Hes6 drives a network with therapeutic potential in castrate-resistant prostate cancer

Project description:Castrate-resistant prostate cancer (CRPC) is poorly characterized and heterogeneous and while the androgen receptor (AR) is of singular importance in early prostate cancer, other factors such as c-Myc and the E2F family also play a role in later stage disease. Hes6 is a transcription co-factor that has been associated with neurogenesis during gastrulation, a neuroendocrine phenotype in the prostate and metastasis in breast cancer but its role in prostate cancer remains uncertain. Here we show that Hes6 is controlled by c-Myc and AR and drives castration resistance in prostate cancer. Hes6 activates a cell-cycle enhancing transcriptional network that maintains tumour growth and nuclear AR localization in castrate conditions. We show aphysical interaction between E2F1 and both Hes6 and AR, and suggest a co-dependency of these transcription factors in castration-resistance. In the clinical setting, we have uncovered a Hes6-associated signature that predicts poor outcome in prostate cancer, which can be pharmacologically targeted. We have therefore shown for the first time the critical role of Hes6 in the development of CRPC and identified its potential in patient specific therapeutic strategies. A single arm, prospective Phase II study of Split-Course PelvicRadiotherapy for Locally Progressive, Castrate Resistant Prostate Cancer

Project description:Interventions: Split-course pelvic radiotherapy will consist of a dose of 55Gy in 22 fractions. This dose will be prescribed as a course of 25Gy in 10 fractions given over 2 weeks, followed by a planned 1 week treatment break, followed by a further 30Gy in 12 fractions given over 2 1/2 weeks. Primary outcome(s): The primary objective of the trial is to test the hypothesis that split-course hypo-fractionated pelvic radiotherapy improves bladder health related quality of life in patients with local symptoms associated with castrate resistant prostate cancer (CRPC). Castrate Resistant Prostate Cancer Response to Taxane is Determined by an HNF1-dependent Apoptosis Resistance Circuit (RNA-Seq)

Project description:Metastatic castrate-resistant prostate cancer (mCRPC) is a genetically and phenotypically heterogeneous cancer where advancements are needed in biomarker discovery and targeted therapy. A critical and often effective component of treatment includes taxanes. We performed one of the first high throughput screens across a cohort of 30 diverse patient-derived CRPC organoids to a library of 78 drugs. Combining quantitative response measures with transcriptomic analyses demonstrated that HNF1 Homeobox A (HNF1A) drives a transcriptional program of taxane resistance, dependent upon cellular inhibitor of apoptosis protein 2 (cIAP2). Monotherapy with cIAP2 inhibitor LCL161 was sufficient to treat HNF1A+ models of mCRPC previously resistant to docetaxel.
