Obstetric complications in pregnant people with Fontan Circulation: identifying candidate pathologic pathways in the placenta

ABSTRACT: Pregnant people with Fontan circulation have high rates of fetal growth restriction (FGR) and preterm birth. The mechanisms underlying these complications are poorly understood. We performed targeted sampling of FGR and non-FGR placentas from pregnancies in individuals with Fontan circulation to characterize the differential expression across pathology-dense regions in these two conditions.

Comparative analysis of miRNA profile in sheep serum and serum-derived extracellular vesicles before and after single stage Fontan surgery

Project description:There are no curative treatment options for patients with single ventricle disease. Rather, the standard of care includes a series of palliative procedures that culminate in the Fontan operation, which, while life-saving, nevertheless reduces longevity and quality of life. The cause of late term death in these patients is typically ascribed to insidious end organ sequelae associated with the Fontan circulation. The etiology underlying these pathological processes remains unknown, but is thought to be diverse and multifactorial, arising from progressive deterioration of end-organ function due to the non-physiologic Fontan hemodynamics and its consequences on tissue perfusion and remodeling.Our overall goal is to evaluate the impact of Fontan physiology on cardiopulmonary remodeling over long periods in an ovine Fontan survival model.

Project description:It has been well documented that pre-eclampsia and unexplained fetal growth restriction (FGR) has a common etiological background, but little is known about the linkage al the molecular level. we have performed global gene expression profiling by oligonucleotide microarrays for placentas from pre-eclamptic, unexplained FGR and normal pregnancies to further elucidate the mechanisms underlying the development of these two disorders. The total number of samples used was 8 from pre-eclampsia, 8 from normotensive pregnancies with FGR and 8 from those without FGR.

Transcriptomic Analysis of Circulating Extracellular Vesicles and Serum during the Perioperative period of Fontan and Glenn Surgery

Project description:Single-ventricle defects are palliatively corrected using the Glenn and Fontan procedures that provide lifesaving relief but lead to early mortality rates and poor prognosis post-operatively. The pathophysiology of these complications is diverse, and there are no clear predictors for these outcomes. As such, we aimed to identify novel biomarkers for single-ventricle associated outcomes through the characterization of the serum transcriptome and small extracellular vesicles (sEVs). We report on molecular responses in serum and sEVs pre-operatively and post-operatively in patients undergoing either Glenn or Fontan operations. We found that transcriptomes of serum RNA and sEV RNA are distinctly different, whereas differences between the Fontan and Glenn transcriptomes did not show any significant changes. An in vitro wound healing assay further confirmed that post-operative sEVs have higher angiogenic potential. Moreover, we used a partial-least square regression model to identify 100 RNAs and 50 microRNAs that are involved in angiogenesis-related pathways and accurately predict our wound healing results and compared them with our previously published models of ovine Fontan surgery. Lastly, an EV-origin analysis showed decreased secretion of sEV from cardiac tissue in post-operative samples and increased secretion of sEVs from stomach and nervous tissue. Overall, this study provided evidence that the circulating sEVs transcriptome are potent biomarkers and demonstrated the role of sEVs as a possible mediator for formation of veno-venous collaterals.

Transcription profiling of decidua basalis from women with normal pregnancies or preeclampsia and/or fetal growth restriction

Project description:The aim of the study was to compare gene expression profiles in decidua basalis from cases with complicated pregnancies to those from healthy pregnant controls. Cases included pregnant women with preeclampsia (PE) and/or fetal growth restriction (FGR). Decidual tissue was obtained by vacuum suction of the placental bed after the placenta was delivered. Women with PE and/or FGR were included as cases. PE was defined as persistent hypertension (blood pressure (BP) ¡Ý 140/90 mm Hg), plus proteinuria (¡Ý0.3g/24 h or ¡Ý2+ according to a dipstick test), developing after 20 weeks of pregnancy. FGR implied birth weight under 2 standard deviations (SD) below the expected birth weight, as related to gestational age (GA) and sex. Due to tissue sampling procedures, only cases delivered by cesarean section (CS) were included. Healthy women with normal pregnancies, undergoing CS for various reasons considered irrelevant to the aim of this study (e. g. breech presentation and previous CS), served as controls. Tissue was immediately submerged in a RNA stabilisation solution (RNAlater, Ambion, Huntingdon, U.K.), incubated at 4¡ãC overnight and stored at -80¡ãC

Genome-wide DNA methylation analysis of placentas from pregnancies complicated by early-onset fetal growth restriction

Project description:Fetal growth restriction (FGR) affects approximately 5–10% of pregnancies and remains a leading cause of perinatal morbidity and mortality. The condition is characterized by the failure of the fetus to achieve its genetically determined growth potential, most commonly as a consequence of placental dysfunction. Despite extensive research, the molecular mechanisms underlying FGR remain incompletely understood. In this study, we investigated whether alterations in placental DNA methylation are associated with early-onset FGR and may contribute to its pathophysiology.

PROTEOMIC ANALYSIS OF THE UMBILICAL CORD IN FETAL GROWTH RESTRICTION AND PREECLAMPSIA

Project description:Fetal growth restriction (FGR) is associated with adverse perinatal outcomes. Pre-eclampsia (PreE) increases the associated perinatal morbidity and mortality. The structure of the umbilical cord in the setting of FGR and PreE has yet to be determined. This study aimed to examine changes in the umbilical cord (UC) composition in pregnancies complicated by FGR and FGR with PreE. UC from gestational age-matched pregnancies with isolated FGR (n = 5), FGR with PreE (n = 5) and controls (n = 5) were collected, and a portion of the UC was processed for histologic and proteomic analysis. Manual segmentation analysis was performed to measure cross-section areas of umbilical cord regions. Wharton's Jelly samples were analyzed on a tims-TOF Pro. Spectral count and ion abundance data were analyzed simultaneously, creating an intersection dataset from a combination of multiple mass spectrometry search and inference engines. UCs from FGR and FGR with PreE had lower cross-sectional area and Wharton's Jelly area compared with control (p &#61 ;0.03). When comparing isolated FGR to control, 28 proteins were significantly different in abundance analysis and 34 in spectral count analysis (p-value < 0.05). Differential expression analysis between PreE with FGR vs controls, demonstrated 48 proteins were significantly different in abundance and 5 in spectral count (p-value < 0.05). The majority of proteomic changes occur in proteins associated with extracellular matrix, cellular process, inflammatory, and angiogenesis pathways.
