Project description:Leprosy is classified from paucibacillary leprosy (TT and BT) to multibacillary leprosy (BB, BL, and LL). Project description:Effective innate immunity against many microbial pathogens requires macrophage programs that upregulate phagocytosis and direct antimicrobial pathways, two functions generally assumed to be coordinately regulated. Here the regulation of these key functions was investigated in human blood-derived macrophages. IL-10 induced the phagocytic pathway, including CD209 and scavenger receptors, resulting in phagocytosis of mycobacteria and oxLDL. IL-15 induced the vitamin D-dependent antimicrobial pathway and CD209, yet the cells were less phagocytic. The differential regulation of macrophage functional programs was confirmed by analysis of the spectrum of leprosy lesions: the macrophage phagocytosis pathway was prominent in the clinically progressive, multibacillary form, whereas the vitamin D-dependent antimicrobial pathway predominated in the self-limited form of the disease and in patients undergoing reversal reactions from the multibacillary to the self-limited form. These data indicate that macrophage programs for phagocytosis and antimicrobial responses are distinct and differentially regulated in innate immunity in bacterial infections. Project description:8 leprosy patients including 4 multibacillary (MB) and 4 paucibacillary (PB), and 8 non-leprosy controls including 4 healthy house contacts (HHCs) and 4 endemic controls (ECs) were included in the study. The immune response differences between leprosy patients and controls were evaluated by analyzing the transcriptional profiles of PBMCs to M. leprae sonicate antigens by RNA-seq. The analyses revealed potential biomarkers (including mRNAs and lncRNAs) preferentially expressed in PBMCs in leprosy patients that may be useful for early diagnosis of leprosy.

Project description:Acute inflammatory episodes are common in leprosy and complicate the course of the disease. Among these episodes, Erythema Nodosum Leprosum (ENL) is more common in multibacillary patients and can cause more systemic symptoms like fever, myalgia, edema, and pain. Today, ENL is managed with thalidomide and corticosteroids, yet the exact mechanisms behind ENL remain elusive and with few pharmacological options available. In this experiment, RNA was sequenced and gene expression was measured from the blood of multibacillary patients with and without ENL in a mixed design, comparing ENL to MB and ENL before and after thalidomide treatment for 7 days.

Project description:Here we report the data generated by short-read sequencing of mRNA (polyA) isolated from granuloma annulare and leprosy skin lesions. Our main aim was to identify putative mRNA biomarkers for distinguishing leprosy from other differential diagnoses. Additionally, we also explored the distinction between MB and PB by using differential expression analysis as well as training a penalized logistic regression to select important features. Our results showed that few genes are enough to differentiate leprosy lesions, including paucibacillary cases, from other morphological and histopathological similar skin diseases. Some of these genes have been replicated in a larger and more heterogeneous sample with RT-qPCR, validating their classification potential. We also suggest important novel gene candidates to improve our understanding of molecular differences between MB and PB lesions, which could either pinpoint new pathways and targets for host-based specialized adjuvant treatment for leprosy. Finally, this dataset has been used to explore the relationship between cornification and keratinocyte-related genes and TGFB-mediated epithelial-mesenchymal transition (EMT), which could indicate that in skin, M. leprae could be de-differentiating, directly or indirectly, other cell types into a progenitor/stem-like phenotype, facilitating mycobacterial survival and migration within the host. Alternatively, this could highlight which pathways are activated during granuloma formation and/or skin barrier assembly/disassembly.

Project description:To further development of our gene expression approach in different groups of leprosy, we have employed whole genome microarray expression profiling as a discovery platform to identify genes with the potential to distinguish leprosy reactions and relapse cases from healthy controls. Human biopsy and skin slit were collected from reaction cases, PB cases, MB cases, relapse cases and healthy contacts. Total 1891,1384, 1134, 475 and 762 no of genes were overexpressed in PB, T1R, T2R, LL and relapse cases respectively. Expression of seven genes (glcB, infB, ftsH, ppgk,relA, rpsR and atpB) from relapse cases was quantified in the same RNA samples by real-time PCR confirming role of theses genes in relapse cases.

Project description:In this study, consistent gene expression changes were analysed to identify gene-to-gene interaction and functional pathway relationships associated with the MAP exposure outcomes of either a paucibacillary or a multibacillary disease form or sheep resilient to disease. The results suggest that although groups of genes are differentially changed in the MAP exposed sheep regardless of clinical outcome, there are distinct variations in gene expression patterns between the paucibacillary and multibacillary forms of disease and of particular note was the finding that MAP exposed animals with evidence of recovery or resilience respond to MAP exposure with a pattern of gene and molecular pathway interactions distinct from those animals that progress to clinical paratuberculosis.

Project description:<p>Leprosy is a chronic infectious disease caused by <i>Mycobacterium leprae</i> (<i>M. leprae</i>). Due to <i>M. leprae's</i>
narrow host range and an inability to be cultured <i>in vitro</i>, the biological investigation of this disease has been difficult. Host
genetic factors have been suggested to play an important role in disease development, but few have been identified.</p>
<p>In this study, we attempted to identify the host genetic factors by performing a two-stage genome-wide association
study (GWAS) in Chinese population. The initial genome-wide scan was done by genotyping 706 patients and 1225 controls using
the Illumina HumanHap610 BeadChip, and the follow-up study was performed by genotyping 93 SNPs in three independent samples
consisting of 3254 cases and 5955 controls. We identified significant association (P<10<sup>-10</sup>) within six genes
CCDC122 (13q14), C13orf31 (13q14), NOD2 (16q12), TNFSF15 (9q32), HLA-DR (6p21) and RIPK2 (8q21), and suggestive
association (P=5.68x10<sup>-6</sup>) within LRRK2 (12q12). We also revealed suggestive evidence for C13orf31, LRRK2,
NOD2 and RIPK2 to show stronger association in the multibacillary form than the paucibacillary form of leprosy. Our
findings highlight the importance of the innate immune response, particularly NOD2-mediated signaling, in leprosy
and suggests a new therapeutic target for leprosy.</p>
