Project description:More humans have died of tuberculosis (TB) than any other infectious disease and millions still die each year. Experts advocate for blood-based, protein biomarkers to help diagnosis TB, which is critical in high-burden countries. However, the protein biomarker pipeline is small. Here, we used the Diversity Outbred (DO) mouse population to address this gap, identifying five protein biomarker candidates. One biomarker, serum CXCL1, met the World Health Organization’s Targeted Product Profile for a triage test to diagnose active TB from latent M.tb infection (LTBI), non-TB lung disease, and normal sera in HIV-negative, adults from South Africa and Vietnam. To find the biomarker candidates, we quantified seven immune mediators and four proteins corresponding to a subset of highly expressed genes unique to progressor DO mice. Next, we applied statistical and machine learning methods to the data, i.e., 11 proteins in lungs from 453 infected and 29 non-infected mice. After searching all combinations of five algorithms and 239 protein subsets, validating, and testing the findings on independent data, two combinations accurately diagnosed progressor DO mice: Logistic Regression using MMP8; and Gradient Tree Boosting using a panel of 4: CXCL1, CXCL2, TNF, IL-10. Of those five protein biomarker candidates, two (MMP8 and CXCL1) were crucial for classifying DO mice; were above the limit of detection in most human serum samples; and had not been widely assessed for diagnostic performance in humans before. In patient sera, CXCL1 exceeded the triage diagnostic test criteria (>90% sensitivity; >70% specificity), while MMP8 did not. Using Area Under the Curve analyses, CXCL1 averaged 94.5% sensitivity and 88.8% specificity for active pulmonary TB (ATB) vs LTBI; 90.9% sensitivity and 71.4% specificity for ATB vs non-TB; and 100.0% sensitivity and 98.4% specificity for ATB vs normal sera. Our findings overall show that the DO mouse population can discover diagnostic-quality, serum protein biomarkers of human TB.

Project description:. In this study we identify a unique signature protein and metabolic biomarker panel for those who improve in cognition and those who decline, assessed by the Montreal Cognitive Assessment (MoCA). We investigate a discovery cohort of Brain in Motion participants involving a quasi-experimental aerobic exercise intervention study and analyze blood plasma before and after a six-month aerobic exercise intervention and 5 years later. We relate the differentially expressed proteins and metabolites to subjective sleep quality measures and fitness level. This study informs on the differences in the plasma of those who decline and those who improve in cognition, identifying pathways that are of importance for preventing cognitive decline.

Project description:Previous studies suggest that monocytes are an important contributor to tuberculosis (TB)-specific immune signatures in blood. Here we carried out single-cell profiling of classical CD14+CD16- and intermediate CD14+CD16+ monocytes in paired blood samples of active TB (ATB) patients at diagnosis and end-treatment. At diagnosis, ATB patients displayed upregulation of interferon signaling genes that significantly overlapped with previously reported blood TB signatures in both CD14+ subsets. In ATB diagnosis, we identified additional transcriptomic and functional changes in intermediate CD14+CD16+ monocytes, such as the upregulation of inflammatory and MHC-II genes, and increased capacity to activate T cells, reflecting overall increased activation in this population. Single-cell transcriptomics revealed that distinct subsets of intermediate CD14+CD16+ monocytes were responsible for each gene signature, indicating significant functional heterogeneity within this population. Finally, we observed that transcriptomic changes in CD14+ monocytes were transient, as they were no longer observed in the same ATB patients mid-treatment, suggesting they are associated with disease resolution.

Project description:We performed RNA-seq analysis on exosomes derived from the clinical specimens of healthy control (HC), active tuberculosis (ATB) and latent tuberculosis infection (LTBI) individuals. Our results revealed the distinguished gene expression panels and patterns of the exosomes for the LTBI and ATB patients: We identified many up-regulated and down-regulated differentially expressed genes (DEGs) in the LTBI and ATB samples, and further screened the top-20 DEGs, which might provide a clue to differentiate HC, LTBI and ATB; We classified all the DEGs into six expression patterns, screened the top-20 genes in each pattern, and mainly focused on those highly expressed in LTBI and ATB; A lot of Mtb genes were only expressed and enriched in the exosomes of LTBI patients; Pathway and functional analysis further indicated the gradually increased deteriorated healthy signals in LTBI and ATB samples, including down-regulated signaling pathways/immune response, and up-regulated apoptosis/necrosis. Our findings not only add new data to tuberculosis clinical studies, but also facilitate the development of potential targets for the diagnosis, prevention and treatment of tuberculosis.

