Project description:In this study participants were experimentally challenged with an infective dose of S. Typhi (1-5x10^3-4 cfu) and followed over at least 14 days. Every study participant was treated with antibiotics (ciprofloxacin, 500 mg twice daily, 14 days) on day 14 after challenge unless diagnosed with typhoid fever at which time treatment commenced immediately (ciprofloxacin, 500 mg twice daily, 14 days). For details about the study design please refer to Waddington et al. CID 2014 May;58(9):1230-40. Criteria for typhoid diagnosis were either microbiological (≥1 positive blood culture collected after day 5) and/or clinical (fever ≥38°C sustained for ≥12hrs). 3mL of venous blood was obtained at pre-challenge baselines, 24hrs prior typhoid diagnosis, day of diagnosis, 24hrs post-diagnosis and day 14 after challenge in those diagnosed with acute typhoid fever. In those who did not develop typhoid despite oral challenge samples were obtained at pre-challenge, day 7 and day 14 after challenge. The samples are labelled with one of the following accronyms: V1: Day 0, i.e. pre-challenge control. D7: day 7 after challenge. TD-24: 24hrs prior to typhoid diagnosis. TD: day of typhoid diagnosis. TD+24: 24hrs after typhoid diagnosis. D14: day 14 after challenge.

Project description:In this study, whole blood samples were used to determine the gene expression of healthy adult volunteers experimentally challenged with S. Typhi Quailes strain. Samples were collected at baseline (pre-challenge; Group: CTRL) and 7 days after challenge (Group: sEF) in individuals who did not reach diagnostic criteria. In those reaching the diagnostic endpoint (typhoid fever; Group: EF) the sample on the day of diagnosis when antibiotic treatment commenced was taken. These samples were derived from the placebo arm of a recent a Single Centre, Randomised, Doubleblind, Placebo Controlled Study to Evaluate M01ZH09 in a Healthy Adult Challenge Model, Using Ty21a Vaccine as a Positive Control. Project description:The whole blood transcriptome 12 hours after challenge with S. Paratyphi A. Samples were collected in a study aiming to find the challenge dose giving a 60-75% attack rate, published in Dobinson et al. PMID: 28158395. Participants were challenged with S. Paratyphi A NVGH308 strain suspended in sodium bicarbonate. Enteric fever was diagnosed based on oral temperature or positive blood culture. Participants were treated with antibiotics either following diagnosis or 14 days without diagnosis. Paired baseline samples are available under Series GSE114033.

Project description:Salmonella Typhi secretes typhoid toxin that activates cellular DNA damage responses (DDR) during acute typhoid fever. Human infection challenge studies revealed that the toxin suppresses bacteraemia via unknown mechanisms. By applying proteomics to the plasma of bacteraemic participants, we identified that wild-type toxigenic Salmonella induced release of lysozyme (LYZ) and apolipoprotein C3 (APOC3). LYZ and APOC3 were expressed and secreted in cultured cells treated with purified typhoid toxin or infected with toxigenic Salmonella. Toxin-induced LYZ and APOC3 were dependent upon DDRs and could be inhibited by targeting kinases ATM and ATR. LYZ caused loss of Salmonella morphology characterised by spheroplast formation. Spheroplast formation was mediated by LYZ with EDTA or lactoferrin, which was identified by proteomics in participants with typhoid fever. LYZ alone inhibited secretion of T3SS virulence effector proteins SipB and SopE. Our findings indicate that toxin-induced DDRs elicit antimicrobial responses, which suppress Salmonella bacteraemia during typhoid fever.

Project description:Typhoid fever is caused by the Gram-negative bacterium Salmonella enterica serovar Typhi. Bulk RNA-sequencing (RNA-seq) data were generated from blood samples obtained from adult human volunteers enrolled in a vaccine trial involving two vaccines against typhoid fever, a plain polysaccharide vaccine, ViPS and a glycoconjugate vaccine, ViTCV. The participants were then challenged with S. Typhi in a controlled human infection model (CHIM).

