Patient 6 and patient 10 CAR-NK study

ABSTRACT: To evaluate whether the findings of our pre-clinical study hold true in patients treated with CAR19/IL-15 NK cells, we isolated and profiled CAR-NK cells and B cells from peripheral blood collected at multiple timepoints from two patients diagnosed with CD19+ relapse/refractory hematological malignancies and treated with CAR19/IL-15 NK cells (11). Cord blood donor characteristics determine the efficacy of IL-15 armored CAR-transduced natural killer cells against CD19-positive B cell tumors

Project description:We conducted a phase I-II clinical trial at our institution that was designed to assess the safety and efficacy of escalating doses of CAR19/IL-15 CB-NK cells as treatment for relapsed/refractory CD19-positive malignancies. NK cells from these optimal CBs were highly functional and enriched in effector-related genes. In contrast, NK cells from suboptimal CBs had upregulation of inflammation, cellular stress and apoptosis programs. Finally, using multiple mouse models, we confirmed the superior anti-tumor activity of CAR-NK cells from optimal CBs.

NK cells engineered with a IL-15/IL-15Rα complex and CD19-targeted CAR show in vitro expansion and tumor regression in a murine xenograft model of B-cell leukemia in vivo

Project description:Natural killer (NK) cells are an attractive therapeutic approach as alternative chimeric antigen receptor (CAR) carriers and unlike T cells, offer their use in allogeneic settings. The translation of NK cells into the clinic is still hampered by the inability of CAR-NK cells to expand in vitro and in vivo. Adoptive transfer of CAR-NK cells holds the promise of therapeutic benefit with a lower rate of adverse events due to the absence of GvHD and cytokine release syndrome. However, adoptive transfer of CAR-NK cells has not yet achieved breakthrough clinical results, and further improvement of CAR-NK cells is necessary. Here, we demonstrated that a fourth-generation CD19-targeted CAR (CAR.19) co-expressing IL-15 linked to its receptor IL-15/IL-15Rα (CAR.19-IL-15/IL-15Rα) significantly enhanced NK-92 cell proliferation, proinflammatory cytokine secretion, and cytotoxic activity against B-cell cancer cell lines in vitro and in a xenograft mouse model. Surprisingly, CAR construct comprising the IL-15/IL-15Rα was superior to CAR co-expressing the soluble form of IL-15 in terms of sustained proliferation, viability, but importantly also in their ability to control CD19+ B-cell lymphoma in a murine xenograft model. Together with the results of the systematic analysis of the transcriptome of activated NK-92 CAR variants, this supports the notion that IL-15/IL-15Rα comprising fourth-generation CARs may overcome the limitations of NK cell-based targeted tumor therapies in vivo by providing the necessary growth and activation signals

Robust differentiation of NK cells from MSLN.CAR-IL-15-engineered human iPSCs with enhanced anti-tumor efficacy against solid tumors [scRNA-seq]

Project description:Human induced pluripotent stem cells (iPSCs) offer a promising source for producing standardized, off-the-shelf CAR-NK products. However, the complex and time-consuming iPSC-NK (iNK) manufacturing challenges clinical use. Here, we identified LiPSC-GR1.1 as a superior iPSC line for iNK production. By engineering LiPSC-GR1.1 with a mesothelin (MSLN)-targeting CAR and IL-15, we achieved efficient robust differentiation of iPSCs into mature and activated iNKs, which demonstrated enhanced tumor-killing efficacy, superior tumor-homing, and vigorous proliferation. Single-cell transcriptomic analysis revealed that TGF-β-producing tumor cells upregulated MHC molecules and downregulated MSLN expression post-CAR-IL-15 iNK treatment. Tumor-infiltrating CAR-IL-15 iNKs exhibited high levels of CAR, IL-15, and NK-activating receptors, negligible checkpoint exhaustion markers, and extremely low levels of NK suppressive factors CISH, TGFBR2, and BATF, enabling them to sustain activation, metabolic fitness, and effective tumor killing within the TGF-β-rich hypoxic tumor microenvironment. Overall, we developed a MSLN.CAR-IL-15-engineered GR1.1-iNK product with enhanced anti-tumor efficacy for solid tumor treatment.

B cell directed CAR-T cell therapy results in activation of CD8+ cytotoxic CAR-negative bystander T cells in both non-human primates and patients

Project description:There is growing appreciation for the emergence of CARneg bystander T cells after CAR-T cell infusion. However, their phenotypic and transcriptomic hallmarks and mechanisms of activation remain uncertain. We performed single-cell RNA-Seq (scRNA-Seq) on non-human primate (NHP) and patient-derived T cells to interrogate CARneg T cells following B cell targeted CAR-T cell therapy. In a NHP model, we observed a distinct population of activated CD8+ CARneg T cells emerging during CAR-T cell expansion. These bystander CD8+ CARneg T cells exhibited a unique transcriptional signature with upregulation of NK-cell markers (KIR3DL2, CD160, KLRD1), chemokines and chemokine receptors (CCL5, XCL1, CCR9), and downregulation of naive T cell-associated genes (SELL, CD28). A transcriptionally similar population was identified in patients following Tisangelecleucel infusion. Mechanistic studies revealed that IL-2 and IL-15 exposure induced bystander-like CD8+ T cells. These T cells efficiently killed leukemic cells through a TCR-independent mechanism. Together, these data identify bystander CD8+ T cells as a novel mechanism by which CAR-T cell infusion can induce further anti-leukemic activity, measurable in both NHP and in patients.

IL-15 activates mTOR and primes stress-activated gene-expression leading to prolonged anti-tumor capacity of NK cells

Project description:Treatment of hematological malignancies by adoptive transfer of activated natural killer (NK) cells is limited by poor post-infusion persistence. We compared the ability of interleukin-2 (IL-2) and IL-15 to sustain human NK cell functions following cytokine withdrawal to model post-infusion performance. In contrasts to IL-2, IL-15 mediated stronger signaling through the IL-2/15 receptor complex and provided functional advantages. Genome-wide analysis of cytosolic and polysome-associated mRNA revealed cytokine dependent differential mRNA levels and translation during cytokine activation but also that most gene expression differences were primed by IL-15 and only manifested after cytokine withdrawal. IL-15 augmented mTOR signaling, which correlated with increased expression of genes related to cell metabolism and respiration. Consistently, mTOR inhibition abrogated IL-15-induced functional advantages. Moreover, mTOR-independent STAT-5 signaling contributed to improved NK cell function during cytokine activation but not following cytokine withdrawal. The superior performance of IL-15 stimulated NK cells was also observed using a clinically applicable protocol for NK cell expansion. Finally, expression of IL-15 correlated with cytolytic immune functions in patients with B cell lymphoma and favorable clinical outcome. These findings highlight the importance of mTOR regulated metabolic processes for immune cell functions and argue for implementation of IL-15 in adoptive NK cell cancer therapy.

We found thata CREM is a regulatory checkpoint induced by CAR CD3z signaling and IL-15 stimulation that leads to suppression of NK cell function. CREM KO enhances the anti-tumor efficacy of CAR NK cells.
