Circulating miRNAs as Biomarkers for Arsenic-Induced Hepatotoxicity and Systemic Health Impacts in a Rural Pakistani Population


ABSTRACT: Arsenic (As) exposure through contaminated drinking water poses significant health risks, particularly in rural populations with limited access to clean water. This study investigated the impact of chronic As exposure on circulating microRNA (miRNA) profiles as potential biomarkers for organ injury, including hepatotoxicity. Plasma samples from 57 individuals in a rural population of Punjab, Pakistan, were collected and stratified into low, medium, and high As exposure groups based on urinary As levels. Small RNA sequencing identified 15 miRNAs associated with As exposure. Among them, three liver-enriched miRNAs—miR-122-3p, miR-194-5p, and miR-192-5p—were significantly negatively correlated with urinary As levels, suggesting their role in As-induced liver toxicity. Additionally, brain-enriched miRNAs, including miR-92b-3p, were positively associated with As exposure, while miR-376b-3p was negatively associated, indicating potential neurological effects. Pathway enrichment analysis revealed significant associations between these miRNAs and key biological pathways, including oxidative stress, lipid metabolism, and inflammatory responses. Our findings underscore the potential of circulating miRNAs in non-invasive biomonitoring of environmental toxin exposure and provide mechanistic insights into As-induced health effects, including carcinogenic risks and neurological impacts.