Project description:Patient Selection: Three hospitals were involved in the study: Dong Thap Provincial and An Giang Provincial hospitals, both located in the Mekong Delta Region of Vietnam where most of the Typhoid patients were admitted, and the Hospital for Tropical Diseases, Ho Chi Minh City, where the malarial patients were admitted. Infection status of each patient was determined by blood culture for the Typhoid patients and by microscopic examination of stained thick and thin peripheral-blood slides for the patients suspected of having Malaria. Patients with blood cultures positive for S. Typhi or S. enterica serovar Paratyphi A (S. Paratyphi A) (samples designated ST) or peripheral blood smears positive for Plasmodium falciparum or Plasmodium vivax (samples designated PF) were consented and enrolled in the study. A total of 28 S. Typhi positive patients and one S. Paratyphi A positive patient were enrolled in the first phase of the study and a further 9 S. Typhi and one S. Paratyphi A positive patient were enrolled in the second phase. Nine patients with Malaria caused by P. falciparum and one caused by P. vivax were enrolled. Sixteen uninfected healthy individuals (samples designated HC) were also enrolled in the study (one sample each). The Typhoid patients were randomly chosen to receive either Azithromycin or Gatifloxicin (20mg/kg/day for 7 days) antibiotics. Uncomplicated malaria patients were treated for 3 days with Artekin (dihydroartemisinin with piperaquine phosphate). Venous blood (2.5ml) was collected for total RNA extraction into PaxGene RNA collection tubes (Qiagen) at various times. The first sample (T1) was taken immediately on admission or entry into the study, then samples were collected, 3 (T3), 6-9 (T9 for Typhoid, T7 for Malaria), and 28-45 (T28) days following entry into the study and an average of 9 months (T9M) later. The PaxGene tubes were stored at 4degC until RNA was extracted as per the manufacturers instructions. Purified RNA was shipped on dry ice to Stanford University, CA for use in Microarray analyses. Microarrays: Experimental and reference samples (Human Universal RNA reference {Stratagene}) were amplified using the MessageAmp II aRNA Kit (Ambion) as per the manufacturer's instructions. Four micrograms amplified RNA was labeled through indirect incorporation of Cy5 (experimental) and Cy3 (reference) dyes (protocols can be found at http://cmgm.stanford.edu/pbrown/protocols/index.html). The labeled probes were then mixed and hybridized to Stanford Human cDNA arrays with 42 K features. Arrays were scanned using a Genepix 4000A laser scanner and data extracted with Genepix Pro Software version 5.1 (Axon Instruments). The data were then uploaded into the Stanford Microarray Database (SMD). Project description:Enteric fever is a major public health problem and causes numerous deaths annually. Ty21a is the only efficacious oral, live attenuated typhoid vaccine currently licensed for use, however, its mechanism of protection is poorly understood. To address this knowledge gap, we interrogated transcriptional profiles following vaccination with Ty21a and an immunogenic experimental oral live attenuated vaccine, M01ZH09, and related these findings to immunogenicity, and incubation period and disease severity following challenge with Salmonella Typhi four weeks after vaccination. Despite originating from the same parent strain (Ty2), we detected marked differences in the gene expression between both vaccines. Analysis of the transcriptome 7 days after M01ZH09 vaccination implicated transcriptional patterns associated with the cell cycle correlated significantly with humoral immunogenicity 28 days after vaccination. In contrast, significantly induced T and NK cell responses were associated with Ty21a vaccination, and integrative analysis indicated signatures reflecting amino acid metabolism with delayed onset of disease. Stimulation of PBMCs collected from participants prior to and following vaccination with the two vaccine strains in vitro confirmed the superior capacity of Ty21a to induce NK cells, validating gene expression results. These data provide insight into the effects of oral live attenuated typhoid vaccines on the human molecular immune response and underline the involvement of T cell response signatures with protection following challenge.

Typhi, studies of typhoid pathogenesis in animal models are limited to a murine non-typhoidal pathogen. More recently, human challenge models have been conducted, providing insight into immune correlates of infection outcomes, which are still incompletely understood. Here, we performed an integrated single-cell analysis of immune responses from the human S. Typhi challenge model and mouse model of typhoid disease, to associate biological mechanism with human infection outcome. Most prominent, we revealed immune subsets with a hypoxia-related signature in the blood of individuals who developed disease in the human challenge model. This signature was also evident in the mouse model in activated macrophages infiltrating into the Peyer's patches, but not during infection with a mutant strain impaired for gut invasion. We further identified hypoxia-related signature as a general immune correlate of disease outcome in other infection-and inflammatory-related diseases. Collectively, we identified a hypoxia-associated immune signature that correlates with disease outcomes in humans. Using a mouse model, we demonstrated that this signature is driven by bacterial invasion to the Peyer's patches, implicating a causal role in the pathogenesis of typhoid fever.
