed throughout the vein wall. This response was found to be further exacerbated following vein graft implantation, resulting in a cascade of maladaptive gene regulatory networks. Together, these results suggest that distension initiates the upregulation of pathological pathways that may ultimately contribute to bypass graft failure and presents potential early targets warranting investigation for targeted therapies.

Project description:Tissue samples have been isolated during corornary artery by-pass grafting (CABG)surgery from the atheroscelrotic arterial wall (AAW, aortic root puncture for proxmal ligation of by-pass vessel), non-atherosclertoci arterial wall (NAAW, distal part of mammary artery used a graft for LAD), liver, skeletal muscle (Recturs m), pericardial mediastinal visceral fat) in CAD patients. Carotid lesions samples from 25 validation patients. We used microarrays to measure global gene expression in these tissues to cluster groups of functionally associated genes and then, second step clustering of patients within each cluster to link clusters to patient phenotypes primarily coronary stenosis (CAD) and Intima media thickness (IMT) in carotid stenosis patients. .

A Light-Driven Bioactive Nerve Conduit for Trigeminal Neuralgia: Integrated Decompression and Photosynthetic Oxygenation Restores Mitochondrial Function

Project description:Vascular compression and concomitant hypoxia exacerbate trigeminal neuralgia (TN), yet current therapies rarely target both pathologies. We developed a photosynthetic nerve guide conduit (SFM-Gel@ NGC) by embedding auto fluorescent microalgae in silk fibroin hydrogel, enabling simultaneous mechanical decompression and oxygen release. The construct exhibited suitable mechanical properties, tunable oxygen generation, and excellent biocompatibility. Under hypoxia, SFM-Gel promoted neuronal cell proliferation and function by reducing ROS and restoring mitochondrial activity, while downregulating HIF-1α and ATP/P2X3 pathways. In a rat TN model, auto fluorescent SFM-Gel@ NGC implantation alleviated mechanical allodynia, repaired nerve morphology, and suppressed BDNF/NGF and ATP/P2X3 inflammatory signaling. Transcriptomic analysis confirmed modulation of hypoxia/ROS/NF-κB pathways during immune/inflammatory responses to compression. This integrated decompression-oxygenation strategy offers a transformative therapy for TN.

Disruption of Notch1 and Gata5 in mice leads to congenital aortic valve disease

Project description:Congenital aortic valve disease (AVD) is one of the most common type of congenital heart defects whose molecular and genetic basis is poorly understood. Pathogenic variants in NOTCH1 and GATA5 have been associated with AVD including aortic valve stenosis (AVS) and bicuspid aortic valve (BAV). Whereas previous genetic mouse models of AVD with Notch1 haploinsufficiency or Gata5 deletion alone display limitations with regards to significant perinatal lethality or a partially penetrance, and here, we generate a new murine model by intercrossing Notch1 and Gata5 heterozygote mice. We demonstrate that Notch1;Gata5 compound mutant mice display highly penetrant and clinically relevant congenital AVS without early lethality. Echocardiographic examination shows progressive AVS and aortic dilatation in Notch1+/-;Gata5-/- compound mutant mice at 6 and 16 weeks of age. Histologic analysis of 16 week old Notch1;Gata5 compound mutant mouse hearts identifies myxomatous aortic valves with thickened and dysmorphic leaflets. Morphologic analysis reveals the presence of BAV in a subset of Notch1;Gata5 compound mice. Furthermore, histologic analysis demonstrates AVS and aortic valve leaflet thickening with proteoglycan accumulation at embryonic day 18.5 and postnatal day 10, consistent with congenital disease. RNA-sequencing analysis of postnatal day 10 aortic valves demonstrates dysregulation of extracellular matrix (ECM) genes in Notch1+/-;Gata5-/- compound mutant mice. In conclusion, we demonstrate a novel genetic interaction between Notch1 and Gata5, which is critical for the proper aortic valve development, and show that Notch1+/-;Gata5-/- compound mutant mice represent a novel mouse model of congenital and progressive AVS.
