Multiomics Analyses Unveil Immune Landscape in Pediatric and Adult Sepsis [bulk RNA-seq]

ABSTRACT: Sepsis is defined as a life-threatening organ dysfunction caused by a dysregulated host response to infection. The immune response varies significantly based on the anatomical source of infection. This study investigates the transcriptomic profiles associated with sepsis using bulk RNA sequencing. A total of 207 PBMC samples were analyzed, including those from healthy adults, adult ICU controls, adult sepsis patients, healthy children, pediatric ICU controls, and pediatric sepsis patients. The analysis of this dataset reveals both shared and source-specific gene expression patterns, providing insights into the molecular mechanisms underlying sepsis and informing the development of targeted therapeutic strategies.

Project description:Sepsis is a leading cause of global mortality, characterized by organ dysfunction due to a dysregulated immune response to infection. This study aims to delineate the immune landscape in sepsis through a combined analysis of single-cell RNA sequencing (scRNA-seq) with TCR and BCR sequencing. A total of 133 samples were analyzed, including PBMCs from healthy adults, adult ICU controls, adult sepsis patients, healthy children, pediatric ICU controls, and pediatric sepsis patients. The integration of scRNA-seq with TCR and BCR sequencing revealed both common and unique immune cell populations across different infection sources and between adults and children. These findings provide insights into the pathophysiology of sepsis and suggest potential targets for personalized therapeutic approaches.

This study uses cellular indexing of transcriptomes and epitopes by sequencing (CITE-seq) to profile surface protein expression and transcriptomics at the single-cell level in PBMC samples from 40 adults: 3 healthy controls, 5 ICU controls, and 32 sepsis patients. The sepsis patients are categorized by infections originating from different anatomical sites. The findings aim to support sepsis stratification and guide the development of targeted therapies based on infection sources.

Unique Patients from the Genomics of Pediatric SIRS and Septic Shock Investigators (GPSSSI)

Project description:This dataset is composed of the unique patients (276; at the Day 1 timepoint) that are present in the six other GEO datasets published by Hector Wong and the Genomics of Pediatric SIRS and Septic Shock Investigators. This dataset thus includes all unique patients from GSE4607, GSE8121, GSE9692, GSE13904, GSE26378, and GSE26440. These are only from the Day 1 timepoint. The original studies examined pediatric patients admitted to the ICU, who were later classified as either SIRS (non-infectious) or Sepsis or Septic Shock (infectious). There is also a group of healthy controls. Although the original studies examine patients at both ICU day 1 and ICU day 3, here we have only aggregated ICU day 1 patients. All samples here were downloaded as .CEL files and re-normalized together using gcRMA using R package 'affy'. The normalized data can be found on the series record and contains the gcRMA normalized expression values.

Proteomic analysis of human sepsis monocytes

Project description:In this project we performed a comprehensive exploration of monocyte molecular responses in a cohort of patients with septic shock via label-free shotgun proteomics. We enrolled adult (≥18 years old) patients with sepsis from community-acquired infections, diagnosed according to the Third International Consensus Definitions for Sepsis and Septic Shock (Sepsis-3) criteria. Blood samples were obtained within the first 72 hours from the diagnosis of sepsis (sepsis phase) and on de day before ICU discharge (recovery phase). The Control group consisted of age matched healthy volunteers. We excluded subjects with AIDS, advanced cancer, hematological diseases, and pregnancy.

SARS-CoV-2 RNAemia and proteomic biomarker trajectory inform prognostication in COVID-19 patients admitted to intensive care

Project description:Prognostic characteristics inform risk stratification in intensive care unit (ICU) patients with coronavirus disease 2019 (COVID-19). We obtained blood samples (n = 474) from hospitalized COVID-19 patients (n = 123), non-COVID-19 ICU sepsis patients (n = 25) and healthy controls (n = 30). Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) RNA was detected in plasma or serum (RNAemia) of COVID-19 ICU patients when neutralizing antibody response was low. RNAemia was associated with higher 28-day ICU mortality (hazard ratio [HR], 1.84 [95% CI, 1.22–2.77] adjusted for age and sex). In longitudinal comparisons, COVID-19 ICU patients had a distinct proteomic trajectory associated with RNAemia and mortality. Among COVID-19-enriched proteins, galectin-3 binding protein (LGALS3BP) and proteins of the complement system were identified as interaction partners of SARS-CoV-2 spike glycoprotein. Finally, machine learning identified ‘Age, RNAemia’ and ‘Age, pentraxin-3 (PTX3)’ as the best binary signatures associated with 28-day ICU mortality.

Transcriptomic analysis reveals abnormal repair and remodeling in survivors of critical illness with sustained muscle weakness

Project description:ICU acquired weakness (ICUAW) is a complication of critical illness characterized by structural and functional impairment of skeletal muscle that may persist for years after ICU discharge with many survivors developing protracted courses with few regaining functional independence. Elucidating molecular mechanisms underscoring sustained ICUAW is crucial to understanding outcomes linked to different morbidity trajectories as well as for the development of novel therapies. Quadriceps muscle biopsies and functional measures of muscle strength and mass were obtained at 7 days and 6 months post-ICU discharge from a cohort of ICUAW patients. Unsupervised co-expression network analysis of transcriptomic profiles identified discrete modules of co-expressed genes associated with the degree of muscle weakness and atrophy in early and sustained ICUAW. Modules were enriched for genes involved in skeletal muscle regeneration and extracellular matrix deposition. Collagen deposition in persistent ICUAW was confirmed by histochemical stain. Modules were further validated in an independent cohort of critically ill patients with sepsis-induced multi-organ failure and a porcine model of ICUAW, demonstrating disease-associated conservation across species and peripheral muscle type. Our findings provide a pathomolecular basis for sustained ICUAW, implicating aberrant expression of distinct skeletal muscle structural and regenerative genes in early and persistent ICUAW. Proteomic analysis identified age-associated prognostic biomarkers in pneumonia-derived pediatric sepsis

Project description:Pediatric sepsis is a leading cause of mortality in children across the world. Within pediatric sepsis, how developmental age-specific host immune response impact on the occurrence and development of pediatric sepsis is still unknown, especially between infants and toddlers, which were the major susceptible age groups in sepsis. Experimental design In this study, we applied a nested case-control study strategy and analyzed the plasma proteomes of pediatric sepsis patients between infants and toddlers in comparison to their age-matched controls respectively. Each age group consists of three subgroups with different outcomes. One hundred and ten plasma samples were pooled into 16 samples for quantitative identification by LC-MS/MS. Results It was totally quantified 677 proteins. In comparison to toddlers, infant sepsis patients were characteristic with dominant neutrophil-mediated defense, suppressed adaptive immunity and NK-mediated cytotoxicity. Besides, pentose phosphate pathway was more up-regulated in infants and associated with poor outcome. Moreover, combined Hp and Thbs1 with the AUC value of 0.958 (95%CI, 0.868, 1.000) was confirmed as potential infant-adapted prognostic marker of poor outcome in sepsis. Conclusion and clinical relevance Our proteomic analysis of age-associated pediatric sepsis combined with clinical laboratory data provided a comprehensive insight into the complex, multifactorial heterogeneous host response to pediatric sepsis and allowed identification of critical differences between infant and toddler sepsis. Moreover, we confirmed that combined Hp and Thbs1 is more infant-adapted and serves as a potential prognostic biomarker for poor outcome of infant sepsis, promoting the application of age-adapted precision medicine in pediatric sepsis.
