Project description:Individuals homozygous for the "Z" mutation in alpha-1 antitrypsin deficiency are known to be at increased risk for liver disease. It has also become clear that some degree of risk is similarly conferred by the heterozygous state. A lack of model systems that recapitulate heterozygosity in human hepatocytes has limited the ability to study the impact of a single Z alpha-1 antitrypsin (ZAAT) allele on hepatocyte biology. Here, we describe the derivation of syngeneic induced pluripotent stem cells (iPSCs) engineered to determine the effects of ZAAT heterozygosity in iPSC-hepatocytes (iHeps). We find that heterozygous MZ iHeps exhibit an intermediate disease phenotype and share with ZZ iHeps alterations in AAT protein processing and downstream perturbations including altered endoplasmic reticulum (ER) and mitochondrial morphology, reduced mitochondrial respiration, and branch-specific activation of the unfolded protein response in cell subpopulations. Our model of MZ heterozygosity thus provides evidence that a single Z allele is sufficient to disrupt hepatocyte homeostatic function.

Pulmonary gain-of-function toxicity in alpha-1 antitrypsin deficiency

Project description:Overall, we found that AAT is expressed in alveolar type 2 cells in addition to alveolar macrophages in primary human lung tissue. AT2s and alveolar macrophages in ZZ-AATD exhibit a distinct transcriptomic signature including inflammatory pathway and unfolded protein response gene set enrichment. AT2s in ZZ-AATD additionally exhibit evidence of cell stress including ATF4 regulon enrichment and expression of cleaved caspase 3. Together, these findings extend the prevailing paradigm of emphysema pathogenesis in ZZ-AATD and suggest AAT-expressing resident lung cells as logical therapeutic targets for future study.

RNA Editing for the Treatment of Alpha-1 Antitrypsin Deficiency

Project description:Alpha-1 antitrypsin deficiency (AATD) is both a gain- and loss-of-function disease that impacts the liver and lung.1 Most severe cases result from a homozygous missense mutation in the SERPINA1 gene (Z mutation). While current therapies and those in development may ameliorate lung or liver disease, few are designed to address both.1 We have developed SERPINA1-994, a N-Acetylgalactosamine-conjugated chemically modified oligonucleotide that elicits adenine-to-inosine RNA editing using endogenous adenosine deaminases acting on RNA enzymes, to correct SERPINA1 Z transcripts. We show that SERPINA1-994 edits 50% of the Z transcript in hepatocytes of NSG-PiZ mice, which increases total serum AAT levels and induces the production of wild-type protein. SERPINA1-994 addresses loss of AAT function in lung by increasing the neutrophil elastase inhibition capacity of mouse serum and gain-of-function in liver by decreasing protein aggregation, decreasing inflammation and correcting gene expression patterns. SERPINA1-994 relies on a clinically proven delivery technology and directs highly specific RNA rather than DNA editing, so there is no risk for permanent off-target effects. Overall, these data suggest that SERPINA1-994 changes a ZZ homozygous state, which is associated with a high risk for AATD, to a low-risk MZ-like phenotype.

Single cell RNA sequencing of AATD iPSC-derived hepatocytes following Z mutation correction by base-editing

Project description:We performed single cell transcriptomic profiling of cells derived from induced human pluripotent stem cells (iPSCs) using hepatocyte directed differentiation protocol which derived iHeps. We used the alpha-1 antitrypsin deficiency (AATD) iPSC line, PiZZ1. On day 15 of differentiation, we transfected cells with base editors to correct the AATD Z mutation. On day 21 of differentiation, we captured cells for 10x Genomics Single cell capture (V3 protocol). Control cells (MZ and MM stably corrected iHeps) were included in 1 lane, mixed at a ratio of 1:1 and then genotype was identified using the Vartrix Single-Cell Genotyping tool (10X Genomics). Louvain clustering identified 4 major clusters of cells, including a cluster of cells undergoing ER stress. Base editing decreased the number of cells undergoing ER stress.

Modeling Familial Cancer with iPSC Approaches

Project description:In vitro modeling of human disease has recently become feasible with the adoption of induced pluripotent stem cell (iPSC) technology. Here, we established patient-derived iPSCs from an Li-Fraumeni Syndrome (LFS) family and investigated the role of mutant p53 in the development of osteosarcoma (OS). Several members of this family carried a heterozygous p53(G245D) mutation and presented with a broad spectrum of tumors including OS. Osteoblasts (OBs) differentiated from iPSC-derived mesenchymal stem cells (MSCs) recapitulated OS features including defective osteoblastic differentiation (OB differentiation) as well as tumorigenic ability. Systematic analyses revealed that the expression of genes enriched in LFS-derived OBs strongly correlated with decreased time to tumor recurrence and poor patient survival. In silico cytogenetic region enrichment analysis (CREA) demonstrated that LFS-derived OBs do not have genomic rearrangements and hence are a particularly valuable tool for elucidating early oncogenic events prior to the accumulation of secondary alterations. LFS OBs exhibited impaired upregulation of the imprinted gene H19 during osteogenesis. Restoration of H19 expression in LFS OBs facilitated osteogenic differentiation and repressed tumorigenic potential. By integrating human imprinted gene network (IGN) and functional genomic analyses, we found that H19-mediates suppression of LFS-associated OS through the IGN component DECORIN (DCN). Downregulation of DCN impairs H19-mediated osteogenic differentiation and tumor suppression. In summary, these findings demonstrate the feasibility of studying inherited human cancer syndromes with iPSCs and also provide molecular insights into the role of the IGN in p53 mutation-mediated tumorigenesis. mRNAseq profiling during mesenchymal stem cell differentiation to osteoblasts.
