Candida albicans hyphae induce lung epithelial cytotoxicity and lung barrier disruption in mice and humans [in vitro]


ABSTRACT: The relationship between Candida albicans recovery from the lower respiratory tract (LRT) of mechanically-ventilated patients and adverse clinical outcomes remains uncertain. We utilized mouse models to determine whether C. albicans mediates lung injury. Intratracheal C. albicans potentiated lipopolysaccharide-mediated lung barrier disruption. Notably, in both wild-type and neutrophil-depleted mice, intratracheal C. albicans alone was sufficient to induce lung injury. In lung epithelial cell culture, C. albicans-induced cytotoxicity and barrier disruption were attenuated with a yeast-locked strain unable to form hyphae. Mice intratracheally inoculated with yeast-locked C. albicans showed reduced barrier disruption compared to isogenic control. In mechanically-ventilated humans with LRT Candida, we observed hyphae and elevated biomarkers of lung epithelial injury in LRT biospecimens, as well as worse clinical endpoints of lung injury. Therefore, our study suggests that C. albicans hyphae can directly cause lung epithelial injury and barrier disruption and highlights a critical area for future research.