Project description:The deleterious impact of antibiotics (ATB) on the microbiome negatively impacts immune checkpoint inhibitor (ICI) response. We assessed the efficacy of DAV132, a colon-targeted adsorbent in 70 healthy volunteers (HV) randomized to receive ceftazidime-avibactam or piperacillin-tazobactam alone or in combination with oral administration of DAV132. DAV132 significantly decreased fecal but not plasma ATB concentration. When coadministered with either ATB, DAV132 prevented loss of microbiome diversity and induced rapid recovery of the baseline microbiota composition. Moreover, bacterial probe set qPCR-based assay confirmed metagenomics results that DAV132 preserved several commensals such as Alistipes shahii, Blautia obeum and Faecalibacterium praunsnitzii. Fecal microbiota transplantation in murine tumor models from HV treated with ATB alone reduced antitumor responses to anti-PD-1, while transplanted samples from HV treated with ATB+DAV132 circumvented resistance to anti-PD-1. This anti-tumor response in mice was associated with tumor microenvironment increased the ratio of total CD8+ T cells/ regulatory T cells, and three distinct activated CD8+ T cell population whiting the tumor within the tumor, as well as gene expression of interferon gamma (INFγ) in the mesenteric lymph nodes. In addition, a unique gene signature with downregulation of IL-6 and reactive oxygen pathways was found in the mice treated with FMT from ATB+DAV132 sample and treated with anti-PD-1. DAV132 represents a new strategy for overcoming ATB-related dysbiosis and further studies are warranted to evaluate its efficacy in cancer patients on ICI.

Project description:Circular RNAs (circRNAs) play a critical role in pathological mechanisms of Mycobacterium tuberculosis (Mtb) and can be used as a new biomarker for active tuberculosis (ATB) diagnosis. Therefore, we identified significantly dysregulated circRNAs in ATB patients and healthy controls (HC) and explored its molecular mechanism. We found that hsa_circ_0002371 was significantly up-regulated in PBMCs of ATB patients and H37Rv- or BCG-infected THP-1 human macrophages. Functional experiments demonstrated that hsa_circ_0002371 inhibited autophagy of BCG-infected THP-1 human macrophages and promoted intracellular BCG survival rate. Mechanistically, hsa_circ_0002371 promoted the expression of hsa-miR-502-5p, and hsa_circ_0002371 overexpression-induced protective effects in BCG-infected THP-1 human macrophages was largely overturned by the inhibition of hsa-miR-502-5p. Notably, hsa-miR-502-5p inhibited autophagy via suppressing autophagy related 16 like 1 (ATG16L1) in BCG-infected macrophages and thus promoting intracellular BCG growth. In summation, hsa_circ_0002371 increased the suppression of hsa-miR-502-5p on ATG16L1 and inhibited autophagy to promote Mtb growth in macrophages. In Conclusion, our data suggested that hsa_circ_0002371 was significantly up-regulated in the PBMCs of ATB patients compared with HC. The hsa_circ_0002371/hsa-miR-502-5p/ATG16L1 axis promoted the survival of intracellular Mtb and inhibited autophagy in macrophages. Our findings suggested hsa_circ_0002371 could act as a potential diagnostic biomarker and therapeutic target.

Project description:We developed the Peptide-Driven Identification of TCRs (PDI-TCR), an approach that leverages antigen-specific peptide pools, bulk TCR sequencing, and statistical analyses to pinpoint antigen-specific TCRs. We discerned antigen-specific TCRs within a group of active tuberculosis (ATB) patients and individuals with a positive Interferon-γ release assay (IGRA+). We found Mtb-specific effectors to have an HLA-DR+ Th1* phenotype, and to be exclusive to active disease. Conversely, HLA-DR- Mtb-specific T cells appeared in Th1 cells in both ATB and IGRA+ cohorts. We also identified Cross-reactive T cells, that were abundant ex vivo regardless of disease state having a cytotoxic phenotype.
