Trimodulin and pneumonia

ABSTRACT: This RNAseq dataset evaluates gene expression to assess the therapeutic potential of trimodulin in a complex mouse model mimicking critically ill CAP patients infected with S. pneumoniae. In addition, we combined trimodulin with ampicillin to address if trimodulin has further added benefit in addition to reducing bacterial burden.

In addition before vs 7 days post influenza vaccination volunteer samples are assayed. 3 groups of samples. First is bacterial pneumonia patients with 6 subjects sampled for up to 5 days. Second group is severe influenza infection with 4 subjects sampled for up to 5 days. Third group is influenza vaccination with 18 subjects sampled before and 7 days post vaccination.

Proteomic Profiling of Neutrophils and Plasma in Community-Acquired Pneumonia Reveals Crucial Proteins in Diverse Biological Pathways Linked to Clinical Outcome

Project description:Neutrophils are crucial first responders in respiratory infections and play a dichotomous role in the host response during community-acquired pneumonia (CAP): providing protection and potentially causing damage. Existing research on neutrophil function in CAP relies on animal or ex-vivo studies, leaving a gap in human-centered investigations. Addressing this, we used mass spectrometry to characterize the neutrophil proteome of CAP patients at general ward admission and related the proteome to controls and clinical outcomes. We included 57 CAP patients and 26 controls and quantified 3482 proteins in neutrophil lysates and 386 proteins in concurrently collected plasma. We found that the extensively studied granule-related proteins in non-human models did not drive the neutrophil proteome changes associated with CAP. Proteome alterations were primarily due to an increased abundance of proteins related to (aerobic) metabolic activity and (m)RNA translation/processing, concurrent with a diminished presence of cytoskeletal organization-related proteins (all pathways p<0.001). Higher and lower abundances of specific proteins, primarily constituents of these pathways, were associated with prolonged time to clinical stability in CAP. Moreover, we identified a pronounced presence of platelet-related proteins in neutrophil lysates of particularly viral CAP patients, suggesting the existence of neutrophil-platelet complexes in non-critically ill CAP patients. Of the measured proteins in neutrophils, 4.3% were also detected in plasma. Our findings offer novel insights into the neutrophil proteome in human CAP and might serve as a proteomic resource for forthcoming investigations. Additionally, our study could pave the way for targeted strategies to fine-tune neutrophil responses, potentially improving CAP patient management.

Dysregulation of Streptococcus pneumoniae zinc homeostasis breaks ampicillin resistance in a pneumonia infection model

Project description:Streptococcus pneumoniae is the primary cause of community-acquired bacterial pneumonia with rates of penicillin and multi-drug resistance exceeding 80% and 40%, respectively. The innate immune response generates a variety of antimicrobial agents to control infection including zinc stress. Here, we characterized the impact of zinc intoxication on S. pneumoniae, revealing disruptions in central carbon metabolism, lipid biogenesis and peptidoglycan biosynthesis. Characterization of the pivotal peptidoglycan biosynthetic enzyme GlmU revealed an exquisite sensitivity to zinc inhibition. Disruption of the sole zinc efflux pathway, czcD, rendered S. pneumoniae highly susceptible to β-lactam antibiotics. To dysregulate zinc homeostasis in the wild-type strain, we investigated the safe-for-human use ionophore PBT2. PBT2 rendered wild-type S. pneumoniae strains sensitive to a range of antibiotics. Using an invasive ampicillin-resistant strain, we demonstrate in a murine pneumonia infection model the efficacy of PBT2+ampicillin treatment. These findings present a therapeutic modality to break resistance of drug-resistant S. pneumoniae.

Dysregulation of Streptococcus pneumoniae zinc homeostasis breaks ampicillin resistance in a pneumonia infection model

Project description:This study reports on the co-administration of a zinc ionophore (PBT-2) and ampicillin to break antibiotic resistance of Streptococcus pneumoniae in a murine pneumonia model. The molecular mechanism for this heightened antimicrobial activity was identified through transcriptomic and metabolomic analyses of a wild type strain and a zinc efflux mutant to identify cellular targets of zinc intoxication. This revealed that zinc intoxication induces numerous cellular disruptions, which when combined with frontline antibiotics, can break antibiotic resistance and potentially preclude further resistance from emerging.

BIOLOGICAL EFFECTS OF CORTICOSTEROIDS ON PNEUMOCOCCAL PNEUMONIA IN MICE AND HUMANS

Project description:Rationale: Streptococcus pneumoniae is the most common bacterial cause of community acquired pneumonia. Some clinical trials have demonstrated a beneficial effect of corticosteroid therapy in community acquired pneumonia, but the mechanisms of this benefit remain unclear. Objectives: To investigate the biologic effects of corticosteroids in pneumococcal pneumonia in mice and in patients Methods: We studied lower respiratory tract transcriptomes from an observational cohort of mechanically ventilated patients and from a pneumonia model in mice. We also carried out comprehensive physiologic, biochemical, and histological analyses in mice to identify mechanisms of lung injury in S. pneumoniae with and without adjunctive steroid therapy. Measurement and Main Results: Transcriptomic analysis identified pleiotropic effects of steroid therapy on the lower respiratory tract in critically ill patients with pneumococcal pneumonia, findings that were reproducible in mice. In mice with pneumonia, dexamethasone in combination with ceftriaxone reduced (1) pulmonary edema formation, (2) alveolar protein permeability, (3) proinflammatory cytokine release, (4) histopathology lung injury score, and (5) hypoxemia, but did not increase bacterial burden. Conclusions: In combination with appropriate antibiotics in mice, treatment of pneumococcal pneumonia with steroid therapy reduces hypoxemia, pulmonary edema, lung permeability, and histologic criteria of lung injury, and also altered inflammatory responses at the protein and gene expression level. The concordance of transcriptional data in the mouse model and in patients with pneumococcal pneumonia supports the translational relevance of this work.

Transcription profiling of pig postischaemic restituted intestinal mucosa after further ischaemic insult

Project description:Background: Restitution of the surface epithelium is a critical healing process early after ischaemic injury of the gut mucosa. Repeated episodes of intestinal ischaemia occur in many critically ill patients. This study evaluates the resistance of postischaemic restituted intestinal mucosa to a further ischaemic insult. Methods: The superior mesenteric artery (SMA) in pigs was cross-clamped for 1 hour, reperfused for 4 hours, cross-clamped once again for 1 hour, and finally reperfused for 3 hours. Intestinal injury was evaluated with morphometry. Intestinal permeability (from blood to lumen was assessed by transport of fluorescein isothiocyanate dextran (FD-4). Microarray and QRT-PCR analysis were performed.
