Project description:Early diagnosis increases the treatment success rate for active tuberculosis (ATB) and decreases mortality. MicroRNAs (miRNAs) have been studied as blood-based markers of several infectious diseases. We performed miRNA profiling to identify differentially expressed (DE) miRNAs using whole blood samples from 10 healthy controls (HCs), 15 subjects with latent tuberculosis infection (LTBI), and 12 patients with ATB, and investigated the expression of the top six miRNAs at diagnosis and over the treatment period in addition to performing miRNA-target gene network and gene ontology analyses. miRNA profiling identified 84 DE miRNAs in patients with ATB, including 80 upregulated and four downregulated miRNAs. Receiver operating characteristic curves of the top six miRNAs exhibited excellent distinguishing efficiency with an area under curve (AUC) value > 0.85. Among them, miR-199a-3p and miR-6886-3p can differentiate between ATB and LTBI. Anti-TB treatment restored the levels of miR-199b-3p, miR-199a-3p, miR-16-5p, and miR-374c-5p to HC levels. Furthermore, 108 predicted target genes were related to the regulation of cellular amide metabolism, intrinsic apoptotic signaling, translation, transforming growth factor beta receptor signaling, and cysteine-type endopeptidase activity. The DE miRNAs identified herein are potential biomarkers for diagnosis and therapeutic monitoring in ATB.

Project description:Previous studies suggest that monocytes are an important contributor to tuberculosis (TB)-specific immune signatures in blood. Here we carried out single-cell profiling of classical CD14+CD16- and intermediate CD14+CD16+ monocytes in paired blood samples of active TB (ATB) patients at diagnosis and end-treatment. At diagnosis, ATB patients displayed upregulation of interferon signaling genes that significantly overlapped with previously reported blood TB signatures in both CD14+ subsets. In ATB diagnosis, we identified additional transcriptomic and functional changes in intermediate CD14+CD16+ monocytes, such as the upregulation of inflammatory and MHC-II genes, and increased capacity to activate T cells, reflecting overall increased activation in this population. Single-cell transcriptomics revealed that distinct subsets of intermediate CD14+CD16+ monocytes were responsible for each gene signature, indicating significant functional heterogeneity within this population. Finally, we observed that transcriptomic changes in CD14+ monocytes were transient, as they were no longer observed in the same ATB patients mid-treatment, suggesting they are associated with disease resolution.

Project description:Sarcoidosis is a granulomatous systemic inflammatory disease predominantly affecting lungs. It shares histopathological, clinical and immune features with tuberculosis (TB). There are currently no diagnostic tests to formally identify sarcoidosis; clinicians first need to rule out the presence of other diseases, including TB. We characterized Mycobacterium tuberculosis (Mtb)-specific immune responses in blood of pulmonary sarcoidosis patients in comparison to active tuberculosis (ATB), Mtb-sensitized and non-sensitized healthy controls using flow cytometry and transcriptomics. We found that sarcoidosis was associated with i) a marked reduction in the frequency of antigen-reactive T cells in response to both Mtb peptides and Mtb lysate, ii) increased frequency of monocytes ex vivo and after in vitro stimulation, and iii) increased expression of phagocytic genes compared to Mtb-infected and healthy cohorts. Combination of Mtb-specific T cell and monocyte gene or flow cytometry signatures in Mtb-stimulated PBMC could efficiently distinguish sarcoidosis from ATB with high specificity and sensitivity, and to a lower extent from Mtb-sensitized and non-sensitized healthy controls. These biomarkers offer promising aspects for developing effective diagnostic tests for sarcoidosis, in particular for differential diagnosis from tuberculosis.

In this study, we observed that the lncRNA-Activated by TGF-β (lncRNA-ATB) was upregulated in hepatocellular carcinoma (HCC) metastases and associated with poor prognosis. lncRNA-ATB promotes the invasion-metastasis cascade, which suggest that lncRNA-ATB, a mediator of TGF-β signaling, could predispose HCC patients to metastases and may serve as a potential target for anti-metastatic therapies.

Project description:We performed RNA-seq analysis on exosomes derived from the clinical specimens of healthy control (HC), active tuberculosis (ATB) and latent tuberculosis infection (LTBI) individuals. Our results revealed the distinguished gene expression panels and patterns of the exosomes for the LTBI and ATB patients: We identified many up-regulated and down-regulated differentially expressed genes (DEGs) in the LTBI and ATB samples, and further screened the top-20 DEGs, which might provide a clue to differentiate HC, LTBI and ATB; We classified all the DEGs into six expression patterns, screened the top-20 genes in each pattern, and mainly focused on those highly expressed in LTBI and ATB; A lot of Mtb genes were only expressed and enriched in the exosomes of LTBI patients; Pathway and functional analysis further indicated the gradually increased deteriorated healthy signals in LTBI and ATB samples, including down-regulated signaling pathways/immune response, and up-regulated apoptosis/necrosis. Our findings not only add new data to tuberculosis clinical studies, but also facilitate the development of potential targets for the diagnosis, prevention and treatment of tuberculosis.

Project description:72 candidate biomarkers evaluated for TB diagnosis using mRNA purified from whole blood of Active TB patients recruited in the UK and India, LTB and two groups of controls from the UK (i) from a low incidence region and (ii) individuals variably domiciled in the UK and India. Tuberculosis (TB) remains a major global threat and diagnosis of active TB ((ATB) both extra-pulmonary (EPTB) and pulmonary (PTB)) and latent TB (LTBI) remains challenging, particularly in high-burden countries which still rely heavily on conventional methods. Although molecular diagnostic methods are available, e.g., Cepheid GeneXpert, they are not universally available in all high TB burden countries. There is intense focus on immune biomarkers for use in TB diagnosis, which could provide alternative low-cost, rapid diagnostic solutions. In our previous gene expression studies, we identified peripheral blood leukocyte (PBL) mRNA biomarkers in a non-human primate TB aerosol-challenge model. Here, we describe a study to further validate select mRNA biomarkers from this prior study in new cohorts of patients and controls, as a prerequisite for further development. PBL mRNA was purified from ATB patients recruited in the UK and India, LTBI and two groups of controls from the UK (i) a low TB incidence region (CNTRLA) and (ii) individuals variably-domiciled in the UK and Asia ((CNTRLB), the latter TB high incidence regions). Seventy-two mRNA biomarker gene targets were analyzed by qPCR using the Roche Lightcycler 480 qPCR platform and data analyzed using GeneSpring™ 14.9 bioinformatics software. Differential expression of fifty-three biomarkers was confirmed between MTB infected, LTBI groups and controls, seventeen of which were significant using analysis of variance (ANOVA): CALCOCO2, CD52, GBP1, GBP2, GBP5, HLA-B, IFIT3, IFITM3, IRF1, LOC400759 (GBP1P1), NCF1C, PF4V1, SAMD9L, S100A11, TAF10, TAPBP, and TRIM25. These were analyzed using receiver operating characteristic (ROC) curve analysis. Single biomarkers and biomarker combinations were further assessed using simple arithmetic algorithms. Minimal combination biomarker panels were delineated for primary diagnosis of ATB (both PTB and EPTB), LTBI and identifying LTBI individuals at high risk of progression which showed good performance characteristics. These were assessed for suitability for progression against the standards for new Tuberculosis diagnostic tests delineated in the published World Health Organization (WHO) technology product profiles (TPPs)
