Project description:High-grade gliomas are aggressive primary brain cancers with poor response to standard regimens, driven by immense heterogeneity. In isocitrate dehydrogenase (IDH) wild-type high-grade glioma (glioblastoma, GBM), increased intra-tumoral heterogeneity is associated with more aggressive disease. Recently, spatial technologies have emerged to dissect this complex heterogeneity within the tumor ecosystem by preserving cellular organization in situ. Here, we construct a high- resolution molecular landscape of GBM and IDH-mutant high-grade glioma patient samples to investigate the cellular subtypes and spatial communities that compose high-grade glioma using digital spatial profiling and spatial molecular imaging. This uncovered striking diversity of the tumor and immune microenvironment, that is embodied by the heterogeneity of the inferred copy- number alterations in the tumor. Reconstructing the tumor architecture revealed brain-intrinsic niches, composed of tumor cells reflecting brain cell types and microglia; and brain-extrinsic niches, populated by mesenchymal tumor cells and monocytes. We further reveal that cellular communication in these niches is underpinned by specific ligand-receptor pairs. This primary study reveals high levels of intra-tumoral heterogeneity in high-grade gliomas, associated with a diverse immune landscape within spatially localized regions.

Project description:Diffuse gliomas represent the most prevalent class of primary brain tumor. Despite significant recent advances in the understanding of glioblastoma (WHO IV), its most malignant subtype, lower-grade (WHO II and III) glioma variants remain comparatively understudied, especially in light of their notably variable clinical behavior. To examine the foundations of this heterogeneity, we performed multidimensional molecular profiling, including global transcriptional analysis, on 101 lower-grade diffuse astrocytic gliomas collected at our own institution, and validated our findings using publically available gene expression and copy number data from large independent patient cohorts. We found that IDH mutational status delineated molecularly and clinically distinct glioma subsets, with IDH mutant (IDH mt) tumors exhibiting TP53 mutations, PDGFRA overexpression, and prolonged survival, and IDH wild-type (IDH wt) tumors exhibiting EGFR amplification, PTEN loss, and unfavorable disease outcome. Furthermore, global expression profiling revealed three robust molecular subclasses within lower-grade diffuse astrocytic gliomas, two of which were predominantly IDH mt and one almost entirely IDH wt. IDH mt subclasses were distinguished from each other on the basis of TP53 mutations, DNA copy number abnormalities, and links to distinct stages of neurogenesis in the subventricular zone (SVZ). This latter finding implicates discrete pools of neuroglial progenitors as cells of origin for the different subclasses of IDH mt tumors. Project description:Background: Isocitrate dehydrogenase mutant (IDH-mut) glioma represents a distinct subtype of adult glioma. Although IDH-mut glioma displays slightly better prognosis among adult gliomas, these tumors are still incurable largely due to the lack of effective treatments. Targeting SHP2 has been shown to be a promising therapeutic approach in various types of malignancies, and our previous research demonstrated its efficacy against IDH wildtype (WT) glioblastoma (GBM). However, whether IDH-mutant gliomas are responsive to SHP2 inhibition remains unknown. Methods: Bioinformatic and biological analyses revealed increased expression and activation of PDGFRα-SHP2-ERK pathway in clinical IDH-mutant gliomas and patient-derived IDH-mut glioma stem-like cells (GSCs). Therapeutic effects of a SHP2 inhibitor on inhibition of the PDGFRα-SHP2-ERK signaling and tumorigenicity of IDH-mut GSCs, individually or in combination with radiation therapy (RT) were assessed by cell growth, glioma sphere formation, cell differentiation, flow cytometry, immunoblotting, orthotopic brain tumor xenograft, bioluminescent tracking, and immunohistochemistry. Results: We found a marked increase in PDGFRα expression in clinical IDH-mutant gliomas and IDH-mut GSCs, leading to the activation of the downstream SHP2-ERK pathway. Treatment with a blood-brain barrier-penetrable SHP2 inhibitor, SHP099 significantly inhibited GSC tumorigenic properties in vitro and in vivo by impeding SHP2-ERK axis and inducing cellular differentiation. Additionally, SHP099 treatment synergistically enhanced cytotoxicity of RT on IDH-mut GSCs in vitro and in orthotopic glioma xenografts, the standard treatment for IDH-mut glioma. Mechanistically, the PDGFRα -SHP2-ERK axis is activated in IDH-mut gliomas while radiation activates the SHP2-ERK pathway. Targeting SHP2 suppressed the activated ERK signaling thereby enhancing cytotoxicity of RT against IDH-mut gliomas. Conclusion: Targeting SHP2 in combination with RT for treating IDH-mut glioma displays promising therapeutic efficacy through suppressing a preferentially activated SHP2-ERK axis in IDH-mut gliomas.

However, the genetic and cellular state changes that drive IDH-mutant glioma progression under treatment remain incompletely understood. Here, we integrated single-nucleus transcriptomic profiles, chromatin accessibility profiles and bulk DNA/RNA sequencing from 75 temporally separated gliomas across 35 patients comprising both the oligodendroglioma and astrocytoma IDH-mutant glioma tumor types. We show that malignant cell states transcriptionally resemble stages of normal glial-neuronal lineage development or a reactive mesenchymal-like state, mirroring states previously described in IDH-wildtype glioblastoma. Malignant cell states displayed distinct chromatin accessibility profiles that were comparable between both IDH-mutant glioma types. The abundance of less differentiated malignant cells increased with grade and with genetic alterations such as PDGFRA amplification. Longitudinal analysis highlighted two major malignant cell state transition patterns. First, reduced lineage differentiation and increased proliferative malignant cells at recurrence were pronounced in gliomas that acquired recurrence-associated genetic events, including treatment-associated hypermutation, increased copy number changes, and cell cycle alterations. Second, increased mesenchymal-like state abundance occurred independently of acquired genetic alterations and instead coincided with heightened macrophage expression.
