Spatial immune landscape of large artery atherosclerotic and cardioembolic thrombus retrieved from mechanical thrombectomy

ABSTRACT: Cardioembolic (CE) and large artery atherosclerosis (LAA) strokes are two main causes of acute ischemic stroke (AIS), carrying a high risk of recurrence. This study investigated the immune cell-specific role in thrombosis between CE and LAA stroke using NanoString GeoMX Digital Spatial Profiler.

Twenty-three control samples and sixty-nine cardioembolic stroke samples were assayed. Twenty-three control samples and sixty-nine cardioembolic stroke samples were assayed. Cardioembolic stroke subjects were analyzed at three time points: less than 3 hours, 5 hours, and 24 hours following the event.

A Comparative Serum Proteomic Analysis in Large-Artery Atherosclerosis Patients Progressing to Acute Ischemic Stroke: Discovery of Predictive Biomarker Panels

Project description:To predict acute ischemic stroke (AIS) in large-artery atherosclerosis (LAA), we combined serum proteomics with machine learning to identify diagnostic biomarkers. Using a dual-cohort design, we analyzed proteins and validated targets through advanced proteomic techniques. Machine learning identified two protein panels: one distinguishing AIS from LAA and another differentiating AIS/LAA from healthy controls. These panels capture dynamic pathophysiological processes, offering scalable tools for stroke prediction in high-risk LAA populations, surpassing conventional anatomical assessments.

Thrombus proteomics reveals new candidate biomarkers of stroke etiology

Project description:In this study, we have profiled the protein content of intravascular thrombotic material from stroke patients in the acute stage by quantitative proteomics. To this end, thrombotic material obtained by intra-arterial thrombolysis from stoke patients with different etiologies by TOAST classification (atherotrombotic and cardioembolic) was processed and further underwent proteomics analysis. As a result, we report a number of novel protein biomarkers that may help in differentiating atherotrombotic from cardioembolic stroke

Project description:Accurate ischemic stroke (IS) etiologic diagnosis provides an appropriate secondary prevention strategy and better outcome. Currently, 45% of IS are cryptogenic urging to find novel approaches to enhance diagnostic precision and secondary stroke prevention. OBJETIVE: To study the transcriptomic content of thrombus extracellular vesicles (EVs) to delineate the molecular profile and potential biomarkers of IS etiologies. RESULTS:30 consecutive patients with moderate or severe ischemic stroke (IS) and different IS etiologies were included to perform the transcriptomic analysis of thrombus EVs comprising: atherothrombotic (AT, n=10), Cardioembolic (CE, n=10), and embolic stroke of undetermined source (ESUS, n=10). The RNASeq analysis of EVs detected clusters of differentially expressed genes (DEGs) related to IS etiologies.

Hsp70 protects from stroke in atrial fibrillation patients by preventing thrombosis with no increased bleeding risk

Project description:Atrial fibrillation (AF) is a major risk factor for cardioembolic stroke. Anticoagulant drugs are effective in preventing AF-related stroke. However, the high frequency of anticoagulant-associated major bleeding is a major concern particularly when antiplatelet treatment is simultaneously administered. Here, microarray analysis in peripheral blood cells in eight patients with AF and stroke and eight AF subjects without stroke identified a stroke related gene expression pattern. HSPA1B, which encodes for heat-shock protein 70 kDa (Hsp70), was the most differentially expressed gene. This gene was downregulated in stroke subjects, a finding confirmed further in an independent AF cohort of 200 individuals. Hsp70 knock-out (KO) mice subjected to different thrombotic challenges developed thrombosis significantly earlier than their wild-type (WT) counterparts. In WT mice, Hsp70 inducers (TRC051384, or tubastatin A) delayed thrombus formation. Remarkably, Hsp70 inducers did not increase the bleeding risk even when aspirin was concomitantly administered. Hsp70 induction was associated with an increased vascular thrombomodulin expression, higher circulating levels of activated protein C (APC) upon thrombotic stimulus and increased protection against endothelial apoptosis. Thus, Hsp70 induction is a novel approach to delay thrombus formation with minimal bleeding risk, being especially promising in situations where there is a major bleeding hazard. CircRNA expression profiles in peripheral blood mononuclear cells of patients with large-artery atherosclerotic stroke

Project description:Circular RNAs (circRNAs) are a recently discovered non-coding RNA isoform capable of regulating neurological disease incidence. The present study was designed to characterize the circRNA expression profiles present in large-artery atherosclerosis (LAA)-type acute ischemic stroke patients. Using a RNA-seq approach, we characterized circRNA expression profiles in 5 LAA-stroke patients and 4 controls. We identified 182 and 176 circRNAs that were up- and down-regulated, respectively, in LAA-stroke patients relative to controls. Enrichment analyses suggested these differentially expressed circRNAs to primarily be associated with chromatin modification, autophagy, platelet activation, and neural precursor cell proliferation.
